A case of acute perforation of a diverticulum of the ascending colon is presented. The ultrasound features that helped in making a pre-operative diagnosis are discussed. This rare surgical emergency and the similar clinical entities of perforated caecal or transverse colon diverticula are often clinically misdiagnosed as more common conditions such as acute appendicitis, acute cholecystitis or perforated ulcer. Awareness of this entity will help radiologists in making a correct pre-operative diagnosis.

A comparative analysis of 112 patients' case histories with calculous cholecystitis has revealed that morphological alterations of the gallbladder wall and in the perevesicular area as well as the operative techniques are essential for the wound healing and ultrasound picture in cholecystectomy zone. Ultrasound investigation within postoperative 24 hours enables a significant prognosis of the onset of intestinal paresis, its severity and duration. This is important for conducting early prophylactic measures.

The case of a 20 year old primigesta with a 30 weeks pregnancy, who was admitted to Emergency Service, with acute abdomen by cholecystitis, is presented. At laparotomy, a vesicular wall perforation, was found, partially sealed with epiplon and free biliary fluid in peritoneal cavity. Cholecystectomy was performed; and cesarean section 24 hours after. Therapeutic options in these patients, are analyzed, and surgical treatment risks, during gestation, and otherwise.

We present a case of visceral larva migrans which came as a complete histologic surprise. The patient was operated as a case of chronic cholecystitis and was found at operation to have multiple hepatic nodules. A cholecystectomy with a biopsy of the liver nodule was performed. Histopathologically a diagnosis of visceral larva migrans was made. To our knowledge this is the first case report of the disorder from India. A discussion based on the review of literature is presented.

This study is concerned with the nature and distribution of mineral in the gallbladder of a patient with chronic cholecystitis. Light and electron microscopic imaging revealed the mineral to be in the epithelial cells of the mucosa and fibroblasts of the submucosa. In the epithelial cells at the early stages of deposition, mineral was located in the nuclei and throughout the cytoplasm in association with interdigitating cell processes and apical microvilli but was absent in mitochondria. Elemental and electron diffraction analyses indicated the mineral inclusions to be apatite in nature.

The cholecystectomy by video-laparoscopy has made a revolution in biliary surgery. We present the first 100 cases operated with this technique in acute and chronic cholecystitis. Eighty five percent (85%) with admissions of less than 24 hours, a low morbidity and convalescence period of less than seven days. To perform this procedure is necessary basic training in surgery and endoscopy, the appropriate equipment and team of surgeons.

The case of a 64 year old female who was known to have gallstones is presented. She was admitted to the Hospital following an attack of acute cholecystitis. Ten days after vigorous conservative treatment cholecystectomy was performed. The histological examination showed the presence of the gallbladder leiomyosarcoma. Primary sarcomas of the gallbladder are rare, leiomyosarcoma being the most infrequent type, their preoperative diagnosis almost impossible and their prognosis poor.

The author believes that the organization and quality of emergency aid for patients with acute diseases of the abdominal organs can be improved on the basis of their prophylaxis by means of regular medical examination and planned application of healthful measures in cases with hernias, chronic cholecystitis and peptic ulcer, timely hospitalization of patients in surgical clinics when signs of "acute abdomen" appear, and the introduction of effective forms of health education among the population.

Gallbladder disease, and the patients with symptoms suggestive of gallbladder disease, remains a widespread clinical problem that requires investigation by imaging techniques. Sonography, and to a lesser extent cholescintigraphy, has facilitated the evaluation of these patients and allows the diagnosis of cholelithiasis, and its principal complication of cholecystitis, with a high degree of reliability and accuracy. Interventional techniques are being developed that may provide nonsurgical approaches to improved therapy of gallbladder calculi and their complications.

During elective cholecystectomy due to cholelithiasis and chronic cholecystitis of a 76-year old male patient, a nodular soft tumor of 8 cm. in diameter was found in the caudal region of the pancreas. The tumor was suspected for malignancy during surgery. However, during routine microscopical examination of the biopsy specimen, a large number of Michaelis-Guttman bodies were found, thus establishing the diagnosis of classical malakoplakia.

