A qualitative exploration of contraceptive use and discontinuation among women with an unmet need for modern contraception in Kenya.

BACKGROUND: Addressing the unmet need for modern contraception underpins the goal of all family planning and contraception programs. Contraceptive discontinuation among those in need of a method hinders the attainment of the fertility desires of women, which may result in unintended pregnancies. This paper presents experiences of contraceptive use, reasons for discontinuation, and future intentions to use modern contraceptives. METHODS: Qualitative data were collected in two rural counties in Kenya in 2019 from women with unmet need for contraception who were former modern contraceptive users

Integration of deworming into primary healthcare in Western Kenya: a quasi-experimental evaluation.

INTRODUCTION: The WHO 2030 NTD roadmap calls for integrating mass drug administration (MDA) into primary healthcare (PHC) systems. A coverage evaluation survey conducted in Western Kenya exposed the persistent limitations of mass drug administration campaigns, showing that nearly 25% of surveyed individuals never received treatment. The effect of stock-outs and ineffective mobilizations were also noted. To consolidate the gains from previous mass drug administrations (MDAs), integration of MDA into Primary Health Care (PHC) services was piloted through the Deworming Innovation Fund by leveragi

Ruptured uterus in rural Uganda: prevalence, predisposing factors and outcomes.

INTRODUCTION: A ruptured uterus is a life-threatening obstetric complication that remains a major public health concern in low-income countries, particularly in Africa. It is a significant cause of maternal and perinatal morbidity and mortality. In Uganda, the prevalence remains high largely because most women do not deliver in health facilities. Further review of this problem may be helpful in the development of appropriate preventive strategies. METHODS: A five-year retrospective review of all cases of ruptured uterus admitted to Mityana Hospital, Uganda from January 1, 2003 to December 31,

Social support and cancer screening among older black Americans.

BACKGROUND: Age-adjusted cancer mortality is 27% higher for Black Americans than for the general U.S. population, which may result from inappropriate use of cancer detection tests. Social support has been shown to affect adjustment to breast cancer and survival, but it has not been studied as a predictor of use of preventive health care services in the older population. Our hypothesis is that larger social networks are associated with greater utilization of cancer-screening tests in the older population. PURPOSE: The objective of this study was to examine the relationship between social suppo

Ryan White CARE Act Title IV programs: a preliminary characterization of benefits and costs.

Congress enacted the Ryan White Comprehensive AIDS Resources Emergency (CARE) Act in 1990 to address the unmet health needs of persons living with HIV (PLWH) by funding primary healthcare and support services to enhance access to and retention in care. The CARE Act was amended and reauthorized in 1996 and in 2000, and again in December 2006. As originally enacted, the CARE Act was a compromise across a wide political divide. A structure was established that distributed Ryan White CARE Act (RWCA) funds through five federal titles, with different parameters set for each title. Some funds were pl

Venezuela's public health crisis: a regional emergency.

The economic crisis in Venezuela has eroded the country's health-care infrastructure and threatened the public health of its people. Shortages in medications, health supplies, interruptions of basic utilities at health-care facilities, and the emigration of health-care workers have led to a progressive decline in the operational capacity of health care. The effect of the crisis on public health has been difficult to quantify since the Venezuelan Ministry of Health stopped publishing crucial public health statistics in 2016. We prepared a synthesis of health information, beyond what is availabl

Effects of Interventions to Prevent Work-Related Asthma, Allergy, and Other Hypersensitivity Reactions in Norwegian Salmon Industry Workers (SHInE): Protocol for a Pragmatic Allocated Intervention Trial and Related Substudies.

BACKGROUND: Workers in the salmon processing industry have an increased risk of developing respiratory diseases and other hypersensitivity responses due to occupational exposure to bioaerosols containing fish proteins and microorganisms, and related allergens. Little is known about effective measures to reduce bioaerosol exposure and about the extent of skin complaints among workers. In addition, while identification of risk factors is a core activity in disease prevention strategies, there is increasing interest in health-promoting factors, which is an understudied area in the salmon processi

A Qualitative Study on Parental Experiences with Genetic Counseling After a Positive Newborn Screen for Recently Added Conditions on the Recommended Uniform Screening Panel (RUSP).

The goal of newborn screening (NBS) has remained the same despite its significant expansion from its inception as a public health initiative. This goal is to identify infants that are at risk for a set list of conditions and to implement a care plan to prevent, delay, or mitigate adverse health outcomes for those affected. The role of genetic counselors (GCs) in the NBS space is currently evolving, and there is limited research on parental experiences with genetic counseling for more recently added conditions on a list approved by the U.S. Secretary of Health and Human Services called the Reco

The Hypocritical Oath? Unintended Consequences of Prenatal Substance Use Policies and Considerations for Health Care Providers.

Prenatal substance use (PSU) is a serious perinatal health issue in the United States with consequential health effects. To address this issue and protect children from the detrimental effects of substance exposure during pregnancy, the US government amended the Child Abuse Prevention and Treatment Act to provide funding to states with protocol to notify child protective services of PSU cases and develop treatment plans for affected families. Although well-intentioned, this statute resulted in diverse inter- and intrastate interpretations and implementation of PSU regulations nationwide, ultim

Cholera: An Overview with Reference to the Syrian Outbreak.

