A suggested technique for "orthotopic" heart transplantation in a patient with situs inversus.

We describe a technique for "orthotopic" heart transplantation in a patient with situs inversus. The left atrial, aortic, and pulmonary artery anastomoses were performed directly, in the usual manner. The recipient's right atrium was converted into a tunnel, and the donor's right atrium was left intact. Anastomoses were therefore required between the 2 inferior venae cavae (by direct end-to-end anastomosis) and the 2 superior venae cavae (necessitating the insertion of a Dacron-graft). We suggest that even simpler techniques, perhaps not requiring the use of an artificial vascular prosthesis,

Iliac arteriovenous fistula due to spinal disk surgery. Causes severe hemodynamic repercussion with pulmonary hypertension.

We present a case of a 46-year-old man with a pulsatile mass in the left inferior abdominal quadrant that irradiated a continuous murmur extending to the left lumbar region. Despite an 8-year history of cardiomegaly, he appeared to be asymptomatic except for the mass and could recollect no traumatic injury or surgery that might have caused it. Near the vertebral column, we found a small scar, the result of spinal disk surgery 11 years before. Following chest radiography and electrocardiography, we located the suspected arteriovenous fistula by selective angiography of the aorta and its branche

Fungal infection in a dissecting aneurysm of the thoracic aorta.

We describe a 54-year-old man who had an ascending aortic prosthetic graft and a porcine aortic valve prosthesis that were infected by Candida albicans. This infection led to the formation of a dissecting false aneurysm of the remaining transverse and entire descending thoracic aorta, and the man was admitted to our hospital for surgical treatment in February of 1991. Staged in situ graft replacement was performed using Borst's "elephant trunk" repair for the proximal aortic reconstruction and an open distal anastomosis technique for the distal repair. Candida albicans in the residual prosthet

Penetrating heart wounds repaired without cardiopulmonary bypass. Evaluation and follow-up of recent war injuries.

Penetrating cardiac injuries requires urgent management. Between September 1991 and June 1992, 10 patients with penetrating cardiac injuries sustained in war were treated at our hospital in Croatia, which does not have cardiopulmonary bypass facilities. Seven of these patients survived cardiorrhaphy and were discharged from the hospital, subject to follow-up. In 5 of the survivors, the injuries were inflicted by fragments from explosive devices; in 1 survivor, by a bullet; and in 1 survivor, by a rib fragment. The left ventricle was lacerated in 4 patients, the right ventricle in 2, the left a

Balloon dilation of tricuspid and pulmonary valves in carcinoid heart disease.

Heart involvement is usually the cause of death in patients with carcinoid syndrome, who may survive a long time even after the disease has entered an advanced stage. For this reason, carcinoid heart disease patients have undergone surgical replacement of affected valves. Two of our patients were not good candidates for surgery, due to the extent of hepatic metastasis. Alternatively, we performed percutaneous balloon valvuloplasty on both the tricuspid and pulmonary valves in both patients. To our knowledge, there has been only 1 previous report of successful tricuspid valvuloplasty in a case

Cusp-level chordal shortening for rheumatic mitral regurgitation: early results.

From February of 1987 through February of 1991, 25 patients with rheumatic mitral disease underwent cusp-level shortening of the chordae of the anterior mitral leaflet as part of the valvular reconstruction procedure at our institutions. All patients had moderate or severe mitral regurgitation, with prolapse of the anterior mitral leaflet. Seventeen patients also had mitral stenosis. Postoperative echocardiograms, both transthoracic and transesophageal, showed correction of leaflet prolapse and mitral regurgitation. This preliminary report suggests that the technique satisfactorily corrects mi

Cefuroxime prophylaxis in cardiovascular surgery: clinical, microbiological, and ecological impact.

