Medial sural artery perforator flap for repair of the hand.

We report our experience of using the medial sural artery perforator flap in the reconstruction of soft tissue defects in the hand in seven cases with 1 to 2 year follow-up. The flap is harvested from the posteromedial aspect of the leg, just below the knee and superficial to the medial head of the gastrocnemius muscle. It is based on the perforator arteries and veins supplied by the medial sural artery. The flaps ranged in size from 14 x 10 cm to 8 x 6 cm. The donor area was closed directly or by a skin graft. All but one flap survived. The cosmetic results were satisfactory and without appar

Does Perfadex affect outcomes in clinical lung transplantation?

BACKGROUND: The use of a low-potassium-based preservation solution improves gas exchange in experimental models of lung transplantation. However, its efficacy in reducing the incidence of primary graft dysfunction (PGD) and improving patient outcomes in the clinical setting is controversial. METHODS: In this study we measured: oxygenation index (OI); International Society of Heart and Lung Transplantation (ISHLT) PGD grades; extubation times; intensive care unit (ICU) and hospital length of stay; 30-day, 90-day and 1-year survival rates; and bronchiolitis obliterans syndrome (BOS)-free surviv

Ataxin-10, the spinocerebellar ataxia type 10 neurodegenerative disorder protein, is essential for survival of cerebellar neurons.

Spinocerebellar ataxia (SCA) type 10, an autosomal dominant disease characterized by cerebellar ataxia, is caused by a novel pentanucleotide (ATTCT) repeat expansion in the SCA10 gene. Although clinical features of the disease are well characterized, nothing is known so far about the affected SCA10 gene product, ataxin-10 (Atx-10). We have cloned the rat SCA10 gene and expressed the corresponding protein in HEK293 cells. Atx-10 has an apparent molecular mass of approximately 55 kDa and belongs to the family of armadillo repeat proteins. In solution, it tends to form homotrimeric complexes, whi

Fetal organ and xenograft transplantation.

The use of anencephalic fetuses and neonates as organ donors is technically feasible, and xenografts may be. Several ethical questions emerge, however, involving the appropriate use of such organs and what alterations, if any, in obstetric and neonatal management may be acceptable to increase the availability and likelihood of success of such organs.

Identifying ovarian cortex cell subpopulations using multicolor flow cytometry.

CONTEXT: Ovarian tissue autotransplantation is currently the only proven technique for reusing ovarian tissue after fertility preservation by ovarian tissue cryopreservation. However, one of its limitations relates to the quality of the grafts, both in terms of the number of surviving follicles and the quality of the stromal environment, which is essential for follicular development and graft revascularization. The aim of this study was to validate a technique for characterizing and functionally qualifying ovarian tissue in order to identify cell sub-populations of interest. MATERIALS: Ovaria

From Cellular Radiosensitivity to Precision Radiotherapy: Integrating Functional Assays, Genomics, and Clinical Modeling.

Background: Radiotherapy outcomes are determined by the balance between tumor control and normal tissue toxicity, both of which exhibit significant interpatient variability. While radiogenomic and molecular approaches have identified determinants of radiosensitivity, their predictive performance remains limited when used in isolation. Methods: This review provides a comprehensive synthesis of functional assays, genomic biomarkers, and integrative models used to assess radiosensitivity, including radiation-induced lymphocyte apoptosis (RILA), chromosomal radiosensitivity assays, micronucleus fo

Impact of soft-tissue management techniques on immediate implant placement: a randomized controlled trial.

PURPOSE: In immediate implant placement, both open and closed healing techniques are used, but their comparative esthetic and tissue stability outcomes remain under debate. This study aimed to evaluate and compare these two approaches to support clinical decision-making. METHODS: In this prospective, randomized controlled trial, 46 patients received a total of 48 implants, assigned to either an open healing group (n = 25) or a closed healing group (n = 23). Clinical and radiological assessments were conducted at three time points: T1 (pre-extraction), T2 (3 months post-op), and T3 (12 months

One ligand, multiple fates: the multi-receptor signaling network of FSTL1 and receptor-selective intervention strategies.

Follistatin-like protein 1 (FSTL1) is a secreted glycoprotein involved in cardiovascular protection, tissue repair, inflammation, fibrosis, and tumor biology. Accumulating evidence indicates that FSTL1 exerts opposing effects across tissues and disease contexts, producing either protective or pathogenic outcomes. Although this functional duality is generally considered context dependent, its mechanistic basis remains incompletely understood. Recent studies suggest that FSTL1 does not signal through a single receptor but engages or modulates multiple functionally distinct receptor systems, incl

Gut microbiota-derived metabolites as context-dependent modulators of Nrf2 signaling in inflammation-driven carcinogenesis.

