Risk factors for persistent numbness following decompression surgery for lumbar spinal stenosis.

OBJECTIVE: Decompression surgery is a mainstay of surgical treatment for lumbar spinal stenosis (LSS). However, up to 30% of patients have low satisfaction due to residual symptoms. In the clinical setting, improvements in leg pain are more significant than those in leg numbness. Residual numbness could be related to the relatively low satisfaction rate. However, few studies have focused on numbness; thus, elucidating the risk factors and rate of residual numbness would benefit surgeons and patients. This study aimed to clarify the risk factors for and rate of residual numbness after decompres

Revision lumbar surgery in elderly patients with symptomatic pseudarthrosis, adjacent-segment disease, or same-level recurrent stenosis. Part 2. A cost-effectiveness analysis: clinical article.

OBJECT: Despite advances in technology and understanding in spinal physiology, reoperation for symptomatic adjacent-segment disease (ASD), same-level recurrent stenosis, and pseudarthrosis in elderly patients continues to occur. While revision lumbar surgery is effective, attention has turned to questions on the utility and value of the revision decompression and fusion procedure. To date, an analysis of the cost and health state gain associated with revision lumbar surgery in elderly patients with symptomatic pseudarthrosis, ASD, or same-level recurrent lumbar stenosis has yet to be performed

[Surgical management and prognosis of iatrogenic peripheral facial nerve injury following middle ear surgery].

OBJECTIVE: To discuss the causes, sites, management strategies and curative effects of accidental facial nerve paralysis in the middle ear surgery. METHODS: Forty two cases with peripheral facial nerve paralysis following middle ear surgery who underwent surgical exploration and reanimation were analyzed. Facial nerve decompression, primary end-to-end anastomosis, interpositional nerve grafts with the great auricular nerve and nerve substitution of facial-hypoglossal anastomosis were applied to restoration of the facial nerve function. The facial nerve function was graded according to House-B

An Application of Combined Transpetrosal Approach With Microneurovascular Transposition in Recurrent Trigeminal Neuralgia Caused by Dolichoectatic Vertebral Artery.

BACKGROUND: In general, the superior cerebellar artery is the most prevalent vessel that compresses the trigeminal nerve root exit zone (TGNREZ) and is responsible for trigeminal neuralgia (TGN). Nevertheless, the surgical outcome is less efficacious when the offending vessel is a dolichoectatic vertebrobasilar vessel (DVB). In addition, the potential for postprocedural adhesion and fibrosis renders recurrent TGN after prior surgery, an extremely challenging operation. METHODS: We describe our surgical strategy and procedure that effectively resolved recurrent TGN in a patient whose offending

Percutaneous intervention in portosystemic shunts in Budd-Chiari syndrome.

Budd-Chiari syndrome is a rare, often fatal illness resulting from hepatic venous outflow obstruction. Surgically created portosystemic shunts are frequently necessary in these patients for portal decompression. Over the past 15 years, 30 patients have been surgically treated for this condition at the Johns Hopkins Hospital. Of the 22 long-term survivors (range, 6 months to 13 years), five (23%) have required further surgical intervention and five (23%) have required percutaneous intervention for shunt complications. Percutaneous procedures included angioplasty (n = 10), atherectomy (n = 1), a

Impact of prior interventional treatment on the complications after Frey procedure for chronic pancreatitis.

PURPOSE: Chronic pancreatitis is an inflammatory disease responsible for pain partially explained by pancreatic duct dilatation. Early surgery has become the treatment of choice for hypertrophic pancreatic head with main pancreatic duct dilatation. Frey procedure (FP), combining both surgical resection and decompression, is one of the standard surgical procedures. However, a "step-up approach" with endoscopic or limited surgical procedures is still frequently proposed before referring to expert pancreatic centres. The aim of the study was to evaluate the impact of a prior treatment on post-ope

[Compartment syndrome of the leg secondary to a muscle tear in an anticoagulated elderly patient].

Compartment syndrome is a clinical-surgical condition characterized by an increase in intracompartmental pressure that leads to progressive tissue damage and can have devastating consequences if not treated promptly. High-energy trauma is the most common cause, though in frail and anticoagulated patients, it can arise from daily activities. We present the case of a 63-year-old man with multiple comorbidities and anticoagulant therapy who developed compartment syndrome secondary to a muscle tear and required decompressive fasciotomy. Early suspicion of compartment syndrome in anticoagulated pat

Consensus guidelines for the management of primary central nervous system lymphoma for low and middle-income countries.

