Discordant refraction in male monozygotic twins.

As part of the Genes In Myopia (GEM) Study, the authors describe a pair of monozygotic twins who presented with discordant hypermetropia. Both twins also reported amblyopia, but the cause differed. The phenomenon of refractive discordance in twins is rare, with this case representing only the second to ever be reported.

Posterior polymorphous dystrophy and keratoglobus in a child.

A 13-year-old boy presented with gradually progressive deterioration of vision in both eyes, bilateral photophobia, and regular headaches. Clinical examination, anterior segment findings, and specular microscopy findings were consistent with the diagnosis of posterior polymorphous dystrophy and keratoglobus. To the authors' knowledge, this is the first pediatric case and the second case overall of the simultaneous occurrence of posterior polymorphous dystrophy and keratoglobus.

Prolonged constant eye misalignment leads to failure to develop binocular vision in childhood ocular myasthenia gravis.

PURPOSE: Variable eye misalignment and blepharoptosis in childhood ocular myasthenia gravis can lead to permanent binocular visual loss. However, a standard ophthalmologic intervention for this condition has yet to be fully established. This study investigated the influence of variable eye misalignment and asymmetric blepharoptosis on the development of binocular vision in childhood ocular myasthenia gravis. METHODS: The authors retrospectively reviewed clinical records of consecutive patients with childhood ocular myasthenia gravis whose age of onset was younger than 36 months and who had fo

Anisometropic amblyopia and nasolacrimal duct obstruction.

The authors describe five children with nasolacrimal duct obstruction who developed anisometropic amblyopia in the same eye. Because three children had no indication of vision loss at their initial presentation, cycloplegic refraction and periodic visual acuity screenings for such patients is recommended.

Spontaneous resolution of stage 4B retinopathy of prematurity.

An unfavorable outcome occurs in 31% of cases of treated severe threshold retinopathy of prematurity (ROP) and includes a retinal detachment of the posterior pole. Early treatment with laser of high risk prethreshold or type 1 ROP has significantly reduced this complication. However, despite prompt laser treatment, retinal detachments continue to be seen. Treatment of tractional partial retinal detachments involving the posterior pole (stage 4B ROP) is surgical. Successful reattachment of stage 4B ROP with a lens-sparing vitrectomy is high. The authors describe an infant who had spontaneous re

Retinopathy of prematurity in twin-twin transfusion syndrome.

The authors present the first reported case of retinopathy of prematurity in twin-twin transfusion syndrome. A 31-year-old woman gave birth to twins at 31 weeks' gestation. The pregnancy had been complicated by twin-twin transfusion syndrome and had been managed with two amnioreductions and laser ablation of the interplacental blood vessels. The donor twin, born at 1,310 g, had threshold retinopathy of prematurity and underwent argon laser retinal photocoagulation treatment. The recipient twin (1,775 g) did not show any evidence of retinopathy of prematurity.

Incidence of retinopathy of prematurity in a neonatal intensive care unit in Nepal.

PURPOSE: To determine the incidence of retinopathy of prematurity (ROP) in low birth weight premature infants admitted to the neonatal intensive care unit of a children's hospital in Kathmandu. METHODS: A hospital-based prospective study was performed in neonates with a gestational age of less than 36 weeks or birth weight of less than 2,000 g during 1 year. Ophthalmologic examination was performed 2 to 4 weeks after birth and patients were observed until the retinal vascularization was complete. ROP was classified using the revised International Classification of Retinopathy of Prematurity.

Importance of the 'third criterion' for retinopathy of prematurity screening in developing countries.

PURPOSE: Developing countries often need a modification of the American screening guidelines for retinopathy of prematurity. METHODS: A total of 1,302 eyes in 651 infants with retinopathy of prematurity were analyzed by the parameters of gestational age and birth weight suggested by the American guidelines. RESULTS: With the use of current gestational age and birth weight guidelines, 86 of 1,302 eyes (6.6%) were missed, and 38 of these eyes (2.9%) had potentially treatable or more severe retinopathy of prematurity and were in need of treatment. CONCLUSION: Use of the "third criterion" (ie,

Langerhans cell histiocytosis mimicking preseptal cellulitis.

A 7-year-old girl presented with signs of preseptal cellulitis that initially responded to antibiotics but then relapsed. Computed tomography scan revealed a cystic lesion in the preseptal tissues with associated soft tissue swelling and lacrimal gland inflammation. Anterior orbitotomy revealed a hemorrhagic-appearing lesion extending from the preseptal tissues subperiosteally along the roof of the orbit. The lesion was excised and histopathology and immunohistochemical staining confirmed a diagnosis of Langerhans histiocytosis. Management of this condition depends on the extent of systemic in

Clinical pathologic study of a cohort of patients with retinoblastoma from a developing country.

PURPOSE: To describe the histopathologic characteristics of all retinoblastoma biopsy specimens received at one institution. METHODS: All retinoblastoma biopsy specimens received between January 2000 and December 2005 were evaluated and a total of 109 eyes from 101 patients were studied. RESULTS: There were 9 (8.3%) well differentiated, 25 (23.0%) moderately differentiated, and 66 (60.5%) poorly differentiated retinoblastomas. The most common growth pattern was mixed, comprising 81 (74.4%) cases. Vitreous seeding, invasion of the anterior chamber, and invasion of the optic nerve beyond the l

Retinopathy of Prematurity and Shaken Baby Syndrome.

Retinopathy of prematurity is a vasoproliferative disease that occurs mostly in premature and low birth weight neonates. It is a potentially blinding disease that should be screened vigorously. On the other hand, shaken baby syndrome is a non-accidental trauma that is both common and avoidable. Ophthalmologists play a crucial role in diagnosing and treating the shaken infant. The authors describe a child with the combination of retinopathy of prematurity and shaken baby syndrome.

Bilateral Retinoblastoma Presenting with Unilateral Phthisis.

Phthisis of the eye is an uncommon presenting sign of retinoblastoma. The authors report a case of bilateral retinoblastoma in a 14-month-old girl who presented with phthisis of the left eye. Enucleation of the painful left eye was performed. Spontaneously regressed, mostly calcified retinoblastoma was diagnosed. The patient underwent chemoreduction. Plaque radiotherapy and transpupillary thermotherapy were applied on the right side. Subsequent ophthalmoscopic examination revealed lesions showing "cottage-cheese" regression. Retinoblastoma should be suspected in any child presenting with phthi

The safety and efficacy of glaucoma medication in the pediatric population.

Topical glaucoma medications are widely used for childhood glaucoma, although little is known concerning the use of the newer glaucoma medications in this population. The majority of the references cited were extracted from PubMed. A literature review of all English language reports related to glaucoma medication in the pediatric population since 1980 was performed. Medical therapy of pediatric glaucoma contains four groups of drugs: beta-blockers (timolol and betaxolol), carbonic anhydrase inhibitors (dorzolamide), alpha2-agonists (brimonidine), and prostaglandin analogs (latanoprost). Timolo

Abnormal ocular electrophysiology in Sjögren-Larsson syndrome.

Two siblings who were born out of consanguineous marriage in an Indian family and had typical clinical manifestations of Sjögren-Larsson syndrome were found to have normal electroretinogram and abnormal visual evoked potential results. Abnormal visual evoked potential results along with the presence of distinctive foveal and parafoveal deposits, which could have a prognostic bearing on the visual acuity of the patients, emphasize the importance of a complete ophthalmological and electrophysiological evaluation in all patients with Sjögren-Larsson syndrome.
