In the present study, we reported a case of the right aortic arch which was found in the course of dissection at Juntendo University in 1993 from a cadaver of a 92-year-old female. The present case possessed as its terminal branch the right subclavian artery. The ascending aorta arose from the left ventricle and ran upwards, curved dorsally and became the right aortic arch. The microvascular anatomy of the sesamoid complex was investigated in 15 cadaver specimens using histology and tissue clearing (Spalteholz) techniques. Variations in anatomy were found quite frequently during bilateral gonadal vein dissection and resection for pelvic varices in the past 5 years (120 cases). In about one fourth of the cases, routine gonadal phlebography was not technically feasible or did not correlate exactly with the operative findings. MR imaging was performed through the carpal tunnel in 18 wrists of nine normal volunteers and compared with cryomicrotome sections from cadaver wrists. The influence of HLA-A,B and DR matching and pregraft blood transfusions on the graft and patient survival rate of 305 recipients of first cadaver grafts in a single centre has been studied. All three factors significantly influenced the graft survival rate. Experience with 525 kidney transplants performed at the Johannesburg Hospital from 1966 to 1982 is reviewed. The 1-year graft survival rates for HLA-identical related living donor (RLD) transplants, haplo-identical RLD transplants, and first and subsequent transplants from cadaver donors (CDs) are 95%, 72%, 59% and 52% respectively. We examined the possibility that retroplacental source gamma-globulin (RPGG), with its content of anti-HLA antibodies, would improve cadaver kidney graft survival rates. In a 5-year controlled prospective study of 208 transplants, we found that the addition of RPGG to a standard immunosuppressive drug regimen (azathioprine and prednisone) resulted in significant improvement of the cumulative survival rate (CSR) of first and second grafts.