To determine the efficacy of cefuroxime as a prophylactic agent against infection, we retrospectively evaluated patients who had undergone cefuroxime therapy for cardiovascular surgery procedures in the years 1987 and 1990. In the 403 evaluable patients (203 in 1987 and 200 in 1990), we noted a postoperative surgical wound infection rate of 3% and a nosocomial infection rate of 3%. These rates have remained constant in 1987 and 1990. Despite more than a 6-fold increase in cefuroxime consumption during this same period, we have not observed a selection for staphylococcal infections in cardiovas

latrogenic pseudoaneurysm of the innominate artery.

Central venous cannulation by the subclavian approach is a commonly performed procedure with an overall complication rate of up to 11%. In the English literature, however, there has been no previous documentation of pseudoaneurysm of the innominate artery as a complication of right subclavian vein catheterization. We report a case of a 55-year-old woman on chronic hemodialysis admitted to the hospital with pneumonia and sepsis who underwent multiple attempts at placement of a right subclavian vein catheter. These were unsuccessful and resulted in arterial puncture. The patient was discharged,

Late results after repair of partial atrioventricular septal defect in adolescents and adults.

From January 1966 through December 1985, 29 adolescents and adults underwent surgical repair of a partial atrioventricular septal defect at our institution. The patients included 20 females and 9 males, whose ages ranged from 16 to 47 years (mean, 27.6 +/- 10.1 years). Preoperatively, 24 patients were in New York Heart Association functional class I or II, and 5 were in class III. The pulmonary artery systolic pressure ranged from 22 to 62 mmHg (mean, 38.3 +/- 12.7 mmHg). The pulmonary-to-systemic flow ratio ranged from 1.4 to 2.9 (mean, 2.3 +/- 0.5). Pseudo-preexcitation in tricuspid atresia.

Because we have observed a relatively large number of tricuspid atresia patients with a short P-R interval and slurring of the initial QRS pattern suggesting preexcitation, we conducted a retrospective study to determine the frequency of this electrocardiographic pattern and whether or not this represented the presence of a true atrioventricular bypass tract. Three pediatric cardiologists reviewed the surface electrocardiograms of 183 consecutive tricuspid atresia patients who had been evaluated at the Mayo Clinic between 1980 and 1986. The patients' ages ranged from 4 months to 21 years; the

Quadricuspid aortic valve. Case report and review of the literature.

Quadricuspid aortic valve is a rare congenital anomaly that may be found unexpectedly at surgery or diagnosed preoperatively by means of echocardiography or aortography. The most common hemodynamic abnormality associated with this anomaly is aortic insufficiency. We describe a representative case involving a 62-year-old man with severe aortic insufficiency and mild aortic stenosis. At operation, the aortic valve was found to have 4 cusps. The valve was replaced with a mechanical prosthesis. The patient was released 7 days after operation and was clinically well at 6-month follow-up.

Asymptomatic strut fracture in DeBakey-Surgitool aortic valves: incidence, management, and metallurgic aspects.

From August 1971 through November 1972, we implanted 62 Model 2 DeBakey-Surgitool aortic valve prostheses in 62 patients, 4 of whom later had clinically asymptomatic strut fractures. In 1 case, the patient died suddenly, and autopsy revealed detachment of the ball-cage; in each of the other 3 cases, fractures of 2 struts close to the base of the prosthesis were diagnosed fluoroscopically, and the patients underwent successful reoperation. The interval between implantation and reoperation ranged from 11 months to 16 years, 9 months. In 1 patient, retrospective study of chest radiographs reveale

Clinical analysis of 15,089 operations on the heart and great vessels: results from the People's Republic of China.

Over a 28-year period, from November 1957 through November 1985, 15,089 patients underwent operations on the heart and great vessels at The Shanghai Chest Hospital. Herein we examine this experience, with particular attention to factors that affected mortality. Seven thousand two hundred eighty-one patients underwent procedures for mitral stenosis; 149 (2.0%) of these died. Two thousand one hundred thirty-three patients underwent closure of a patent ductus arteriosus; this procedure was associated with 9 deaths (0.4%), 5 of which were caused by massive hemorrhage. After 1964, over 98% of the p

Pentoxifylline therapy: a new adjunct in the treatment of pulmonary hypertension?

