This case report illustrates the difficulties that can arise when diagnosing a CNS infection. Following an episode of gastroenteritis, our patient developed dizziness, a fever up to 39 °C, photophobia, double vision, multiple cranial nerve deficits, and a resting nystagmus with alternating directions, and presented to the emergency department. Blood inflammatory markers were unremarkable, and a contrast-enhanced head CT scan revealed no abnormalities. The patient was discharged home with a provisional diagnosis of Meniere's disease. Two days later, the patient developed increasing lethargy and

Autoimmune diseases (AIDs) constitute a heterogeneous group of disorders characterized by immune dysregulation, loss of self-tolerance, and chronic inflammation, which leads to tissue damage and organ dysfunction. Current therapies for AIDs are often limited by their lack of specificity, systemic side effects, and insufficient restoration of immune tolerance. Recent advances in nanotechnology and bioengineering have introduced immune and associated cell-derived membrane vesicles (IACMVs) as a promising therapeutic platform. Derived from macrophages, dendritic cells, neutrophils, platelets, or

The immune system is increasingly recognized as playing an important role in the pathogenesis of the neurodegenerative movement disorder Parkinson's disease (PD). In particular, key PD risk genes LRRK2 (which encodes leucine-rich repeat kinase 2) and GBA1 (which encodes glucocerebrosidase) are highly expressed in monocytes and implicated in the regulation of immune inflammatory pathways and lysosomal function. While preclinical studies demonstrate that missense mutations in LRRK2 and GBA1 can modulate the response to inflammatory stimuli, studies in primary immune cells from PD patients harbor

Imagine a world where memories fade, loved ones become strangers, and the sense of self dissolves. This is the harsh reality for nearly 50 million people worldwide suffering from Alzheimer's disease (AD), a progressive neurodegenerative disorder marked by memory loss, cognitive decline, and behavioral changes. With global prevalence on the rise, the demand for effective therapies is urgent. Yet, despite decades of research, a cure remains out of reach, and current treatments offer only temporary relief, failing to address the root causes of the disease. AD's complex pathology, including amyloi

Alzheimer's Disease (AD) is a complex neurodegenerative disorder characterized by progressive cognitive decline and hallmark pathological features, such as amyloid-beta plaques and tau protein tangles. Despite substantial research, current therapeutic strategies remain primarily symptomatic, with limited success in preventing or reversing disease progression. One major challenge is the Blood-Brain Barrier (BBB), which restricts the delivery of therapeutic agents to the brain. Nanotechnology provides innovative solutions to these challenges by enabling the development of targeted drug delivery

Guillain-Barré Syndrome (GBS) is an immune-mediated disorder affecting the peripheral nerves and nerve roots, potentially leading to severe complications such as respiratory failure and autonomic dysfunction. It is commonly preceded by gastrointestinal or respiratory infections, with various bacterial and viral pathogens identified as potential triggers. However, GBS associated with parasitic infections, such as malaria, is a rare occurrence in the medical literature. We present the case of an 18-year-old male who developed progressive bilateral lower extremity weakness, eventually involving t

Alzheimer's disease (AD) is a progressive neurodegenerative disorder. It is characterised by the following: amyloid-β (Aβ) deposition, tau hyperphosphorylation, neuroinflammation and oxidative stress. Unfortunately, there is no curative treatment available. Recently, natural products have attracted growing interest as potential therapeutic agents for AD, thanks to their multi-target actions and favourable safety profiles. This review highlights recent advances in the use of various natural compounds, including flavonoids, phenolic compounds, saponins, terpenoids, alkaloids and coumarins, with

Inflammation and thrombosis are traditionally considered two separate entities of acute host responses to barrier breaks. While inciting inflammatory responses is a prerequisite to fighting invading pathogens and subsequent restoration of tissue homeostasis, thrombus formation is a crucial step of the hemostatic response to prevent blood loss following vascular injury. Though originally designed to protect the host, excessive induction of either inflammatory signaling or thrombus formation and their reciprocal activation contribute to a plethora of disorders, including cardiovascular, autoimmu

BACKGROUND: Acute necrotizing encephalopathy is a rare but severe neurological disorder characterized by rapid onset of fever, altered mental status, seizures, and multifocal brain lesions, particularly involving the thalami and brainstem. Often triggered by viral infections, its pathogenesis involves a hyperinflammatory response, resulting in blood-brain barrier disruption and necrosis of neural tissue. While influenza and herpesviruses are common etiological agents, adenovirus is a less frequently reported cause.