Chronic inflammation fosters cancer development by sustaining redox imbalance, oxidative stress and persistent tissue damage. A key component to this process is the transcription factor Nrf2 (nuclear factor erythroid 2-related factor 2), which coordinates antioxidant and detoxification programs to maintain redox homeostasis. While transient Nrf2 activation protects non-malignant tissues by limiting cell and tissue damage, persistent or dysregulated Nrf2 signaling in premalignant tissue and established tumors can promote cancer cell survival, metabolic adaptation and resistance to chemotherapy.

The 'obesity paradox' in patients undergoing transcatheter aortic valve implantation. A specialized narrative review.

Obesity has long been identified as a noteworthy risk factor for cardiovascular diseases, including aortic stenosis. Nevertheless, an unexplained phenomenon known as the obesity paradox has arisen in the context of transcatheter aortic valve implantation (TAVI), inquiring conventional beliefs. The obesity paradox refers to better clinical outcomes observed in obese individuals undergoing TAVI. These include reduced all-cause mortality, lower rates of procedural complications, and improved long-term survival rates compared to their lean counterparts. Several theories have been proposed to expla

The impact of heterotopic ovarian tissue transplantation with varied volumes on reproductive endocrine function in oophorectomized rats.

BACKGROUND: This study aimed to investigate the effects of different volumes of ovarian tissue transplantation on the reproductive endocrine function of rats after oophorectomy. METHODS: Female rats were selected to establish a castration model and then underwent different volumes of ovarian tissue transplantation. Group I served as the sham operation group. The transplantation group was divided into five subgroups based on the calculated ratio of ovarian weight to body weight in normal female rats, δ = (2.52 ± 0.17) ×10-4: Group II: transplanted ovarian volume was δ; Group III: 0.75δ; Group

Stress-induced eosinophil activation contributes to postoperative morbidity and mortality after lung resection.

Respiratory failure occurs more frequently after thoracic surgery than abdominal surgery. Although the etiology for this complication is frequently attributed to underlying lung disease present in patients undergoing thoracic surgery, this notion is often unfounded because many patients with normal preoperative pulmonary function often require prolonged oxygen supplementation even after minimal resection of lung tissue. Using a murine model of pulmonary resection and peripheral blood samples from patients undergoing resection of the lung or abdominal organs, we demonstrated that lung surgery i

N-acetylcysteine ameliorates chemotherapy-induced impaired anxiety and depression-like behaviors by regulating inflammation, oxidative and cholinergic status, and BDNF release.

Mood disorders caused by chemotherapy have become more important as the survival of cancer patients increases, and new studies in this field will contribute to the prevention of this disorder. For this purpose, we used methotrexate, a chemotherapeutic agent frequently preferred in oncological cases. Mtx was administered as a single dose of 100 mg/kg intraperitoneally to male Wistar albino rats. Since oxidative stress plays an important role in chemotherapy-induced emotional impairment, n-acetylcysteine (NAC), a potent antioxidant, was administered at 500 mg/kg in two doses before Mtx administr

Importance of Mitochondria in Cardiac Pathologies: Focus on Uncoupling Proteins and Monoamine Oxidases.

On the one hand, reactive oxygen species (ROS) are involved in the onset and progression of a wide array of diseases. On the other hand, these are a part of signaling pathways related to cell metabolism, growth and survival. While ROS are produced at various cellular sites, in cardiomyocytes the largest amount of ROS is generated by mitochondria. Apart from the electron transport chain and various other proteins, uncoupling protein (UCP) and monoamine oxidases (MAO) have been proposed to modify mitochondrial ROS formation. Here, we review the recent information on UCP and MAO in cardiac injuri

Usefulness of real-time elastography for diagnosing lymph node metastasis of skin cancer: does elastography potentially eliminate the need for sentinel lymph node biopsy in squamous cell carcinoma?

BACKGROUND: The metastatic involvement of regional lymph nodes is the most important prognostic factor for overall survival of skin cancer patients. The sonographic technique of freehand real-time tissue elastography (RTE), which displays tissue rigidity as a colour overlay of the tissue image, was developed. OBJECTIVE: Our purpose was to evaluate the benefit of RTE for detecting lymph node metastases of skin cancer non-invasively before operation. METHODS: We first selected lymph nodes of skin cancer patients which had already been diagnosed by biopsy as being reactive or metastatic, and th
