Finite Element and Computational Fluid Dynamics Analysis of a Biodegradable Implant for Large Femoral Bone Defects.

The treatment of large bone defects represents a major clinical challenge. Biodegradable metallic 3D-printed scaffolds are promising for orthopedic applications due to their ability to provide sufficient mechanical support while gradually degrading and supporting bone healing. This study investigates the mechanical and fluid transport properties of Fe-10Mn-1Pd scaffolds for femoral fracture repair. The performance of 10 different lattice structures was first evaluated using finite element modeling (FEM) and validated by experimental compression testing, identifying the parallel face-centered c

The use of a biocompatible carbon fiber implant for cervical and lumbar spine fusion in the treatment of spondylodiscitis: a case report.

Osteomyelitis and discitis are rare conditions that predominantly affect immunocompromised patients. Surgical intervention is generally reserved for patients who do not respond to medical therapy or who present with spinal instability, extensive tissue destruction or abscess formation. Carbon fiber implants constitute a notable advancement in the management of these conditions, providing high biocompatibility, radiolucency that facilitates postoperative imaging, a favorable strength-to-weight ratio, enhanced bone healing, reduced complication rates, and a modulus of elasticity comparable to co

Simultaneous Implant Placement and Sinus Lift Using Leukocyte-Platelet Rich Fibrin: A Retrospective 40-Month Study.

The aim of this study was to report the 40-month clinical radiographical outcomes of maxillary sinus floor augmentation using leukocyte-platelet rich fibrin (L-PRF) as a single graft in combination with immediately placed implants. Twenty partially edentulous individuals with missing teeth in the region associated to maxillary sinus were included in this retrospective study. A lateral sinus floor elevation was performed considering a complete wear of the bone wall of the maxillary sinus and immediately placing the implant and filled the maxillary sinus with L-PRF clots as a single graft. After

Trapezoid Fracture Associated with Scaphoid Fracture in a Football Goalkeeper.

INTRODUCTION: Trapezoid fractures are uncommon in sports. We presented a rare case of a trapezoid fracture associated with a scaphoid fracture caused by punching a ball in a football goalkeeper. CASE PRESENTATION: A 19-year-old male who played as a football goalkeeper visited our hospital with complaints of sustained pain from the right wrist to the hand after punching a ball. Scaphoid fracture was diagnosed on plain radiographs, whereas trapezoid fracture was overlooked. Computed tomography revealed a displaced trapezoid fracture associated with a scaphoid fracture. Both fractures were succe

Giant cell tumour of the femoral neck: Failure of curettage-cavity filling cementation with screw fixation, a case report.

BACKGROUND: Giant cell tumours are common primary long bone tumours with femoral neck locations infrequent and notorious for pathological fractures. Treatment with simple curettage often results in local recurrence. Aggressive treatment that combines tumour resection with cement filling and internal fixation aims at preserving native joint function. CASE REPORT: The authors intend to illustrate the short falls of such conservative approach through a case report of a femoral neck giant cell tumour in a 37 year old patient. Patient had undergone curettage-cavity filling with screw fixation for

Associated Reconstruction of Anterior Cruciate and Anterolateral Ligaments With Single Asymmetric Hamstring Tendons Graft.

The anterior cruciate ligament (ACL) rupture is a common disease that accounts for 250,000 cases/year in the United States. The anterolateral ligament (ALL) has been suggested to be an important restraint for rotational instability, and its reconstruction provides a reinforcement to the ACL reconstruction in this aspect, especially in high-demand athletes and in knees with high-grade pivot shift. Different techniques for associated ACL and ALL reconstruction have been described, but the ideal technique remains unclear. Several facts of these techniques may entail a concern to the surgeon, such

Pre-tibial synovial cyst after reconstruction of the anterior cruciate ligament: case report.

Arthroscopic reconstruction of the anterior cruciate ligament has been modernized through new surgical techniques and new materials. When tibial fixation is performed using an absorbable screw, complications may occur, such as formation of a pre-tibial cyst. The case described here is about a patient who presented an anteromedial synovial cyst in his right knee, three years after having undergone ACL reconstruction. The patient did not present any pain nor any complaints other than a mass that progressively increased in size, worsened after physical activities. Imaging examinations were reques

A method using vestibulo-sulcoplasty combining a split-thickness skin graft and a palatal keratinized mucosa graft for peri-implant tissue secondary to oral cancer surgery.

Twelve patients presented with oral submucosal fibrosis and loss of keratinized gingiva in a compromised vestibule of a severely deficient mandibular edentulous ridge secondary to oral cancer surgery. They received implant rehabilitation with a total of 49 fixtures without major bone graft augmentation. To overcome vestibular compromise, soft tissue management consisting of simultaneous vestibulo-sulcoplasty, split-thickness skin graft (STSG), and palatal keratinized mucosa graft (KMG) was performed as a second stage when healing abutment was transferred to replace the cover screw of the denta

[A long-distance runner with a painful sesamoid bone in the forefoot].

