Changing landscape of 16 S-RMTase-mediated aminoglycoside resistance in clinical Klebsiella pneumoniae isolates from Chile between 1997 and 2023.

The emergence of 16S rRNA methyltransferase (16S-RMTases) poses a major therapeutic challenge, as they confer high-level resistance to all clinically relevant aminoglycosides. In Chile, data on these enzymes remain scarce. This study investigated the epidemiological distribution and genetic relatedness of Klebsiella pneumoniae strains carrying 16S-RMTase genes. Antibiotic susceptibility was assessed using the Kirby-Bauer method and complementary techniques. Gene detection was performed via conventional PCR, and genotyping via ERIC-PCR. Among 1,200 K. pneumoniae clinical strains collected betwe

Volumetric MRI of dorsal root ganglia as a biomarker for disease progression and response to AAV treatment in a mouse model of Fabry disease.

Noninvasive and objective biomarkers for disease-associated pathology are critical for clinical trials. For Fabry disease, one important pathological change due to the deficiency of the lysosomal enzyme α-galactosidase A (α-GAL) caused is accumulation of globotriaosylceramide (Gb3) in dorsal root ganglion (DRG) neurons, which manifests as the overall DRG hypertrophy. Magnetic resonance imaging (MRI) has been successfully used to noninvasively measure DRG enlargement in Fabry patients, and DRG volumetric MRI can be a potential noninvasive biomarker for Gb3 accumulations in DRG neurons in clinic

Small fiber neuropathy in pediatric female heterozygotes of Fabry disease: a twin case report.

BACKGROUND: Fabry disease, an X-linked lysosomal storage disorder, is associated with significant multisystem involvement, including small fiber neuropathy. While neuropathic pain is well-documented in females with Fabry disease, the objective confirmation of small fiber dysfunction in pediatric heterozygotes remains underreported. This study highlights the role of quantitative sensory testing (QST) in diagnosing small fiber neuropathy in young female patients. CASE REPORT: We present twin sisters, heterozygous for a pathogenic GLA variant (c.298 A > T), who exhibited symptoms of small fiber

VEXAS syndrome: Current clinical, diagnostic and treatment approaches.

VEXAS syndrome, is a hemato-inflammatory chronic disease characterized with predominantly rheumatic and hematologic systemic involvement. It was first described in 2020 by a group of researchers in the United States. VEXAS syndrome is a rare condition that primarily affects adult males and is caused by a mutation in the UBA1 gene located on the X chromosome. Its pathogenesis is related to the somatic mutation affecting methionine-41 (p.Met41) in UBA1, the major E1 enzyme that initiates ubiquitylation. Mutant gene lead to decreased ubiquitination and activated innate immune pathways and systemi

Hereditary Hemochromatosis: Rapid Evidence Review.

Hereditary hemochromatosis is an autosomal recessive disorder that disrupts iron homeostasis, resulting in systemic iron overload. It is the most common inherited disorder among people of northern European ancestry. Despite the high prevalence of the gene mutation, there is a low and variable clinical penetrance. The deposition of excess iron into parenchymal cells leads to cellular dysfunction and the clinical manifestations of the disease. The liver, pancreas, joints, heart, skin, and pituitary gland are the most commonly involved organs. Hereditary hemochromatosis is usually diagnosed in th

Prevalence and clinical correlates of rheumatoid factor and anticitrullinated protein antibodies in patients with idiopathic inflammatory myopathy.

OBJECTIVE: As rheumatoid factor (RF) and anticitrullinated protein antibodies (ACPAs) are not routinely tested in idiopathic inflammatory myositis (IIM), little is known about their prevalence and clinical implications in this patient group. In antisynthetase syndrome (ASS), presence of ACPA is reportedly associated with more severe and erosive arthritis. We aim to retrospectively determine the prevalence of RF and ACPA in a cross-sectional cohort of 121 patients diagnosed with IIM and to assess clinical associations. METHODS: Serum samples from 121 patients diagnosed with polymyositis (n=30)
