Total hip arthroplasty after hip arthrodesis: functional outcomes, and mid-term survivorship integrating the fusion technique complexity score.

BACKGROUND: Conversion of hip arthrodesis to total hip arthroplasty (THA) is technically demanding because of altered anatomy, soft-tissue contractures, and long-standing biomechanical adaptations. Preoperative complexity varies widely, and contemporary data on functional recovery and complications remain limited. This study evaluates functional and radiographic outcomes after conversion THA and introduces a Fusion-to-THA Surgical Complexity Score (FTCS) to characterise preoperative difficulty and explore its association with perioperative outcomes. PATIENTS AND METHODS: We retrospectively re

A Telerehabilitation Intervention for Youths With Arthrogryposis Multiplex Congenita: Protocol for a Pilot Study.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is characterized by joint contractures present in at least two body areas. In addition to these contractures, individuals with AMC can have decreased muscle mass, leading to limitations in activities of daily living. Exercise has the potential to maintain or improve the range of motion and muscle strength. However, this type of intervention necessitates frequent follow ups that are currently difficult to provide for youths with AMC because they often live far from a specialized hospital. To overcome this distance challenge, telecommunication

Drosophila myosin mutants model the disparate severity of type 1 and type 2B distal arthrogryposis and indicate an enhanced actin affinity mechanism.

BACKGROUND: Distal arthrogryposis (DA) is a group of autosomal dominant skeletal muscle diseases characterized by congenital contractures of distal limb joints. The most common cause of DA is a mutation of the embryonic myosin heavy chain gene, MYH3. Human phenotypes of DA are divided into the weakest form-DA1, a moderately severe form-DA2B (Sheldon-Hall Syndrome), and a severe DA disorder-DA2A (Freeman-Sheldon Syndrome). As models of DA1 and DA2B do not exist, their disease mechanisms are poorly understood. METHODS: We produced the first models of myosin-based DA1 (F437I) and DA2B (A234T) us

Removable Rigid Dressings for Postoperative Management of Transtibial Amputations: A Review of Published Evidence.

UNLABELLED: Forty years of clinical experience and peer-reviewed research studies support the use of nonweight-bearing removable rigid dressings (RRDs) as an effective means of postoperative management of transtibial amputations. We reviewed the published medical evidence regarding the use of RRDs as a postoperative management strategy, culminating in an evidence-based practice recommendation. Published peer-reviewed literature on the topic was searched and classified by level of evidence based on the research design using the scale recommended by the PM&R (level I through V). The search uncov

Unidirectional barbed suture versus standard monofilament for urethrovesical anastomosis during robotic assisted laparoscopic radical prostatectomy.

PURPOSE: V-Loc™180 (Covidien Healthcare, Mansfield, MA) is a new unidirectional barbed suture that may reduce loss of tension during a running closure. We evaluated the use of the barbed suture for urethrovesical anastomosis (UVA) during robotic assisted laparoscopic prostatectomy (RALP). Time to completion of UVA, post-operative anastomotic leak rate, and urinary incontinence were compared in patients undergoing UVA with 3-0 unidirectional-barbed suture vs. 3-0 Monocryl™ (Ethicon, Somerville, NJ). MATERIALS AND METHODS: Data were prospectively collected for 70 consecutive patients undergoing

Results of open arthrolysis for elbow stiffness. A series of 22 cases.

Severe posttraumatic elbow stiffness represents a significant invalidity. Between 1990 and 2005 two surgeons performed open elbow arthrolysis in 30 adult patients (6 women, 24 men, mean age 30.8 years). All cases resulted from severe initial trauma, which had occurred on average 15.5 months previously. Four patients had extrinsic and 18 had mixed contractures; 13 had heterotopic ossifications. Operative complications included two peroperative joint instabilities and 3 transient nerve palsies. Seven elbows were remobilized under anaesthesia, one month after the arthrolysis. Twenty-two patients

[Congenital unilateral muscular hyperplasia of the hand - a rare malformation].

This is a report on eight cases of a rare congenital malformation in the upper extremity, consisting of a unilateral muscular hyperplasia. In addition to the hand, all segments of the upper extremity may be affected. The hyperplasia is always unilateral, preferably on the right hand side, in combination with accessory muscles. Hereditary dependence or association with other malformations has not been observed. Six of eight patients were male. Shoulder and arm function were normal in all cases. Ulnar drift of the fingers in the metacarpophalangeal joints (six of eight patients), flexion contrac

Management of Dupuytren disease of the little finger.

BACKGROUND: The surgical management of Dupuytren disease (DD) is associated with a high rate of complications. Recurrences are relatively common and may result in permanent disability, particularly when the little finger (LF) is involved. This study aims to provide both objective and subjective information, along with professionals' experiences. METHODS: A questionnaire survey, comprising both open and closed questions, was distributed to hand surgeons, physiotherapists, and occupational therapists engaged in the management of DD across five continents. The involvement and role of the LF in D

H syndrome presenting with bilateral cheek enlargement and an SLC29A3 gene variant.

An adolescent girl presents with bilateral cheek enlargement, hyperpigmentation and hypertrichosis of the lower extremities with otological, cardiac, endocrine, and hepatosplenic involvement. Clinical findings supplemented by histopathological and wide exome sequencing results led to the diagnosis of H syndrome. The genetic testing showed a homozygous frameshift mutation in the SLC29A3 gene involving unique exon and codons. This case highlights the unique characteristics of H syndrome observed in a Filipino female with a variant of the SLC29A3 gene mutation.