Cholera is an acute type of diarrheal disease caused by intestinal infection with the toxin-producing bacteria Vibrio cholerae. The disease is still endemic in almost 69 countries, accounting for around 2.86 million cases and 95,000 deaths annually. Cholera is associated with poor infrastructure, and lack of access to sanitation and clean drinking water. The current cholera outbreak in Syria is associated with more than 10 years of conflict, which has devastated infrastructures and health services. There were 132,782 suspected cases reported between August 25, 2022 and May 20, 2023 in all 14 g

Alcohol Screening, Brief Intervention, and Referral to Treatment (SBIRT) for Girls and Women.

Females ages 12 and older are the fastest growing segment of alcohol consumers in the United States, with the past decade showing a 16% increase in alcohol use per 12-month period and a 58% increase in high-risk drinking (i.e., > 3 drinks in a day and/or > 7 drinks in a week) per 12-month period. The increase in alcohol use and risk drinking poses unique and serious consequences for women. Women have a more rapid progression to alcohol-related problems and alcohol use disorders (AUD) than men, and if pregnant, women can potentially expose the fetus to alcohol. Screening, brief intervention, an

Psychosocial functioning of individuals with schizophrenia in community housing facilities and the psychiatric hospital in Zurich.

Individuals with severe mental illness frequently have difficulties in obtaining and maintaining adequate accommodation. If they are not willing or able to adapt to requirements of traditional supported housing institutions they may live in sheltered and emergency accommodation. Adequate mental health services are rarely available in these facilities. The aim of the present study was to evaluate mental health, functional and social status of individuals living in community sheltered housing facilities. A cross-sectional survey of n=338 individuals in sheltered housing compared to a sample of p

[Mass gatherings - health risks and preventive strategies].

Experience from mass gatherings - usually attended by at least 25'000 persons - shows that approximately one in a thousand participants will consult with an on-site medical emergency service. Communicable diseases usually play a minor role. Historically outbreaks of meningococcal disease were recorded after the hajj, but this has been well controlled in the past few years subsequent to vaccinations and other measures required by the Kingdom of Saudi Arabia health authorities. Major stress of the regional public health system is associated with accidents and non-communicable diseases, the major

Adverse childhood experiences reported by adults --- five states, 2009.

Adverse childhood experiences (ACEs) include verbal, physical, or sexual abuse, as well as family dysfunction (e.g., an incarcerated, mentally ill, or substance-abusing family member; domestic violence; or absence of a parent because of divorce or separation). ACEs have been linked to a range of adverse health outcomes in adulthood, including substance abuse, depression, cardiovascular disease, diabetes, cancer, and premature mortality. Furthermore, data collected from a large sample of health maintenance organization members indicated that a history of ACEs is common among adults and ACEs are

[Smoking and passive exposure to tobacco smoke among members of medical rescue teams].

In Poland approximately 33% adults are active smokers. Both active smoking and passive exposition to tobacco smoke leads to serious diseases and causes many other problems. This is why the decrease of smoking prevalence seems to be one of the most important targets of contemporary medicine. In this task the attitude of medical staff plays a great role (prevention, education, behaviour of a medical employee as a positive example). The research was carried out among 48 employees of Regional Station of Ambulance Service in Poznan (16 physicians, 19 medical rescuers and 13 nurses). The group compr

Emerging aspects of psychosocial risks: violence and harassment at work.

In the last twenty years, psychosocial risks have become crucial in Occupational Health. Particularly, there is an increasing interest about psychological and physical violence at the workplaces. Psychological violence (mobbing or workplace bullying) is described as a situation in which the person has been the victim of negative acts directed to the person and work, with offences, discriminations and isolation. Physical violence at work, still underestimated in many parts of the world, is becoming a topical subject both for its frequency and its pathogenic potential and consist of violence amo

Medico-legal aspects of abortion in Europe.

OBJECTIVE: The practice of abortion in a particular country reflects culture, economic status, religion and the law. Various aspects of abortion in Europe - laws, rates and practices - are presented. RESULTS: Abortion is completely prohibited in Ireland and Malta. In Poland it is permitted only to save the woman's life or protect her physical health. On the grounds of protecting the woman's mental health it is also permitted in Northern Ireland, Portugal, Spain and Switzerland. On socioeconomic grounds abortion is permitted in Finland, Great Britain and Hungary. In the other European countrie

Performing arts medicine.

Arts medicine has come of age, resulting from 3 important developments over the past decade: improved methods of diagnosis and treatment, an awareness that artists suffer from special problems related to their occupation and lifestyle, and the establishment of health programs emphasizing an interdisciplinary approach to these patients. We focus on the patterns of illness afflicting performing artists, specifically dancers, singers, actors, and instrumental musicians, and explain some of the things a health care team can do in treating these patients. The conditions governing these patients' li
