Efficacy of a modified double-guidewire technique using an uneven double lumen cannula (uneven method) in patients with surgically altered gastrointestinal anatomy (with video).

BACKGROUND: Balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography (BE-ERCP) has been reported to be effective for patients with surgically altered gastrointestinal anatomy. However, selective biliary cannulation remains difficult in BE-ERCP. We examined the usefulness of a modified double-guidewire technique using an uneven double lumen cannula (the uneven method) for BE-ERCP in patients with surgically altered gastrointestinal anatomy. METHODS: To clarify the usefulness of the uneven method for selective biliary cannulation in BE-ERCP in comparison to the pancreatic gui

Usefulness of subtraction of 3D T2WI-DRIVE from contrast-enhanced 3D T1WI: preoperative evaluations of the neurovascular anatomy of patients with neurovascular compression syndrome.

BACKGROUND AND PURPOSE: High-resolution 3D MR cisternography techniques such as 3D T2WI-driven equilibrium radiofrequency reset pulse (DRIVE) are used preoperatively to assess neurovascular anatomy in patients with neurovascular compression syndrome, but contrast between vessels and cranial nerves at the point of neurovascular contact is limited. The postprocessing technique subtraction of 3D T2WI-driven equilibrium radiofrequency reset pulse from contrast-enhanced 3D T1WI (sDRICE) provides both high spatial resolution and excellent contrast in depicting the neurovascular contact. We evaluated

Anatomy and histology of the male reproductive tract and spermatogenesis fine structure in the lesser anteater (Tamandua tetradactyla, Myrmecophagidae, Xenarthra): morphological evidences of reproductive functions.

The anatomy and histology of the male genital tract of the lesser anteater were studied. Fine details of spermatozoa regarding their genesis and morphology were also studied in six adult specimens. The testes lie in the pelvic cavity. The deferent duct emerges from the epididymis and opens into the ejaculatory duct, which drains into the membranous urethra. Accessory glands (prostate, seminal vesicle and bulbourethral gland) are histologically similar to those described in other mammals. The short penis presents an urethral orifice, while the corpus spongiosum becomes thinner at the end indica

Anatomy of Atrioventricular Node Artery and Pattern of Dominancy in Normal Coronary Subject: A Comparison between Individuals with and without Isolated Right Bundle Branch Block.

BACKGROUND: Isolated right bundle branch block (RBBB) is a common finding in the general population. The atrioventricular node (AVN) artery contributes to the blood supply of the right bundle branch. Our hypothesis was that the anatomy of the AVN artery and the pattern of dominancy differ between subjects with and without RBBB. METHODS: We retrospectively studied the coronary angiography of 92 patients with RBBB and 184 age- and gender-matched controls without RBBB. All the subjects had angiographically proven normal coronary arteries. The dominant circulation and precise origin of the AVN ar

Evaluation of the vascular anatomy in potential living kidney donors with gadolinium-enhanced magnetic resonance angiography: comparison with digital subtraction angiography and intraoperative findings.

BACKGROUND: X-ray contrast arteriography has traditionally been used for pre-operative evaluation in living kidney donors. However, magnetic resonance angiography (MRA) offers a non-invasive alternative, which has been considered to be less accurate. This study was performed to determine whether MRA in the pre-operative investigation of living kidney donors provides sufficient information. METHODS: From December 2005 to December 2007, 173 potential live donors were evaluated in this study. Donors performed digital subtraction angiography (DSA) and those with one or more accessory arteries at

Brain and sense organ anatomy and histology of the Falkland Islands mullet, Eleginops maclovinus (Eleginopidae), the sister group of the Antarctic notothenioid fishes (Perciformes: Notothenioidei).

The perciform notothenioid fish Eleginops maclovinus, representing the monotypic family Eleginopidae, has a non-Antarctic distribution in the Falkland Islands and southern South America. It is the sister group of the five families and 103 species of Antarctic notothenioids that dominate the cold shelf waters of Antarctica. Eleginops is the ideal subject for documenting the ancestral morphology of nervous and sensory systems that have not had historical exposure to the unusual Antarctic thermal and light regimes, and for comparing these systems with those of the phyletically derived Antarctic s

Abnormal axonal guidance and brain anatomy in mouse mutants for the cell recognition molecules close homolog of L1 and NgCAM-related cell adhesion molecule.

