Syneresis and vitreous hemorrhage in pseudophacia.

The eye of a 60-year-old man with a clinically very successful lens implant became available for pathological study after a very sudden and uncomplicated death about 6 months following cataract surgery. The following changes were found: slight non-granulomatous uveitis associated with foreign-body reaction to the lens implant and its supporting iris suture, absence of the anterior vitreous face, advanced syneresis of the central vitreous, vitreous hemorrhage with diffuse layering of erythrocytes on the inner aspect of the remaining vitreous crust, vitreous traction on peripheral and central re…

Update of ocular reticulum cell sarcoma.

Reticulum cell sarcoma involving the vitreous and the brainstem occurred in a 45-year-old man. He initially was seen with diplopia from a partial left-sided third cranial nerve palsy, which is rare. Later, a typical uveitis developed in the right eye. An initial diagnosis of brainstem glioma, based primarily on the computed tomographic scan findings and clinical history, was ultimately proved erroneous when the correct diagnosis was shown by the results of a cytologic examination of vitreous aspirate. Excellent visual response to a moderately high oral dose of steroids occurred, which has not …

Endophthalmitis after cataract extraction: a retrospective case study.

A 43-year-old man was diagnosed as a case of near-mature senile cataract, chronic simple glaucoma, and pterygium both eyes. Pterygium shaving and extracapsular cataract extraction were done together. Thick aftercataract was diagnosed with moderate iridocyclitis on fourth postoperative day. Treatment was started with atropine and steroids, but without any relief. A second operation of curette evacuation of aftercataract was done to rule out and treat the presumed lens induced uveitis, but the condition further deteriorated. In next four days the eye had to be evacuated. On culture material, Sta…

Immunoglobulin concentrations in human tears in ocular diseases.

Immunoglobulin concentrations in human tears were determined in 165 patients with different eye diseases by a standard radial immunodiffusion method. IgA was present in all the samples in measurable quantity. The mean IgA values were significantly higher than the controls in patients with acute bacterial conjunctivitis, keratomalacia, corneal graft reaction, blepharoconjunctivitis, and acute keratoconjunctivitis. The values in the patients with vernal conjunctivitis, phlyctenular conjunctivitis, acute bacterial corneal ulcer, and acute endogenous uveitis were not significantly different from t…

Pathology of ocular onchocerciasis: human and experimental.

Involvement of the outer eye in Onchocerca volvulus infection is characterized by an inflammatory process around degenerating microfilariae. Individual microfilariae in the cornea may be associated with localized punctate lesions or, especially when present in large numbers, may provoke a sclerosing keratitis due to invasion by fibrovascular pannus from the limbal conjunctiva. Anterior uveitis develops in some patients and can be either granulomatous, probably as a result of direct microfilarial invasion of the iris and ciliary body, or non-granulomatous, in which case the inflammation is like…

The influence of subconjunctival depot corticosteroid on choroidal detachment following retinal detachment surgery.

This study indicates that primary retinal detachment surgery on patients aged 55 or over is complicated by uveitis and choroidal detachment at approximate rates of 24% and 44%, respectively. These complications did not influence the anatomic cure rate or recovery of visual acuity. Although only 89 patients were recruited, the technique of random selection equally distributed the parameters studied and provided results of high reliability. The internal controls of the protocol prevented the examiners from being aware of whether the drug or placebo had been administered. We were concerned that t…

Scar sarcoidosis in burn scars associated with retained surgical staples.

Scar sarcoidosis is a form of cutaneous sarcoidosis characterized by granulomatous inflammation arising within pre-existing scars or sites o … s with polarizable foreign material. The presence of retained staples and subsequent development of uveitis complicated the diagnostic evaluation. This case highlights the diagnostic challenge of differentiating scar sarcoidosis from foreign body granulomatous reactions and emphasizes the potential role of retained foreign material as a trigger for sarcoi …

Ocular Toxoplasmosis Mimicking Lymphoma: Exploring the Correlation and Distinction.

We present a case of a 74-year-old woman with chronic lymphocytic leukemia (CLL) who presented with unilateral blurry vision that had progressively worsened over a few weeks. Ophthalmic examination revealed unilateral anterior chamber, vitreous body inflammation along with retinal infiltration which was initially diagnosed with posterior uveitis. Analysis of vitreous fluid aspiration was negative for bacteria, fungal and viral etiologies. Despite the broad-spectrum intraocular antibiotics, her vision continued to decline, and she later developed retinal detachment. Cytology for lymphoma was ne…

Behçet's disease complicated by pylephlebitis and hepatic abscesses.

A 22 year old man presented with fever, abdominal pain, weight loss and diarrhea. Past medical history revealed recurrent aseptic meningitis, uveitis, and erythema nodosum. Further inquiry unveiled a prominent history of oral aphthous ulcers; all features of Behçet's disease. Imaging revealed mesenteric arteritis and pylephlebitis, septic thrombophlebitis of the portal vein, a previously unrecognized complication of Behçet's disease, with multiple intrahepatic abscesses. Portal venography demonstrated an extensively diseased, expanded, and obstructed portal venous system. Blood cultures and po…

Bromocriptine in rheumatic and autoimmune diseases.

BACKGROUND AND OBJECTIVES: Multiple lines of evidence support the concept that the anterior pituitary hormone prolactin has a pathogenic role in rheumatic and autoimmune diseases including, but not limited to, rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), Reiter's syndrome, psoriatic arthritis, and uveitis. Conversely, the dopaminergic agonist bromocriptine appears to have therapeutic effects through suppression of pituitary prolactin secretion and, perhaps, through actions on peripheral dopamine receptors. This article reviews the experimental and clinical data supporting the…

Recurrence of herpetic keratouveitis following cataract surgery.

PURPOSE: Herpetic eye disease (HED) is a leading cause of infectious vision loss worldwide. Cataract surgery is known to be implicated in triggering the recurrence of herpes keratouveitis. This study evaluates the current evidence on the association between HED and cataract surgery, focusing on the risk factors and potential pathogenic mechanisms and preventive strategies involved with reactivation or new-onset herpes-related keratitis, uveitis, or keratouveitis. METHODS: A comprehensive search of databases (Cinahl, Embase, Scopus, ProQuest, Web of Science, and PubMed) was conducted to identi…
