The epidemiology of forearm nerve injuries--a retrospective study.

The aim of this study was to investigate the mechanisms and etiologic factors of forearm nerve injuries. This retrospective survey included all patients treated surgically in Clinical Department of Neurosurgery, Clinical Center of Serbia, from January 1, 2000 to December 31, 2010. All relevant data were collected from medical records. Statistical procedures were done using the PASW 18 statistical package. Our study included 104 patients that underwent surgery after forearm nerve injury. The majority of admitted patients were male (n = 84; 80.8%) and only 20 (19.2%) were female. Ulnar nerve inj

Golf-induced injuries of the wrist.

Golf injuries of the wrist are rare, but when they occur they can be devastating for the avid golfer, competitive amateur golfer, or the professional golfer, as the hand and wrist are so integral to the game. The majority of golf injuries are overuse injuries of the wrist flexor or extensor tendons. The left wrist (in the right-handed golfer) is the most common location. Analysis of the golf swing finds excessive motion of the left wrist (in the right-handed golfer), along with a catapulting function, accounting for vulnerability of the left wrist to injury. Hyperextension and radial deviation

Anterior Approach for Ankle Arthrodesis.

End-stage ankle arthritis may be treated successfully with either an ankle arthrodesis or arthroplasty. Both surgical interventions have demonstrated success with regard to pain relief and function. Ankle arthrodesis is indicated for patients with recalcitrant ankle pain despite appropriate nonoperative intervention. Patients who have a history of posttraumatic arthritis from a high-energy injury, soft-tissue compromise, limited range of motion, or deformity may be superior candidates for arthrodesis. The surgical technique can be undertaken with either an open or an arthroscopic approach. An

Biomechanical analysis of a combined double-bundle posterior cruciate ligament and posterolateral corner reconstruction.

BACKGROUND: Failure to address both components of a combined posterior cruciate ligament and posterolateral corner injury has been implicated as a reason for abnormal biomechanics and inferior clinical results. HYPOTHESIS: Combined double-bundle posterior cruciate ligament and posterolateral corner reconstruction restores the kinematics and in situ forces of the intact knee ligaments. STUDY DESIGN: Controlled laboratory study. METHODS: Ten fresh-frozen human cadaveric knees were tested using a robotic testing system through sequential cutting and reconstructing of the posterior cruciate lig

[Chronic instability of the proximal tibio-fibular articulation: hemi-long biceps ligamentoplasty by the Weinert and Giachino technique. Apropos of 3 cases].

PURPOSE OF THE STUDY: Recurrent dislocations of the superior tibiofibular joint are rare. We report three cases of Weinert and Giachino ligament reconstruction. MATERIALS: From 1989 to 1994, two soccer players and one young girl (10 years old) presented recurrent dislocation of the superior TFJ. The mechanism was a direct trauma, and the type of dislocation was antero-lateral. These patients were free of tibial fracture, major ligament lesion of the knee, and neurologic symptoms (peroneal nerve injury). Pain was over the fibular head, mechanical and increased by jumping. A lateral "cracking"

Lower donor-site morbidity using QT autografts for ACL reconstruction.

PURPOSE: Comparing scar cosmesis and regional hypoesthesia at the incision site between quadriceps tendon (QT), bone-patellar tendon-bone (BPTB), and hamstring tendon (HT) for anterior cruciate ligament (ACL) reconstruction. METHODS: Ninety patients undergoing ACL reconstruction with QT, HT or BPTB were evaluated at 1-year post-op. Scar cosmesis was assessed using the patient and observer scar assessment scale (POSAS) and length of the incision. Sensory outcome was analyzed by calculating the area of hypoesthesia around the scar. The classical ACL reconstruction functional follow-up was measu

Single-stage Achilles tendon reconstruction using a composite sensate free flap of dorsalis pedis and tendon strips of the extensor digitorum longus in a complex wound.

The reconstruction of the posterior heel including a wide defect of the Achilles tendon is difficult as a result of complicated infection, deficient soft tissue for coverage, and functional aspects and defects of the tendon itself. As a single-stage procedure, various methods of tendon transfer and tendon graft have been reported along with details of local flaps or island flaps for coverage. With advances in microsurgical techniques and subsequent refinements, several free composite flaps, including tendon, fascia, or nerve, have been used to reconstruct large defects in this area without fur

Treatment of stage II posterior tibial tendon dysfunction with flexor hallucis longus transfer and medial displacement calcaneal osteotomy.

