Seven cases of acute cholecystitis (4 women and 3 men, mean age 73 years) were observed in a series of 192 patients treated by endoprosthesis for cancerous obstruction of the bile ducts between october, 1984 and october, 1986. The suspected clinical diagnosis was confirmed by ultrasonography. Cholecystostomy was performed by percutaneous puncture under ultrasonic guidance. A technique is introduced for vocal cord mobilization in which a tunnel is made through the inner perichondrium of the thyroid cartilage and a medical grade silicone is placed from the upper part of the thyroid cartilage and pulled out over the cricothyroid membrane. As silicone polymer implants are used more widely for arthroplasty of the wrist, hand, and foot, the complication of silicone-induced erosive arthritis in these areas is being recognized with increasing frequency. The technique of subperiosteal implantation of multiple, small glass beads for correction of enophthalmos associated with anophthalmos was first described in 1967. Reported complications of this procedure include ptosis, anesthesia of the distribution of the supraorbital or infraorbital nerve, and migration of the implants into the orbit or sinuses. A case of orbital cellulitis has been reported. We now report a case in which the extremely serious complication of intracranial migration of glass bead implants, with subsequent cerebrospinal fluid leak, occurred 17 years postimplantation.

A successfully rehabilitated anophthalmic socket must hold and support a prosthetic device that mimics the contralateral globe. The goal is symmetry. Static symmetry of the palpebral apertures, canthal angles, and superior sulci are basic objectives. Tissue expansion offers a new alternative in plastic surgery. Its applications are numerous for reconstruction of the scalp, head and neck, trunk, breast, and extremity. Over the past year I have used tissue expansion for reconstruction in 13 patients, with multiple expanders in six patients. Complications included two exposed expanders requiring early removal, and two mechanical malfunctions requiring replacement. Expansion of previously irradiated tissue has been unsatisfactory.

A technique is described for improving catheter control in patients with neuropathic bladder who have inadequate sphincter tone to retain an indwelling catheter without leaking or to remain dry with a regime of intermittent catheterisation. In 1975, a new Silastic device was developed by the authors to be used in the staged reduction of viscera in newborns with giant gastroschisis or omphalocele. That device and their early experience were described in 1977. This article reports the results of use of this device over the past 10 years. Rectal prolapse is best treated by intra-abdominal or perineal procedures that either resect the redundant rectosigmoid colon or fix the rectum within the pelvis. Recently there has been interest in tests to assess the physiological response to surgical implant corrosion products. Both cell culturing in metal-bearing solutions and intramuscular injection of such solutions have been carried out. This paper examines the effects of the constraints of multicomponent equilibrium conditions on the characteristics of the solutions used in these biocompatibility tests. It is demonstrated that, unbuffered, they will have pH values shifted from neutral and that, in the buffered state, these solutions may contain both dissolved metal ions and insoluble hydroxides.

Three basic types of procedures are used by the author to reanimate the face paralyzed by interruption of the facial nerve at the skull base. In order of choice they are direct nerve repair or grafting; hypoglossal-facial nerve anastomosis; and regional facial reanimation (brow lift, implantation of a gold weight or spring in the eyelid, tightening of the lower eyelid, and temporalis muscle transposition to reanimate the mouth). The indications for and expected results of each approach are presented with special emphasis on the usefulness of regional reanimation.

Surgery is not always the best approach for immediate resolution of nasal airway problems. In some instances, surgery may be deferred, unnecessary, contraindicated or frankly refused by the patient. For some of these patients, the use of a simple intranasal prosthesis to enhance the passage of air through the nasal valve region is beneficial. Five patients meeting these criteria were fitted with a prosthesis and carefully evaluated pre- and postinsertion with rhinomanometry. Other situations are discussed where such a prosthetic device might be helpful.

Although the artificial genitourinary sphincter is ideally suited for incontinent patients who can empty the bladder completely, we have observed 22 patients with artificial sphincters who also perform intermittent self-catheterization to augment bladder emptying. Despite a paucity of published clinical trials, use of the Angelchik antireflux prosthesis has increased dramatically since its introduction in 1979. Serious complications of this device have begun to appear. We dealt with two cases of erosion of the prosthesis into the gastrointestinal tract, one of which represents, to our knowledge, the first reported case of a gastroenteric fistula secondary to the device.
