A 39-year-old Japanese man was admitted to our hospital after experiencing recurrent episodes of pancreatitis over the previous 2 years. On the first episode, he had been admitted to our hospital with elevated serum amylase levels and epigastralgia. Abdominal computed tomography (CT) revealed a diffuse, uncircumscribed area with heterogeneous density in the pancreas. No previous history of pancreatitis, gallstones, drinking, or abdominal injury was elicited. Magnetic resonance cholangiopancreatography (MRCP) demonstrated that the Wirsung duct was unconnected to the Santorini's duct. Endoscopic

STUDY DESIGN: Case report.
OBJECTIVES: To present an interesting case of a 14-year-old male with acute paresis of upper extremities and progressive difficulty with lower extremities. The patient is a competitive wrestler and was performing his daily abdominal workout 'sit-ups' with hands interlocked behind his head. During the end and immediately following his abdominal workout, he felt progressive weakness in his upper extremities. He was rushed to the hospital within an hour and seen in the emergency room and admitted to the neurology service for a presumed thromboembolic event.
SETTING: N

PURPOSE: We report our experience with hand-assisted retroperitoneoscopic nephroureterectomy for upper urinary-tract urothelial cancer.
PATIENTS AND METHODS: Our initial 10 cases of clinical stage T(1)N(0)M(0) renal pelvic and ureteral tumors treated with hand-assisted retroperitoneoscopic nephroureterectomy are included in the present report. Nephrectomy was conducted retroperitoneoscopically with hand assistance via a lower-abdominal midline incision. Resection of the lower ureter together with the bladder cuff was performed as open surgery and the specimen was removed en bloc via the same

BACKGROUND: Radiotherapy is potentially curative in early stages of follicle center lymphoma. Frequent side effects are pancytopenia, nausea and abdominal discomfort. A radiation-induced liver injury with serious clinical symptoms and changes in liver function is a rare complication.
CASE REPORT: Whole abdomen was irradiated in a 49-year-old patient with a centrocytic-centroblastic lymphoma, stage IA (localization: left inguinal region). A total dose of 30 Gy was delivered in a weekly fractionation of five times 1.5 Gy. Kidneys were protected by shielding after a dose of 13.5 Gy, liver blocks

An accurate description of the number, location and relative position of the air sacs and their diverticula in the ostrich is essential for a better understanding of the pathogenesis of air sacculitis in this bird. The air sacs were studied in ten ostriches of varying ages by latex or silicone casting of the respiratory tract and dissection. Results revealed that the air sacs of the ostrich conform to the general pattern in birds. Cervical, lateral and medial clavicular, cranial and caudal thoracic, and abdominal air sacs are present. The left and right medial clavicular air sacs fuse with eac

PURPOSE: We report our clinical experience with anterior pelvic osteotomy in 16 patients who underwent surgery for bladder exstrophy. The technique and its difficulties are discussed.
MATERIALS AND METHODS: Anterior pelvic osteotomy of the superior ramus of the public bone is a simple and efficient method to facilitate symphyseal approximation and abdominal wall closure without or with low tension on the suture lines in neonates who undergo surgery for bladder exstrophy. Older children in whom surgery has been delayed can also benefit from this method. Compared to other methods of osteotomy i

Liver laceration is the most common cause of mortality following blunt abdominal trauma in children. To determine the optimal route for volume expansion in the clinical setting of blunt abdominal injury, we compared the response to fluid resuscitation delivered above (Group 1) or below (Group 2) the diaphragm in a canine model of liver laceration. After induction of hemorrhagic shock by resection of 45% to 55% of the right median hepatic lobe followed by immediate closure of the laparotomy incision, we monitored pulse, arterial and venous pressures, urine output, pH, and lactate levels for thr

INTRODUCTION: Exploratory laparotomy has been the main method for surgical interventions in penetrating abdominal trauma (PAT). Minimally invasive surgery (MIS) has been utilized in the treatment of selected trauma patients as an alternative to laparotomy. This study aimed to investigate the utilization of MIS and open approaches in patients with PAT.
METHODS: Patients who underwent surgical treatment for PAT were included in this study using the National Inpatient Sample 2016-2020. Patients who underwent diagnostic laparoscopy or other laparoscopic or robotic procedures during admission were

A 49-year-old man with hypertension for 2 years, was incidentally found to have right intrarenal aneurysms during a routine abdominal ultrasound examination. He reported fluctuating blood pressure readings over the past 18 months. Laboratory parameters were within normal limits, with a serum creatinine level of 1.05 mg/dL. An abdominal CT angiogram revealed a large lobulated intrarenal aneurysm in the lower pole, which communicated with another smaller cortical aneurysm. Additional features suggested the presence of an arteriovenous (AV) fistula. Upon further questioning, the patient disclosed

BACKGROUND: Nonoperative management of traumatic blunt splenic injury is preferred over splenectomy because of improved outcomes and reduced complications. However, variability in treatment is previously reported with respect to hospital profit types and ownership.
METHODS: Our study objectives were to investigate the past decade's trends in pediatric splenic injury management and to determine whether previously reported disparities by hospital type have changed. We analyzed data from the Kid's Inpatient Database from Healthcare Cost and Utility Project for Years 2000, 2003, 2006, and 2009. M

BACKGROUND: The computed tomographic signs of hypoperfusion (CTSHs) have been reported in radiology literature as preceding the onset of clinical shock in children, but its correlation with tenuous hemodynamic status in adult blunt trauma patients has not been well studied. We hypothesized that these CT findings represent a clinically hypoperfused state and predict patient outcomes.
METHODS: We retrospectively reviewed 52 adult blunt trauma patients who presented to our Level I trauma center with an Injury Severity Score (ISS) greater than 15 and a systolic blood pressure less than 90 mm Hg a

A 60-year-old patient underwent triple coronary artery bypass grafting following an inferoseptal myocardial infarction and early onset of exertional angina. Four years later he was involved in a car accident during which he sustained an abdominal and thoracic trauma. Approximately 1 month after discharge, a ventricular septal defect was diagnosed by two-dimensional Doppler echocardiography with patency of all grafts at coronary angiography. Closure of the septal defect was successfully accomplished through a right atrial approach. Rupture of the ventricular septum following blunt chest trauma

From 1963 to 1993, 64 Patients with traumatic lesions of the diaphragm were treated. 58 Patients were admitted after blunt trauma, 6 Patients after a penetrating trauma. 87.5% of the patients were male with an average age of 37 years. In about 80% of cases other severe traumatic lesions were noted in association. With 64% extremities, 50% abdomen, 47% thoracic cavity and 38% head injuries were concomitant lesions. Due to the association with pelvic fractures (45%) a special diagnostic emphasis on diaphragmatic ruptures is recommended. In 83% the left diaphragm is involved. Over the last years

BACKGROUND: Oesophageal trauma is uncommon. The aim of this study was to conduct a descriptive analysis of penetrating oesophageal trauma and determine risk factors for oesophageal related complications and mortality in the National Trauma Data Bank (NTDB).
METHODS: Patients with penetrating oesophageal trauma from Levels 1 and 2 trauma centres in the NTDB (2007 and 2008) that specified how complication and comorbidity data were recorded were selected. Data collected included age, injury severity score (ISS), abbreviated injury scores (AIS), lengths of stay (LOS) and ventilation days, systoli
