Right ventricular failure after repair of tetralogy of Fallot (TOF) is associated with late mortality and morbidity. We report a case of tricuspid and mitral regurgitation with severe pulmonary regurgitation (PR) diagnosed 31 years after repair of TOF. The patient, a 48-year-old woman, presented with severe ventricular arrhythmia and exercise intolerance. She was treated successfully with triple valve replacement, following which her symptoms improved dramatically. She has remained well for 2 years postoperatively.

Two hundred adults who underwent surgery for congenital heart disease at our institution within a four year period were the basis for this report. Clinical data was obtained, i.e. demographic details, past medical history, physical findings, EKGs, echocardiograms, catheterization and angiography material as well as the New York Heart Association (NYHA) class. Intraoperative findings, perioperative management, complications and morbidity and mortality were assessed. After discharge the patients were followed on an outpatient basis. Again the clinical findings, laboratory results and NYHA class

OBJECTIVES: The reported superiority of mitral valve (MV) repair for isolated MV regurgitation has not been confirmed in mitroaortic valve surgery. Our goals were to evaluate the feasibility of repair in patients undergoing mitral and aortic valve surgery and to identify factors predisposing to MV replacement, to compare long-term outcomes (survival and MV reoperation) of repair and replacement and to perform a subgroup analysis in patients with rheumatic MV disease.
METHODS: From January 1992 through December 2016, 1122 consecutive patients were submitted to concomitant aortic and MV surgery

OBJECTIVE: Functional tricuspid regurgitation (FTR) is the most common type of tricuspid insufficiency and occurs approximately in 30% of patients with mitral valve disease. The major etiologic factor in the triggering of right ventricular dilation and thus causing functional tricuspid regurgitation, is pulmonary artery hypertension secondary to mitral valve disease. We aimed to analyze long-term outcomes of patients with mild tricuspid regurgitation at the time of mitral valve replacement.
METHODS: Sixty-six patients with mild tricuspid insufficiency who underwent mitral valve replacement we

The effects of mechanical ventilation with and without positive end-expiratory pressure (PEEP) on hemodynamic performance and blood-gas exchange were studied in ten patients following open-heart surgery. Ventilation at constant tidal volume (15 ml/kg body weight) with 10 cm H2O PEEP following aortic valve replacement (AVR) IN FIVE PATIENTs without pulmonary vascular disease was associated with the following significant changes: a rise in arterial Po2, a fall in the alveolar-arterial Po2 gradient when Fio2 = 1.0, decreases in calculated Qs/Qt and cardiac index. Using a similar pattern of ventil

Only 13 cases of pulmonary arteriovenous malformation (PAVM) combined with rheumatic heart disease have been reported, and only 3 have been treated simultaneously by surgery. Transcatheter embolotherapy, the first-line treatment of PAVM, avoids major surgery, general anesthesia, and the loss of pulmonary parenchyma, but the subsequent PAVM reperfusion can occur through recanalization of the embolized arteries. We report a case of PAVM with rheumatic heart disease, in which both diseases were simultaneously treated by surgery.

Operations for left ventricular outflow tract abnormalities are centred on hemodynamic conditions that relate to subvalvar stenosis, valvar stenosis/regurgitation, aortic annular hypoplasia, and supravalvar aortic stenosis. Operative interventions over the years have evolved because the intervening outcomes proved to be unsatisfactory. The resection for subvalvar aortic stenosis has progressed from a fibrous "membrane" resection to a more extensive fibromuscular resection. Operative solutions for valvar aortic stenosis and regurgitation have resulted in operative interventions that depend on s

The child with significant valvular heart disease may present in any single instance both the most difficult of management decisions or a surprisingly simple one. Thus, if a direct surgical approach on the valve is feasible, the problems may very well be quite simple and straightforward; conversely, if valve replacement is indicated such may not be the case. We have recently reported our experience with one type of valvular heart condition, congenital aortic stenosis, in respect to determination of surgical approach based upon the ability to predict the orifice area and valvular anatomy. This

Although the characteristics and pathophysiology of major aortopulmonary collateral artery (MAPCA) have been better understood, treatment of pulmonary atresia and ventricular septal defect with MAPCA remains challenging. The difficulty in treating this disease is the lack of availability of pulmonary homografts required for pulmonary angioplasty in Japan and the need for manipulation of the posterior mediastinum region, which is not performed in normal cardiac surgery. It is important to build an appropriate surgical strategy by means of preoperative computed tomography( CT) and selective angi

A 38-year-old female with anomalous origin of the left coronary artery (LCA) from pulmonary artery was surgically corrected by tubular reconstruction of the left main coronary artery (LMCA) using the pulmonary artery wall, and this repair was performed under beating heart. Thus, the pulmonary artery was divided above the orifice level and just above the pulmonary valve, and the commissure between nonfacing and left side sinuses was dissected away from the pulmonary artery wall to obtain lateral flaps. The pulmonary artery defect was reconstructed with a roll using an autologous pericardial pat…

Patients with adult congenital heart disease (ACHD) represent a growing population at increased risk for infective endocarditis (IE). Their susceptibility is due to specific factors such as uncorrected native congenital defects, previous surgical repairs involving patches or prosthetic materials, and residual shunts or percutaneously implanted devices. These elements increase the likelihood of infection. Anatomical complexities-such as anteriorly located structures, right ventricle to pulmonary artery conduits, surgically or percutaneously implanted valves and pulmonary valve replacements-alon…

BACKGROUND: Pulmonary autograft replacement of the aortic valve is indicated in the young, in patients with an active life style, and when anticoagulation is contraindicated. Its use in patients with a dilated aortic annulus or sinotubular junction has historically had a less satisfactory result.
METHODS: To extend the advantages of the pulmonary autograft to this group of patients, we performed the Ross operation as a root replacement and "fixed" and narrowed the aortic annulus with external woven Dacron in 12 patients, Teflon felt in 5, and pericardium in 3. Twenty patients, aged 7 to 47 ye

We present a case of an emergency operation on acute MR and cardiac rupture due to a blunt thoracic trauma. This 27-year-old male suffered a severe blow to the chest when his motorcycle crashed to a tree. He was brought to this hospital by ambulance. On admission, the patient was in shock due to hypotension. Echocardiogram revealed cardiac tamponade and MR of grade 3/4. Pericardial drainage was performed immediately and his condition improved. Systolic pressure of the pulmonary artery was high at 70 mmHg which measured by Swan Ganz catheter. Isolated cardiac injury was diagnosed based on the r

The objective of this study was to characterize in detail the secondary flow velocity patterns in an in vitro model of a human (adult) pulmonary artery with varying degrees of valvular pulmonic stenosis. A two-dimensional laser Doppler anemometer (LDA) system was used to map the flow fields in the main (MPA), left (LPA), and right (RPA) branches of the pulmonary artery model. The study was conducted in the Georgia Tech right heart pulse duplicator system. A pair of counter-rotating secondary flows were observed in each daughter branch in which the fluid moved outwardly along the side walls and
