Approach to vasculitic neuropathies.

Since vasculitic neuropathy is treatable and potentially debilitating, clinicians should develop an approach to neuropathy that increases the likelihood of uncovering existing systemic or nonsystemic vasculitis. The presence of a connective tissue disease, systemic vasculitis, asymmetric or non--length-dependent axonal polyneuropathy, or multiple axonal mononeuropathies should heighten suspicion, but vasculitic neuropathy can also present as a distal symmetric polyneuropathy with or without other organ involvement. Electrodiagnostic testing utilizing extensive nerve conductions may be helpful …

Granulomatosis With Polyangiitis: A Case of a 62-Year-Old Female Presenting With a Fever of Unknown Origin.

Fever of unknown origin (FUO) continues to present diagnostic challenges despite advancements in medical science. Granulomatosis with polyangiitis (GPA), a rare antineutrophil cytoplasmic antibody (ANCA)-associated small-vessel vasculitis characterized by necrotizing granulomas, is an uncommon but important cause of FUO. This case report describes a 62-year-old female presenting with prolonged fever, malaise, and nonspecific systemic symptoms, ultimately diagnosed with GPA after extensive investigations, including positive C-ANCA and anti-proteinase-3 serology and imaging revealing a cavitatin…

New Onset of Giant Cell Arteritis following ChAdOx1-S (Vaxevria®) Vaccine Administration.

We report a 78-year-old man presenting with persistent headaches in vertex and temporo-parietal area; fatigue, worsening after walking; jaw claudication; scotomas; pharyngodynia; and dry cough after the second dose of the SARS-CoV-2 vaccine (ChAdOx1-S) administration. Laboratory findings showed an elevated C-reactive protein level and FDG-CT PET showed evidence of active large vessel vasculitis with diffuse abnormal artery uptake. Under suspicion of vasculitis, a temporal arteries biopsy was performed; the histopathologic findings demonstrated the transmural inflammatory infiltrate with giant …

Progressive Hemifacial Atrophy With Contralateral Uveitis: A Case Report.

INTRODUCTION: Progressive hemifacial atrophy, known as Parry-Romberg syndrome (PRS), was first described by Parry in 1825. There is a progressive atrophy of facial tissues including skin, bones and muscles. Ophthalmic disorders are common and include keratitis, uveitis, cataract, ipsilateral enophthalmos, optic neuritis, retinal vasculitis and scleral melting. CASE PRESENTATION: We describe a patient with progressive hemifacial atrophy at right facial side who developed granulomatous uveitis and periferic retinal vasculitis in his left eye. We started topical and systemic steroid therapy. Uve…

Atypical Cogan's syndrome associated with coronary disease.

Cogan’s syndrome (CS) is a rare inflammatory disorder characterized by interstitial keratitis and vestibuloauditory abnormalities often associated with various systemic manifestations. Involvement of cardiovascular system resembling systemic vasculitis may lead to severe complications and death. The present report describes a case of a female patient with atypical Cogan's syndrome presented with systemic manifestations and severe coronary and femoral artery stenosis. Despite the clinical improvement after glucocorticoids and cyclophosphamide, the patient required double aortocoronal bypass gra…