Nineteen consecutive patients underwent flexor hallucis longus (FHL) tendon transfer and medial displacement calcaneal osteotomy for the treatment of Stage 2 posterior tibial tendon dysfunction. The FHL tendon was utilized for transfer because it approximates the strength of the posterior tibialis muscle and is stronger than the peroneus brevis muscle. Seventeen patients returned for follow-up examination, follow-up time 18 months (ave.). The AOFAS hindfoot score improved from 62.4/100 to 83.6/100. The subjective portion of the AOFAS hindfoot score improved from 31.0/60.0 to 48.9/60. Weightbea

Wide-Awake Flexor Pollicis Longus Tendon Reconstruction with Evaluation of the Active Voluntary Contraction of the Ruptured Muscle-Tendon.

The purpose of this study was to investigate the clinical outcome of the wide-awake tendon reconstruction for chronic rupture of the flexor pollicis longus tendon with evaluation of the voluntary active contraction distance of the ruptured musculotendinous unit. Eleven consecutive patients underwent tendon reconstruction under wide-awake surgery. If the total of the passive distraction distance and the active contraction distance of the ruptured flexor pollicis longus musculotendinous unit was greater than 30 mm, tendon grafting was performed. If not, tendon transfer was performed using the fo

The Anatomy of the Anterior Intermeniscal Ligament in Children: Implications for Tibial Spine Fractures, Meniscal Injuries, and Anterior Cruciate Ligament Reconstruction.

BACKGROUND: The anterior intermeniscal ligament (AIML) footprint and dimensions in skeletally immature knees have not been well documented. The AIML may play an important role in tibial spine fracture (TSF) reduction and meniscal repair/stability. The AIML is vulnerable to injuries during drill hole placement in anterior cruciate ligament (ACL) reconstruction. Therefore, it is necessary to characterize the anatomy of the AIML in skeletally immature knees to prevent iatrogenic injuries. PURPOSE: To evaluate AIML dimensions, its meniscal attachment patterns, and its relationship to the ACL. ST

Minimally Invasive Approach to Multiligament Knee Reconstruction: Arthroscopic PCL and PLC With Percutaneous LCL Reconstruction.

BACKGROUND: Multiligament knee injuries involving the posterior cruciate ligament (PCL), posterolateral corner (PLC), and lateral collateral ligament (LCL) are rare but complex pathologies. Historically managed with open surgery, these injuries are increasingly being addressed using minimally invasive techniques, which provide anatomical reconstruction and less surgical morbidity. INDICATIONS: We report the case of a 24-year-old male farmer who sustained a high-energy multiligament knee injury after a motorcycle accident. Six months after the accident, he presented with persistent daily insta

Comparing clinical and functional outcomes of anterior cruciate ligament reconstruction using sonoelastografi.

BACKGROUND: Injuries to the anterior cruciate ligament (ACL) are prevalent, particularly following athletic incidents. Most clinicians use MRI for diagnostic purposes and therapy assessment, but it can present challenges in terms of cost and accessibility. While ultrasound guidance (USG) is more readily available, it is inadequate for diagnosing ACL injury in isolation. Therefore, our study aimed to evaluate the effectiveness of sonoelastography, a novel method for assessing tendon injuries, in the context of anterior cruciate ligament (ACL) injuries. METHODS: This study involved a cohort of …

The Use of Dual-Energy X-ray Absorptiometry (DEXA) in Evaluating Recovery After Musculoskeletal Injuries in Athletes: A Scoping Review.

BACKGROUND: Dual-energy x-ray absorptiometry (DEXA) is a widely used imaging modality in sports medicine and orthopaedics due to its accuracy in assessing bone mineral density (BMD) and body composition. Tracking these parameters provides valuable insights into the recovery process after musculoskeletal injuries, which are prevalent among athletes. PURPOSE: To evaluate how DEXA imaging has been used to guide rehabilitation in both recreational and competitive athletes by monitoring changes in BMD and body composition after musculoskeletal injuries. STUDY DESIGN: Scoping review; Level of evid

Sonographic Evaluation of Tendinous Mallet Finger to Estimate the Extent of Extension Lag.

BACKGROUND: Although several reports have addressed tendinous mallet finger (TMF), they have not identified which method best informs surgeons about the extent of residual extension lag in these patients. We aimed to assess the association between sonographic classification of acute TMF and the degree of extension lag remaining at the final follow-up. METHODS: The inclusion criterion was acute TMF with symptom onset within 2 weeks. Thirty-eight patients (23 male and 15 female) participated, with a mean follow-up of 17.1 months (range, 12.3-23.5 months). Range of motion, including extension la