Patients with markedly elevated pulmonary vascular resistance, whether caused by primary pulmonary hypertension or by congenital heart disease, have a grave prognosis, regardless of the type of therapy they undergo. This brief report presents our experience in treating 6 patients (4 women and 2 men) having pulmonary vascular obstructive disease, by administrating pentoxifylline (Trental), a drug that has been used in patients with chronic occlusive systemic arterial disease. Our patients underwent treadmill testing before the study and again 1 to 3 months after initiation of the study. Duratio

Early-phase events with the mitroflow pericardial valve.

From January 1986 to January 1987, 116 Mitroflow pericardial valves were implanted in 98 patients at our center. Ages ranged from 10 to 83 years (mean, 64.9 years). Forty-three patients (44%) were in New York Heart Association Functional Class III or IV at the time of surgery. Twelve patients (12%) had undergone prior cardiac surgery. Ten hospital deaths (10%) (70% Confidence Limits, 7% to 14%) occurred. Incremental risk factors for hospital death included female gender (p = 0.08), higher functional class preoperatively (p = 0.04), and longer cardiopulmonary bypass time (p = 0.05). All 88 hosp

Early thrombosis of a tricuspid valve prosthesis: two-dimensional echocardiographic demonstration.

We report on the case of a 35-year-old woman who had undergone tricuspid valve replacement with a Lillehei-Kaster prosthesis 7 weeks prior to admission to our institution. Real-time two-dimensional echocardiography indicated a large mass on the inferolateral surface of the right atrium, as well as a dense band of echoes on the atrial and ventricular surfaces of the prosthesis. The diagnosis, confirmed by angiography, was early, acute thrombosis of the tilting disc prosthesis. After an unsuccessful infusion of urokinase, the patient was taken to emergency surgery; at operation, a large thrombus

Transvenous catheter ablation of a posteroseptal accessory pathway in a patient with coexistent posteroseptal and right-sided accessory pathways.

Transvenous catheter ablation techniques have recently been used successfully in blocking posteroseptal accessory pathways in patients with Wolff-Parkinson-White syndrome (WPWS). We report the case of a patient in the People's Republic of China who had WPWS and posteroseptal and right anterolateral accessory pathways, and who had failed to respond to conventional antiarrhythmic drug therapy. The posteroseptal accessory pathway was ablated successfully. During 23 months of follow-up there were no complications; this led us to conclude that this technique can be used to achieve successful long-t

Coronary artery anomalies: a review of more than 10,000 patients from the Clayton Cardiovascular Laboratories.

We reviewed the records of 10,661 patients who had undergone coronary angiography at the Clayton Foundation Cardiovascular Laboratories between 1 June 1974 and 15 March 1986, and identified major coronary artery anomalies in 83 adults. In addition, we included in our review 9 adults and 2 adolescents who had been referred for evaluation of anomalies documented elsewhere. Here we present the clinical and angiographic data for all 94 patients (76 men and 18 women). Most patients were men who presented with chest pain. The most common anomaly, found in 38 patients, was origin of left circumflex c

Rapid development of coronary arterial stenosis following valve replacement: an unusual cause of myocardial ischemia.

A 56-year-old woman had unstable angina pectoris 3 months after aortic and mitral valve replacement for rheumatic heart disease. Repeat coronary arteriography revealed critical proximal left anterior descending and circumflex artery stenoses in previously normal coronary arteries. This case emphasizes the need for thorough evaluation of postoperative valvular patients, despite normal preoperative coronary arteriograms.

Catheter ablation techniques in patients with supraventricular tachycardia.

Catheter ablation is an experimental procedure that may prove useful for patients with drug-refractory supraventricular tachycardia. Current techniques involve the use of high-energy direct-current shocks, delivered via electrode catheters. These techniques have been used most commonly for ablating the atrioventricular (AV) junction in patients with supraventricular tachycardia. Such ablation may be of value in a subset of patients with the Wolff-Parkinson-White syndrome. In addition, limited data suggest that direct catheter ablation of posteroseptal accessory pathways may prove safe and effe