Major Depressive Episode (MDE) is one of the most common psychiatric disorders. Often difficult to treat, this disease is one of the leading causes of suicide. A recent study showed an association between GPR56/ADGRG1 mRNA, MDE and response to antidepressant treatment in blood and in brain. Among GPR56 splicing variant, the S4 isoform has recently been associated with microglial synaptic pruning, while microglia are already known as a central player in MDE. Therefore, we hypothesized that S4 is the specific isoform associated to MDE and antidepressant response. To test our hypothesis, an in si

Celiac disease (CD) is one of the most common autoimmune disorders. It is triggered by exposure to dietary gluten proteins resulting in small intestine mucosal injury. Previous studies showed that CD is highly associated with human leukocyte antigens (HLA) class II DQ heterodimers, mainly HLA-DQ2.5 and HLA-DQ8. The aim of the work was to evaluate the distribution of the CD associated risk HLA-DQ haplotypes in CD patients, CD patients with Type 1 diabetes mellitus (T1DM) comorbidity, in at-risk and healthy individuals in an Egyptian cohort. The study included 124 individuals, divided into 4 gro

Subarachnoid hemorrhage (SAH) is a devastating cerebrovascular disorder with high mortality and long-term disability. It is more prevalent in women than men, but most preclinical research has been performed in male animals. Upregulation of lipocalin-2 (Lcn2), an acute-phase protein involved in iron homeostasis and neuroinflammation, has been implicated in hemorrhagic brain injury in male animals. The purpose of this study was to examine whether genetic deletion of Lcn2 also reduces early brain injury after SAH in female mice. Adult female wild-type (WT) and Lcn2 knockout (KO) mice were subject

Immune thrombocytopenia (ITP) is an immunological phenomenon in which antibodies target platelet surface proteins, resulting in platelet destruction, as well as platelet opsonization and clearance by splenic and hepatic macrophages. Antibodies and cytotoxic T cells may also directly target megakaryocytes in the bone marrow, thereby compromising platelet production and further lowering platelet counts. This disease is largely idiopathic but may also result secondary to autoimmune disorders, malignancies, medication side effects, and recent infections. Currently, the understanding of infections

BACKGROUND: Recurrent fever and acute abdominal pain are common reasons for emergency visits, particularly in adolescents. These symptoms often lead to misdiagnosis and unnecessary surgeries like appendectomy. Familial Mediterranean fever (FMF), a rare autoinflammatory disorder, should be considered in patients with periodic febrile abdominal episodes, especially in those with a migration background.
CASE PRESENTATION: A 19-year-old male of Syrian origin presented with acute right lower quadrant abdominal pain, fever, arthralgia, and pleuritic chest pain. Similar episodes had occurred in the

The development of malaria blood-stage vaccines has been hampered by sequence variation in many Plasmodium falciparum proteins involved in erythrocyte invasion. In the past few years, asparagine-rich merozoite antigen (PfARMA) has emerged as a potential vaccine candidate due to its low amino acid sequence diversity and the association between anti-PfARMA antibody levels and protection to malaria. Here, we used samples from P. falciparum-exposed individuals to study naturally acquired B cell and antibody responses to PfARMA. B cell responses to PfARMA were dominated by IgM+ B cells that recogni

TAR DNA-binding protein 43kDa (TDP-43) dysfunction is an early pathogenic mechanism that underlies amyotrophic lateral sclerosis (ALS), a devastating neurodegenerative disorder that lacks disease modifying therapies. We previously developed a mouse model in which TDP-43 is selectively deleted from motor neurons (ChAT-Cre;Tardbp f/f ) that mimics the early stages of ALS. Here, we demonstrate that intravenous delivery of a blood-brain-barrier (BBB) permeable AAV capsid expressing our rationally designed splicing repressor CTR (AAV-PHP.eB-CTR) in symptomatic ChAT-Cre;Tardbp f/f mice markedly slow

INTRODUCTION: Alopecia areata (AA) is an autoimmune disorder, with potential roles of both genetic and environmental factors. Although AA is a specific autoimmune disease targeting hair follicles, its frequent association with other autoimmune diseases supports the notion that the autoinflammation is not confined to the hair follicles but is systemic. Systemic immune-inflammation index (SII), systemic inflammation response index (SIRI), and CRP (C-reactive protein) albumin ratio (CAR) are elevated in various diseases and correlate with disease severity.
OBJECTIVES: The aim of this study was t

INTRODUCTION: Sarcoidosis is a multisystem chronic inflammatory disorder of unknown etiology that primarily affects the lungs and currently has no cure. Macrophages are central to granuloma formation, and βc cytokines tightly regulate their activation and function. This study investigates the role of the βc receptor in granuloma development and evaluates βc antagonism as a potential therapeutic strategy for sarcoidosis.
METHODS: We utilized an in vitro model of human granuloma formation using sarcoidosis patient human peripheral blood mononuclear cells (PBMCs) and an in vivo vimentin-induced
