LUTS in Older Adults: Definitions, Comorbidity Impact, Patient Priorities, and Treatment Strategies for Managing Daytime and/or Nighttime Symptoms - ICI-RS 2024.

INTRODUCTION: Lower urinary tract symptoms (LUTS) are prevalent among older adults, a population that faces multiple medical challenges, with frailty being a major concern. Despite the high prevalence of LUTS, current treatment strategies for older adults are often inadequate. This paper aims to address these issues by defining daytime and/or nighttime LUTS and examining the impact of comorbid conditions on these symptoms. We will identify the priorities of older patients regarding LUTS management and propose treatment strategies to improve outcomes in this vulnerable population. METHODS: Thi

Examining the psychometric properties of brief screening measures of depression and anxiety in chronic pain: The Patient Health Questionnaire 2-item and Generalized Anxiety Disorder 2-item.

OBJECTIVE: Individuals with chronic pain experience anxiety and depressive symptoms at rates higher than the general population. The Patient Health Questionnaire 2-item (PHQ-2) and Generalized Anxiety Disorder 2-item (GAD-2) are brief screening measures of depression and anxiety, respectively. These brief scales are well-suited for use in routine care due to their brevity and ease of administration, yet their psychometric properties have not been established in heterogeneous chronic pain samples when administered over the Internet. MATERIALS AND METHODS: Using existing data from randomized co

Healthcare Resource Utilization for Chronic Rhinosinusitis in Older Adults.

OBJECTIVES: Chronic rhinosinusitis (CRS) is a common and costly health problem in the United States. While often associated with younger adults, CRS can affect the elderly. As the aging population increases in the United States, the cost burden of CRS in older adults is important to assess. The objective of this study is to characterize healthcare resource utilization (HCRU) and healthcare expenditure (HCE) for CRS in this population. METHODS: Patients meeting criteria for CRS with three years of continuous data were identified on IBM® Marketscan Research Databases over a five-year period (20

Risk of Surgery and Mortality in Elderly-onset Inflammatory Bowel Disease: A Population-based Cohort Study.

BACKGROUND: As the prevalence of inflammatory bowel disease (IBD) increases in the elderly population, we sought to characterize IBD-related outcomes in this population. METHODS: We identified incident IBD cases in Ontario, Canada between 1999 and 2008 and categorized subjects by age at diagnosis as young adults (18-40 yr); middle-age adults (41-64 yr); and elderly (≥65 yr) from within population-based health administrative data. We determined the risk of IBD-related surgery and mortality in those with elderly-onset IBD compared with other age groups. RESULTS: Of 21,218 persons with IBD, the

Type 2 Diabetes: Why We Are Winning the Battle but Losing the War? 2015 Kelly West Award Lecture.

Diabetes is among the biggest of the 21st-century global health challenges. In the U.S. and other high-income countries, thanks to investments in science, dedication to implementing these findings, and measurement of quality of care, there have been improvements in diabetes management and declines in rate of diabetes complications and mortality. This good news, however, is overshadowed by the ever-increasing absolute numbers of people with diabetes and its complications and the unprecedented growth of diabetes in low- and middle-income countries of the world. To comprehensively win the war aga

Bowel obstructions and incisional hernias following trauma laparotomy and the nonoperative therapy of solid organ injuries: A retrospective population-based analysis.

BACKGROUND: Small bowel obstruction (SBO) and incisional hernia (IH) represent the most common long-term complications of laparotomy. They may also be more common among injured patients than for elective/nontrauma emergency scenarios. Unfortunately, the population-based incidence of SBO and IH following trauma laparotomy is unknown. The aim of this study was to define the long-term, population-based incidence of SBO and IH following both trauma laparotomy as well as the nonoperative therapy of solid organ injuries. METHODS: All injured patients admitted to a Level 1 trauma center (2002-2013)

Survey of domestic accidents in the elderly in the Province of Genoa (northern Italy).

INTRODUCTION: Accidents in the home are a major public health issue in most industrialised countries, as they are a frequent cause of injury and death. Moreover, since a considerable portion of such accidents involve elderly people, it is important to assess their social impact in this population. In Italy, the available data indicate that well over 3 million people per year suffer accidents in the home, and that this number is rising. The aim of the present study was to evaluate the number, characteristics and causes of domestic accidents among the elderly. MATERIALS AND METHODS: The study p

The Emerging Roles of AI in Self-Directed Stress Management: Systematic Review.

BACKGROUND: Stress is widespread and carries substantial mental health, social, and economic burdens. Yet, access to clinician-led stress management remains constrained by service capacity, cost, and stigma. In response, artificial intelligence (AI)-enabled tools have rapidly proliferated as scalable, self-directed options. However, evidence on how these systems support stress management outside formal clinical settings remains fragmented. OBJECTIVE: This systematic review aimed to synthesize empirical evidence on how AI-enabled technologies are used for self-directed stress management. We ma

Negative effects of semaglutide on bone in obese mice.

GLP1 receptor agonists, such as semaglutide, are used in the management of obesity. While causing weight loss, these drugs reduce both fat and lean mass. The impact of GLP1 receptor agonists on bone health has not been explored. Femur mass, stiffness, and strength are lower in obese male C57BL/6J mice treated with semaglutide, and these effects are more pronounced than in mice that are calorically restricted to the same extent. Decrements in bone are paralleled by lower circulating levels of markers of osteoblast activity. In an additional cohort, femur weight, strength, and stiffness are the
