Arnold-Chiari malformation presenting as sleep apnea syndrome.

Sleep apnea syndrome is a recognized manifestation of Arnold-Chiari malformation that almost invariably has been described in association with other neurological findings. We report a 39-year-old man who presented with severe daytime hypersomnolence. A detailed neurological examination was completely normal. Polysomnography revealed moderately severe mixed central and obstructive sleep apnea and hypopnea with persistence of central apnea and hypersomnolence despite nasal continuous positive airway pressure therapy. A history of severe headaches precipitated by sustained laughter prompted the c

Validation of Polyvinylidene Fluoride Impedance Sensor for Respiratory Event Classification during Polysomnography in Children.

STUDY OBJECTIVES: Polysomnography is the gold standard for diagnosis and characterization of severity of sleep-disordered breathing. Accuracy and reliability of the technology used are critical to the integrity of the study's interpretation. Strict criteria for obstructive sleep apnea in children are lacking and diagnosis often requires consideration of frequency of respiratory events in addition to other measures. Current American Academy of Sleep Medicine recommendations for pediatric patients includes use of respiratory inductance plethysmography (RIP) belts, whereas polyvinylidene fluoride

Accuracy of Actigraphy Compared to Concomitant Ambulatory Polysomnography in Narcolepsy and Other Sleep Disorders.

Actigraphy provides longitudinal sleep data over multiple nights. It is a less expensive and less cumbersome method for measuring sleep than polysomnography. Studies assessing accuracy of actigraphy compared to ambulatory polysomnography in different sleep-disordered patients are rare. We aimed to compare the concordance between these methods in clinical setting. We included 290 clinical measurements of 281 sleep laboratory patients (mean age 37.9 years, 182 female). Concomitant ambulatory polysomnography and actigraphy were analyzed to determine the agreement in patients with obstructive slee

Home sleep apnea testing in patients with continuous flow left ventricular assist device: is it feasible?

STUDY OBJECTIVES: The incidence of sleep-related breathing disorder remains unclear in patients with a continuous flow left ventricular assist device. Polysomnography (PSG) remains the gold standard for diagnosis, but logistical challenges make home sleep apnea testing a more practical alternative. WatchPat has failed to accurately diagnose sleep-related breathing disorder in this population. In contrast, the NOX-T3 utilizes more channels to monitor physiological variables, potentially offering improved diagnostic accuracy. Therefore, we aimed to validate the NOX-T3 vs PSG in detecting sleep-r

The American Academy of Sleep Medicine Inter-scorer Reliability program: respiratory events.

STUDY OBJECTIVES: The American Academy of Sleep Medicine (AASM) Inter-scorer Reliability program provides a unique opportunity to compare a large number of scorers with varied levels of experience to determine agreement in the scoring of respiratory events. The objective of this paper is to examine areas of disagreement to inform future revisions of the AASM Manual for the Scoring of Sleep and Associated Events. METHODS: The sample included 15 monthly records, 200 epochs each. The number of scorers increased steadily during the period of data collection, reaching more than 3,600 scorers by th

Revisiting the Role of Serotonin in Sleep-Disordered Breathing.

Serotonin or 5-hydroxytryptamine (5-HT) is a ubiquitous neuro-modulator-transmitter that acts in the central nervous system, playing a major role in the control of breathing and other physiological functions. The midbrain, pons, and medulla regions contain several serotonergic nuclei with distinct physiological roles, including regulating the hypercapnic ventilatory response, upper airway patency, and sleep-wake states. Obesity is a major risk factor in the development of sleep-disordered breathing (SDB), such as obstructive sleep apnea (OSA), recurrent closure of the upper airway during sleep

Multiple Sclerosis and related disorders.

Multiple Sclerosis (MS) is a common neuroinflammatory disorder which is associated with disabling clinical consequences. The MS disease process may involve neural centers implicated in the control of breathing, leading to ventilatory disturbances during both wakefulness and sleep. In this chapter, a brief overview of MS disease mechanisms and clinical sequelae including sleep disorders is provided. The chapter then focuses on obstructive sleep apnea-hypopnea (OSAH) which is the most prevalent respiratory control abnormality encountered in ambulatory MS patients. The diagnosis, prevalence, and

Mouth opening/breathing is common in sleep apnea and linked to more nocturnal water loss.

BACKGROUND: Mouth opening/breathing during sleep is common in patients with obstructive sleep apnea (OSA), which is probably associated with more water loss and higher risk for nocturnal ischemic heart attack. This study aimed to evaluate nocturnal changes in hematocrit/hemoglobin levels and estimated plasma volume loss in OSA patients and its relation to their OSA severity and mouth open/breathing. METHODS: Sixty OSA patients and fifteen healthy controls were enrolled and underwent overnight polysomnography. Mouth status was evaluated via an infrared camera and nasal/mouth airflow. Hematocri

Severity of Desaturations Reflects OSA-Related Daytime Sleepiness Better Than AHI.