Cell recognition molecules of the L1 family serve important functions in the developing and the mature nervous system. Mutations in genes encoding the L1 family members close homolog of L1 (CHL1) and NgCAM-related cell adhesion molecule (NrCAM) have been found to alter connectivity and morphology of several brain regions. In order to emphasize similarities and differences of these two structurally related molecules, null mutants for CHL1 and NrCAM were directly compared with respect to axonal guidance in the hippocampus and the olfactory bulb and the sizes of the ventricular system and the cer

Intercostal Transdiaphragmatic Port Insertion During Laparoscopic Resection of Liver Tumors Below the Right Diaphragmatic Dome Based on Costal and Pulmonary Anatomy: A Systematic Review.

Several reports advocate placement of ports between ribs for performing laparoscopic liver resection involving segments 7 and 8, but adoption of this method still is debated. Further, precise positioning of these intercostal transdiaphragmatic ports (ICPs) is required to avoid complications. We undertook a literature search to compare outcomes of laparoscopic resection between use and nonuse of ICPs. We further included review of pertinent anatomic features aiming to guide safe introduction of ICPs. Among 380 articles screened, 6 compared aspects of outcomes with respect to ICPs. ICPs reduced

Dominant Orbitofrontal Pial Supply in Anterior Cranial Fossa Dural Arteriovenous Fistula: Angiographic Differentiation from Mixed Pial-Dural Arteriovenous Malformation and Anatomy-Based Treatment Selection.

BACKGROUND: Anterior cranial fossa dural arteriovenous fistulas (ACF DAVFs) usually receive ethmoidal dural supply. Pial arterial supply has been described in intracranial DAVFs, including ACF DAVFs, but a dominant orbitofrontal pial feeder can create diagnostic overlap with mixed pial-dural arteriovenous malformation and make endovascular treatment hazardous. CASE PRESENTATION: A 75-year-old man with atrial fibrillation presented with right middle cerebral artery occlusion and underwent intravenous thrombolysis followed by mechanical thrombectomy. During right internal carotid angiography, t

MRI-Guided High-Intensity Focused Ultrasound in Movement Disorders: Targeting Pathologic Brain Circuits With Precision Imaging, From the AJR Special Series on Critical Anatomy.

MRI-guided high-intensity focused ultrasound (MRgHIFU) has emerged as an alternative to other neuromodulatory interventions for patients with medically refractory movement disorders, including open surgery, radiosurgery, and deep brain stimulation. Microelectrode recordings permit refinement of indirect targets during open procedures for movement disorders. However, their use is not possible during incisionless procedures including MRgHIFU. Traditional MRgHIFU employing indirect targets yields limited efficacy or adverse effects from anatomic or functional variability. Furthermore, the clinica

Sexual dysfunction after female genital mutilation/cutting: A retrospective cross-sectional study of associations with organ-specific genital anatomy, trauma history, and psychiatric comorbidity.

Sexual dysfunction after female genital mutilation/cutting (FGM/C) is poorly characterized, limiting targeted clinical intervention. In a concentrated FGM/C referral population, we aimed to characterize patterns of sexual dysfunction and evaluate associations with organ-specific genital anatomy, trauma history, and psychiatric comorbidities among women with FGM/C. We conducted a retrospective cross-sectional study of adults with FGM/C evaluated between 2014 and 2025 in an obstetrics and gynecology clinic specializing in sexual trauma. Using standardized abstraction tools, we obtained sexual sy

International standardization and optimization group for intersphincteric resection (ISOG-ISR): modified Delphi consensus on anatomy, definition, indication, surgical technique, specimen description and functional outcome.

AIM: Intersphincteric resection (ISR) is an oncologically complex operation for very low-lying rectal cancers. Yet, definition, anatomical description, operative indications and operative approaches to ISR are not standardized. The aim of this study was to standardize the definition of ISR by reaching international consensus from the experts in the field. This standardization will allow meaningful comparison in the literature in the future. METHOD: A modified Delphi approach with three rounds of questionnaire was adopted. A total of 29 international experts from 11 countries were recruited fo

An Alzheimer's Disease Mechanism Based on Early Pathology, Anatomy, Vascular-Induced Flow, and Migration of Maximum Flow Stress Energy Location with Increasing Vascular Disease.

This paper suggests a chemical mechanism for the earliest stages of Alzheimer's disease (AD). Cerebrospinal fluid (CSF) flow stresses provide the energy needed to induce molecular conformation changes leading to AD by initiating amyloid-β (Aβ) and tau aggregation. Shear and extensional flow stresses initiate aggregation in the laboratory and in natural biophysical processes. Energy-rich CSF flow regions are mainly found in lower brain regions. MRI studies reveal flow stress "hot spots" in basal cisterns and brain ventricles that have chaotic flow properties that can distort molecules such as A