Two cases of acute pancreatitis in pregnancy occurred among Vietnamese evacuees in Arkansas. In both cases, Ascaris lumbricoides seemed the likely cause. In endemic areas including the rural southeastern United States, a high index of suspicion for ascariasis is needed because these worms may cause a variety of abdominal disorders including pancreatitis, cholecystitis, and bowel obstruction. In appropriate settings, a therapeutic trial with antihelminthics is indicated.

We report an interesting case of a 102-year-old female who presented with a purulent abdominal discharge on the site of a previous percutaneous cholecystostomy in the setting of emphysematous cholecystitis 12 months prior. Imaging demonstrated a cholecystocutaneous fistula (CCF) and a retained calculus along the fistulous tract. On removing the drainage bag, a 2cm calculus was found at the skin level, which was also removed. The patient was discharged with oral antibiotics and found to be symptom-free at the clinical follow-up.

A 78-year-old male with high-risk surgical presented severe acute cholecystitis and required cholecystostomy. The patient was referred later for assessment of the surgical treatment. A cholangio-MRI revealed a lesion on the gallbladder fundus with hepatic lesions suggestive of metastatic gallbladder carcinoma, which was confirmed in the histological analysis. The tumor progressed despite the chemotherapy through the cholecystostomy tract and developed peritoneal carcinomatosis. The patient did not respond to chemotherapy and he died 12 months later.

Porcelain gallbladder or calcified gallbladder is a rare entity and is considered as the end stage of chronic cholecystitis. This disease is rarely diagnosed preoperatively and usually mimics carcinoma gallbladder. Hereby, we present a rare and interesting case of porcelain gallbladder that was diagnosed preoperatively and managed by cholecystectomy. Gallbladder tuberculosis (GT) is extremely rare, and it is difficult to differentiate from other gallbladder diseases, such as gallbladder carcinoma and Xanthogranulomatous Cholecystitis. A correct preoperative diagnosis of GT is difficult. The final diagnosis is usually made postoperatively according to surgical biopsy. Here, we report a case of a patient who underwent surgery with the preoperative diagnosis of gallbladder carcinoma. We reviewed the literature and present the process of differential diagnosis between two or more conditions that share similar signs or symptoms.

Torsion of the gallbladder resulting in a volvulus is a rare clinical finding that poses a diagnostic challenge preoperatively to both surgeons and radiologists. It is thought to occur secondary to a redundant mesentery where rotation of the gallbladder occurs along the axis of the cystic duct and cystic artery. Gallbladder volvulus commonly presents as acute cholecystitis and is rarely diagnosed preoperatively. An early emergency laparoscopic cholecystectomy is the preferred treatment. This is a case report of a 60 year-old male patient with acute cholecystitis and jaundice, unaware of having situs inversus. Patient underwent successfully laparoscopic cholecystectomy with bile duct exploration in spite of the unusual clinical situation. A literature review using Pubmed database, since 1989, in Spanish and English literature was performed. We found only two reports of cholecystectomy and common bile duct exploration in patients with situs inversus. This is the first case reported in our country.

Porcelain gallbladder (PG) is a rare entity. It's frequently associated with cholecyst lithiasis (90%). Diagnosis is established based on imaging assays (simple abdominal radiography, ultrasound and/or abdominal CT). Pathologically is an extensive process of calcification of the gallbladder wall. The literature mentioned strongly PG association with cancer of the gallbladder. Therefore laparoscopic cholecystectomy was formally contraindicated. We present two cases of PG laparoscopically operated and with this occasion we review particular aspects of diagnosis and treatment of the disease.

A 79-year-old previously healthy man presented with acute acalculous cholecystitis with obstruction of the biliary tract. He was successfully treated with antibiotics and percutaneous transhepatic gallbladder drainage, but returned to the hospital two days after discharge with a rare complication of this technique, biliopleural fistula. A thoracostomy tube was inserted to drain the pleural effusion, and the patient's previous antibiotics reinstated. After two weeks of drainage and antibiotics, the fistula healed spontaneously without the need for further intervention.

Torsion of greater omentum is a rare cause of acute abdomen. However, it should be included in the differential diagnoses in addition to acute cholecystitis, acute appendicitis, cecal diverticulitis, and other variable causes of acute abdomen. Diagnosis is usually made at laparotomy for suspected appendicitis. In some cases, computed tomography demonstrates a successful preoperative detection of omental torsion. We report a case of surgically and pathologically proven torsion with subsequent infarction of greater omentum presented as an acute abdominal pain.
