Improving the screening of tuberculosis in patient admitted in a secondary care hospital of La Habana.

INTRODUCTION: The reduction of tuberculosis reported in admitted patients in a community hospital in La Habana (Cuba) was identified as a quality gap and priority for action. The objective was to increase by 50% the number of bacilloscopies and smear-positive tuberculosis confirmed by December 2017. PATIENTS AND METHODS: A quality improvement initiative was conducted from January 2017 to December 2017 in a 300-bed secondary care teaching hospital. The improvement project was addressed to patients admitted with respiratory infections (upper or lower). The baseline was considered the period fro

Training in cardiac surgery using human cadavers: Effectiveness of "Silent Teachers".

BACKGROUND: Surgical skills acquisition in cardiac surgery requires consistent and hard practice. Furthermore, training using cadaver is advocated as a means of transferring learned skills to the operating room and recreate surgical situations for trainees to practice and hone their skills. We expose our experience in training for cardiac surgical procedures using human cadavers. METHODS: From June 2013 to November 2016, we performed 302 cardiac surgical procedures on 50 human cadavers obtained according to the Ivorian laws in force. Cadavers were preserved in 10% formaldehyde and by cryopres

Video analysis for the evaluation of vaginal births: a prospective observational study.

AIMS OF THE STUDY: Video documentation is nowadays well established in many fields of medicine, but mostly in unreal situations such as simulation training. We here present the application of video for teaching and learning purposes during the birth process. The aim of video documentation during labour is to provide an observational tool for obstetric care by midwives and obstetricians, with the opportunity for evaluation and education afterwards, especially in absence of the woman in labour and her family. METHODS: Between February 2015 and June 2017, we conducted a prospective observational

Academic Medical Support to the Ebola Virus Disease Outbreak in Liberia.

During the Ebola Virus Disease (EVD) epidemic in West Africa (2014-2016), many faculty, staff, and trainees from U.S. academic medical centers (i.e., teaching hospitals and their affiliated medical schools; AMCs) wished to contribute to the response to the outbreak, but many barriers prevented their participation. Here, the authors describe a successful long-term academic collaboration in Liberia that facilitated participation in the EVD response. This Perspective outlines the role the authors played in the response (providing equipment and training, supporting the return of medical education)

LATENT TUBERCULOSIS AMONG ADULT ETHIOPIAN PATIENTS AT CHEST CLINIC, TIKURANBESSA SPECIALIZED HOSPITAL, ADDIS ABABA, ETHIOPIA.

BACKGROUND: One-third of the world population is infected with Mycobacterium tuberculosis. Most people exposed to M. tuberculosis showed no evidence of active disease. About 5-10% of people with latent tuberculosis infection (LTBI) without HIV will progress to develop active tuberculosis (TB) in their lifetimes. This study was conducted to determine the magnitude of latent TB among the adult population at a teaching and referral Hospital in Ethiopia. METHODS: This study was conducted at the Chest clinic of Tikur Anbessa Specialized Hospital during 2010-2013. The study was a cross-sectional st

The integrated anatomy practical paper: A robust assessment method for anatomy education today.

Assessing anatomy in a way that tests higher cognitive domains and clinical application is not always straightforward. The old "spotter" examination has been criticized for only testing low level "identify" knowledge, whereas other assessment modalities such as multiple choice questions do not reflect the three dimensional and application nature of clinical anatomy. Medical curricula are frequently integrated and subject specific examinations do not reflect the case based, spiral, integrative nature of the curricula. The integrated anatomy practical paper (IAPP) is a hybrid of the old "spotter

The effectiveness of airline pilot training for abnormal events.

OBJECTIVE: To evaluate the effectiveness of airline pilot training for abnormal in-flight events. BACKGROUND: Numerous accident reports describe situations in which pilots responded to abnormal events in ways that were different from what they had practiced many times before. One explanation for these missteps is that training and testing for these skills have become a highly predictable routine for pilots who arrive to the training environment well aware of what to expect. Under these circumstances, pilots get plentiful practice in responding to abnormal events but may get little practice in

Emergency obstetric simulation training: how do we know where we are going, if we don't know where we have been?

BACKGROUND: Obstetric emergencies contribute significantly to maternal morbidity and mortality. Current training in the management of obstetric emergencies in Australia and internationally focusses on utilising a multidisciplinary simulation-based model. Arguments for and against this type of training exist, using both economic and clinical reasoning. AIMS: To identify the evidence base for the clinical impact of simulation training in obstetric emergencies and to address some of the concerns regarding appropriate delivery of obstetric emergency training in the Australian setting. METHODS: A

The effect of injection duration and injection site on pain and bruising of subcutaneous injection of heparin.

AIMS AND OBJECTIVES: To determine and compare the effects of four methods of subcutaneous heparin injection on pain and bruising in abdomen and thighs. BACKGROUND: Subcutaneous heparin injection is a common nursing clinical intervention. Nurses frequently inject heparin subcutaneously and this action often results in some complications such as bruising, haematoma, pain and induration in the injection site. There are also some other factors inducing complications associated with heparin injection, including the injection site and the injection duration. DESIGN: A quasi-experimental within-sub

Orthographic processing efficiency in developmental dyslexia: an investigation of age and treatment factors at the sublexical level.

Reading fluency beyond decoding is a limitation to many children with developmental reading disorders. In the interest of remediating dysfluency, contributing factors need to be explored and understood in a developmental framework. The focus of this study is orthographic processing in developmental dyslexia, and how it may contribute to reading fluency. We investigated orthographic processing speed and accuracy by children identified with dyslexia that were enrolled in an intensive, fluency-based intervention using a timed visual search task as a tool to measure orthographic recognition. Effects of three interventions on the reading skills of children with reading disabilities in grade 2.

In a longitudinal intervention study, the effects of three intervention strategies on the reading skills of children with reading disabilities in Grade 2 were analyzed. The interventions consisted of computerized training programs: One bottom-up intervention aimed at improving word decoding skills and phonological abilities, the second intervention focused on top-down processing on the word and sentence levels, and the third was a combination of these two training programs (n = 25 in each group). In addition, there were two comparison groups, 25 children with reading disabilities who received

Evaluation of hand hygiene adherence in a tertiary hospital.

BACKGROUND: Although hand hygiene is the most important measure in the prevention of nosocomial infection, adherence to recommendations among health care workers (HCW) is low. Evaluation of compliance with hand hygiene was carried out in a Spanish teaching hospital. METHODS: In 2005, adherence to hand hygiene was evaluated hospital wide through direct observation, collecting data on hand hygiene carried out whenever indicated (opportunity for hand hygiene). Compliance was defined as handwashing/disinfection in an opportunity for hand hygiene according to hospital protocols. The results were a