Revision for stiff knee after knee replacement.

Stiffness following total knee replacement is defined as >15° flexion contracture and/or flexion <75° or, for other authors, arc of motion <70° or 45° or 50°. Alternatively, it could be defined as a range of motion less than the patient needs in order to be able to do what they wish. The first step in management is to determine the causes: preoperative (history of stiffness, patient-related risk factors, etc.), intraoperative (technical error: malpositioning, oversizing, overhanging, etc.), and postoperative (defective pain management and/or rehabilitation, etc.). Treatment depends on the inte

[Treatment of Dupuytren's disease; limited fasciectomy versus percutaneous needle fasciotomy].

OBJECTIVE: We estimated retreatment risk following limited fasciectomy (LF) or percutaneous needle fasciotomy (PNF) for Dupuytren's disease. DESIGN: Prospective cohort study. METHOD: Using frailty models, we estimated retreatment risk within 1,3,5,10 and 20 years. RESULTS: LF showed a 10-year retreatment risk of 8% (95%CI: 3-13%) for men and 4% (95%CI: 1-8%) for women without an affected first-degree relative, initially treated at 60 years. PNF showed higher risks at 69% (95%CI: 52-86%) and 46% (95%CI: 16-77%). Younger age at initial treatment increased retreatment risk, sex and first-degre

Tumour stage and overall survival in patients with intrahepatic cholangiocarcinoma and primary sclerosing cholangitis - a retrospective cohort study.

BACKGROUND: Patients with primary sclerosing cholangitis (PSC) carry increased risks for malignancy, among which cholangiocarcinoma (CCA) is the most frequent. We aimed to characterise a cohort of patients with PSC and intrahepatic CCA (iCCA) and to compare this cohort with CCA in different localisations. METHODS: We performed a retrospective analysis of our medical database from 01.01.2007 to 30.06.2023 and differentiated CCA according to its localisation within the biliary tract into iCCA, perihilar CCA (pCCA), distal CCA (dCCA), and gallbladder carcinoma (GBC). RESULTS: We identified 8 (2

Modifiable Risk Factors for Prevention in Dupuytren Disease: A UK Biobank Case-Control Study.

BACKGROUND: Dupuytren disease is associated with significant comorbidity and mortality, and it has no existing prevention strategies. It is unclear which modifiable risk factors are most amenable for prevention. This study aimed to determine the strength of modifiable risk factors for Dupuytren disease, and to investigate associations with other diseases. METHODS: Using UK Biobank data, this case-control study analyzed the association between phenotypic variables and Dupuytren disease through multivariable logistic regression. Exposures assessed were age, sex, body mass index, waist-to-hip ra

Reoperative Augmentation Mammoplasty: An Algorithm to Optimize Soft-Tissue Support, Pocket Control, and Smooth Implant Stability with Composite Reverse Inferior Muscle Sling (CRIMS) and its Technical Variations.

BACKGROUND: Reoperative augmentation mammoplasty (RAM) is a challenging procedure, with the highest rates of complications and revision. Complications include implant malposition, lateral displacement, bottoming out, and rotation. These deformities can be addressed with various procedures, but the pocket control and stability of the new smooth implant surface may present limitations. OBJECTIVES: This study revisits a previously described predictable approach in primary breast augmentation and defines a surgical treatment algorithm for RAM technique selection. METHODS: Between 2017 and 2021,

The "Wind Surf" Deformity-Correcting Fixed Flexion Deformity and Hyperextension in Bilateral CAS TKA.

Severe knee arthritis can result in complex coronal and sagittal angular deformities. Windswept deformity is used to describe a varus deformity and contralateral valgus deformity. We recognized a new sagittal pattern at the time of computer-assisted surgery (CAS) in total knee arthroplasty (TKA) in which one knee has a fixed flexion deformity (FFD), while the contralateral knee has a hyperextension deformity. We propose to define it as "wind surf" deformity mimicking the opposite pull of the wind and a surfer. The incidence of "wind surf" deformity in this series was 0.96% among a cohort of 2,

Predictive factors and clinical effects of diabetic hand: A prospective study with 1-year follow-up.

AIMS: Diabetes mellitus is considered an etiological factor for hand-related conditions that are grouped under the term "diabetic hand" (DH), which includes limited joint mobility, Dupuytren's contracture, carpal tunnel syndrome, and trigger finger. This study aimed to identify predictive factors and the clinical effects of DH development among patients with diabetes. PATIENTS AND METHODS: Consecutive Japanese adults with diabetes were prospectively recruited at a single outpatient center. We assessed the presence of DH at baseline and at the 1-year follow-up, which was considered present if

A comparison of patient-reported outcome measures for dupuytren disease: A prospective view.

Treatment success in Dupuytren´s disease has traditionally been assessed by clinical examination and physical measures, but based on patient orientation, patient-reported outcome measures (PROMs) can be used to quantify treatment impacts and health-related quality of life. The aim of this study was to compare the most widely used PROMS in Dupuytren´s disease, their psychometric properties, and their association with objectively measured hand function. We show a prospective study in which two disease-specific questionnaires (URAM scale and SDSS) and two hand-specific questionnaires (briefMHQ an