A 38-year-old long-distance runner presented with pain in the left medial forefoot. In the presence of such symptoms, consideration should be given to a disease of or injury to a sesamoid bone. Radiology revealed a fracture line through the medial sesamoid bone under the first metatarsophalangeal joint. Conservative treatment was initially ineffective. Ultimately, local injections of lidocaine-methylprednisolone at the site of the pain, in the metatarsophalangeal joint and in the fracture line brought relief. In the diagnosis of patients with pain in the medial forefoot, apart from the patient

Intramedullary nailing of humeral shaft fractures.

The development of interlocking humeral nail systems has greatly broadened the indications for nailing of humeral shaft fracture. Rotational control is better than with earlier nail systems, and most nails have an oblong distal hole that allows axial loading of the fracture site with muscle contraction. When nailing is done with closed technique, loss of the fracture hematoma and periosteal stripping are avoided. Even when open reduction is required, periosteal stripping can be kept to a minimum. Surgical wounds are smaller, even when open reduction is necessary, and when closed nailing is don

[Transcutaneous osseointegration for amputees: foundational principles and current concepts].

Transcutaneous osseointegrated prostheses, which anchor titanium alloy implants into the residual limb bone, offer an alternative for amputees suffering from socket-related complications (occurring in up to 75% of cases). The principle is based on the direct titanium-bone interface, which forms a microscopic gap of approximately 20-50 nm that physically blocks bacteria (typically about 500 nm in diameter); combined with the use of high-purity titanium, this makes percutaneous infection risk much lower than that of traditional implants. Mainstream systems include the hollow-screw design (OPRA),

Comparative finite element analysis: internal fixation plate versus intramedullary nail for neer type III-VI proximal humeral fractur.

BACKGROUND: Finite element analysis (FEA) could advance the understanding of fracture fixation and guide the choice of surgical treatment. This study aimed to compare two internal fixation methods in the treatment of displaced proximal humeral fracture (PHF) through FEA. METHODS: Three-dimensional FEA model based on the left shoulder joint of a 67-year-old female patient with PHFs and osteoporosis was adopted, in order to analyze the fixation effect and load stress distribution of internal fixation plates with open reduction and intramedullary nails without opening the fracture in the treatme

Thoracolumbar Posterior Pedicle Screw Fixation and Laminectomy for an Osteoporotic Vertebral Fracture Resulting From Acute Lymphoblastic Leukemia in an Adult Patient: A Case Report.

Vertebral fractures (VFs) occasionally appear as the first manifestation of acute lymphocytic leukemia (ALL) in children. However, in adults, it is uncommon for VFs to lead to a diagnosis of ALL, and surgical intervention is even rarer. We encountered a case of a 42-year-old man with ALL who presented with acute severe back pain, lower limb numbness, dysuria, and hamstring weakness. A CT scan revealed a burst fracture of the L2 vertebra with spinal canal stenosis. Laboratory results indicated pancytopenia, and an emergency bone marrow biopsy suggested leukemia. Emergency surgery, including lum

Complete Dental Implant Restoration in an Individual With Systemic Sclerosis and Microstomia: A Case Report.

Background: Systemic sclerosis (SS) is a debilitating disease that affects oral and any other tissues including skin, bone, blood vessels, and the connective tissues by excessive collagen accumulation. It is a difficult case for oral rehabilitation, let alone dental implantation. In this regard, only few studies have been conducted. This article reports a case of full-mouth implant-supported prostheses in a SS patient. Case: After diagnosing most remaining teeth as hopeless through clinical and radiographic examinations, implant-based fixed prosthesis for both jaws was planned, considering the

Bone Health Optimization in Adult Spinal Deformity Patients: A Narrative Review.

Osteoporosis and low bone mineral density (BMD) pose significant challenges in adult spinal deformity surgery, increasing the risks of complications such as vertebral compression fractures, hardware failure, proximal junctional kyphosis/failure, and pseudoarthrosis. This narrative review examines the current evidence on bone health optimization strategies for spinal deformity patients. Preoperative screening and medical optimization are crucial, with vitamin D supplementation showing particular benefit. Among the pharmacologic agents, bisphosphonates demonstrate efficacy in improving fusion ra

Comparison of the Bone-Anchored Pendulum Appliance and the Bone-Anchored Intraoral Bodily Molar Distalizer: An Original Research.

INTRODUCTION: With the advent of mini-implants, Class II correction has become routine. The study's goal was to compare the "Bone-Anchored Pendulum Appliance (BAPA)" to the "Bone-Anchored Intraoral Bodily Molar Distalizer (BAIBMD)" in terms of clinical efficacy. MATERIALS AND PROCEDURES: Five boys and five girls were among the 10 patients in this split-mouth trial who had to have their molars distalized. On one side, BAPA Construction, and on the other, BAIBMD was piloted. A titanium mini-screw was used to secure both appliances to the bone since this was a spilled-mouth technique. The first

Development and preclinical evaluation of a cable-clamp fixation device for a disrupted pubic symphysis.

BACKGROUND: Traumatic separation of the pubic symphysis can destabilize the pelvis and require surgical fixation to reduce symphyseal gapping. The traditional approach involves open reduction and the implantation of a steel symphyseal plate (SP) on the pubic bone to hold the reposition. Despite its widespread use, SP-fixation is often associated with implant failure caused by screw loosening or breakage. METHODS: To address the need for a more reliable surgical intervention, we developed and tested two titanium cable-clamp implants. The cable served as tensioning device while the clamp secure