STUDY OBJECTIVES: The aim was to investigate how the severity of apneas, hypopneas, and related desaturations is associated with obstructive sleep apnea (OSA)-related daytime sleepiness. METHODS: Multiple Sleep Latency Tests and polysomnographic recordings of 362 patients with OSA were retrospectively analyzed and novel diagnostic parameters (eg, obstruction severity and desaturation severity), incorporating severity of apneas, hypopneas, and desaturations, were computed. Conventional statistical analysis and multivariate analyses were utilized to investigate connection of apnea-hypopnea inde

Diabetes and sleep disturbances: findings from the Sleep Heart Health Study.

OBJECTIVE: To test the hypothesis that diabetes is independently associated with sleep-disordered breathing (SDB), and in particular that diabetes is associated with sleep abnormalities of a central, rather than obstructive, nature. RESEARCH DESIGN AND METHODS: Using baseline data from the Sleep Heart Health Study (SHHS), we related diabetes to 1). the respiratory disturbance index (RDI; number of apneas plus hypopneas per h of sleep); 2). obstructive apnea index (OAI; >or=3 apneas/h of sleep associated with obstruction of the upper airway); 3). percent of sleep time < 90% O(2) saturation; 4)

Navigating sleep apnea in Scandinavia - a journey of contrasts.

STUDY OBJECTIVES: Obstructive sleep apnea is a prevalent condition with substantial physical, psychological, and social burdens, yet limited research explores how patients experience sleep apnea-related services in the Scandinavian context. This study aims to examine lived experiences of diagnosis, treatment, and follow-up services in Southeast Norway to identify opportunities for service improvement. METHODS: Using a Co‑Design approach, the study examined assessment and treatment services through participatory workshops and semi‑structured interviews with ten (10) participants. Reflexive the

Immediate consequences of respiratory events in sleep disordered breathing.

BACKGROUND: In obstructive sleep apnea/hypopnea syndrome, immediate physiological consequences of events have a dual role: censoring artifacts and gauging physiological significance. Newer airflow monitors may have changed the relative importance of these functions. The purpose of this study was to determine the frequency and hierarchy of occurrence of oxygen desaturation, EEG arousal and heart rate changes as immediate consequences of respiratory events. METHODS: Thirty-nine sleep apnea patients underwent polysomnography with airflow detection by nasal cannula. Eighty events/subject were ran

The English bulldog: a natural model of sleep-disordered breathing.

To establish a natural model of sleep-disordered breathing, we investigated respiration during wakefulness and sleep in the English bulldog. This breed is characterized by an abnormal upper airway anatomy, with enlargement of the soft palate and narrowing of the oropharynx. During sleep, the animals had disordered respiration and episodes of O2 desaturation. These were worst in rapid-eye-movement (REM) sleep, with most bulldogs having O2 saturations of less than 90% for prolonged durations. In contrast, control dogs never desaturated. In REM sleep, the bulldogs had episodes of both central and

[Respiratory disorders during sleep].

Sleep-disordered breathing includes snoring, upper airway resistance syndrome, sleep hypopneas and apneas, and is a borderline pathology between several disciplines (neurology, pneumology, cardiology, oto-rhino-laryngology, etc.). The common element is an abnormal increase in upper airway resistance during sleep. In mild cases, this increase accelerates airflow and induces vibrations of the pharyngeal structures (snoring); in severe cases the airway is occluded and airflow ceases (obstructive apnea). Sleep apnea syndrome (SAS) is present in 4% of males and 2% females in the general population.

Modern and multidimensional approach of sleep apneea as a public health problem.

The obstructive sleep apnea syndrome (OSAS) is a highly prevalent disorder, associated with recurring episodes of partial or complete upper airway occlusion during sleep, which results in marked sleep disturbance, excessive daytime sleepiness, irritability and morning headache, among other symptoms. Sleep apnea is strongly associated with impaired neuropsychological function and reduced quality of life and social functioning. Daytime sleepiness leads to an increased risk for traffic and industrial accidents. OSAS is also independently associated with several cardiovascular complications: not o

Canadian Thoracic Society 2011 guideline update: diagnosis and treatment of sleep disordered breathing.

The Canadian Thoracic Society (CTS) published an executive summary of guidelines for the diagnosis and treatment of sleep disordered breathing in 2006⁄2007. These guidelines were developed during several meetings by a group of experts with evidence grading based on committee consensus. These guidelines were well received and the majority of the recommendations remain unchanged. The CTS embarked on a more rigorous process for the 2011 guideline update, and addressed eight areas that were believed to be controversial or in which new data emerged. The CTS Sleep Disordered Breathing Committee pose