Primary lymphoma of the central nervous system (PCNSL) is a rare and aggressive form of extranodal non-Hodgkin lymphoma primarily involving the brain, spinal cord, cerebrospinal fluid, and eyes. The role of surgical intervention in PCNSL is currently limited to biopsy and decompression of critical structures if needed - extended resection is debated. Chemotherapy is the mainstay of treatment. In lower and middle-income countries (LMICs), issues like delayed diagnosis and resource constraints are widespread. These guidelines provide a framework for addressing PCNSL in LMICs, emphasizing the imp

Abdominal Compartment Syndrome Secondary to Constipation in an Adult Patient With Cerebral Palsy.

The majority of patients with constipation can often be treated conservatively with laxatives, suppositories, or enemas in mild cases. However, endoscopic decompression or surgical intervention may be required in some instances. Abdominal compartment syndrome as a result of constipation is rarely seen in the literature. We report a case of faecal impaction, which led to abdominal compartment syndrome in an adult patient with cerebral palsy. With increasing life expectancy, such cases may be increasingly encountered in the adult population. Severe complications of constipation should not be ove

Acute Basilar Artery Occlusion and Death Secondary to Bilateral Vertebral Artery Dissection.

Vertebral artery dissection as a cause of basilar artery thrombosis is an exceedingly rare event that is associated with significant morbidity and poor outcomes. We present an unusual case of bilateral vertebral artery dissection and spinal cord compression in a 21-year-old male involved in a diving accident. The patient received limited antithrombotic therapy in pursuit of surgical spinal decompression, ultimately contributing to thrombosis of the basilar artery in the post-operative period and death following anterior cervical discectomy and fusion. Our goal is to highlight the severity of v

[Paracentesis as abdominal decompression therapy in neuroblastoma MS with massive hepatomegaly].

Neuroblastoma MS with massive hepatomegaly is a small percentage of cases of neuroblastoma. It is more common in infants less than 4-6 weeks of life, and involves, in contrast to the standard of the NB MS, poor prognosis given the complications that can have. In the case of abdominal compartment syndrome it is recommended a quick start of chemotherapy, associating or not radiation therapy, to try to reduce the size of the liver, and if necessary, decompressive laparotomy. We present the case of a patient with NB MS, massive hepatomegaly and threatening symptoms for life, in which the surgical

Potts shunt in patients with primary pulmonary hypertension.

Idiopathic pulmonary arterial hypertension eventually leads to right-sided heart failure and sudden death. Its mortality rate in children is still high, despite improvements in pharmacological therapy, and therefore novel treatments are necessary. The Potts shunt, which creates an anastomosis between the left pulmonary artery and the descending aorta, has been proposed as a theoretically promising palliative surgical technique to decompress the right ventricle. We report the case of a 12-year-old girl with suprasystemic idiopathic pulmonary hypertension and right ventricular failure who underw

The cheilectomy and its modifications.

Hallux rigidus occurs in 10% of persons aged 20 to 34 years but in as much as 44% of people older than 80 years. Surgical intervention has been suggested for cases of hallux rigidus that have failed using conservative methods. The modified cheilectomy is considered by many the first-line treatment for this disease, given the procedure's inherent ability to eliminate degenerate bone and cartilage and decompress the intra-articular space, while sparing considerable cubic content of bone. Once the cheilectomy has been performed, there remains a sufficient volume of bone to perform a more definiti

Management of intracranial hypertension.

Effective management of intracranial hypertension involves meticulous avoidance of factors that precipitate or aggravate increased intracranial pressure. When intracranial pressure becomes elevated, it is important to rule out new mass lesions that should be surgically evacuated. Medical management of increased intracranial pressure should include sedation, drainage of cerebrospinal fluid, and osmotherapy with either mannitol or hypertonic saline. For intracranial hypertension refractory to initial medical management, barbiturate coma, hypothermia, or decompressive craniectomy should be consid

Improved tolerability of open carpal tunnel release under local anaesthetic: a patient satisfaction survey.

Carpal tunnel decompression is the most commonly performed procedure in hand surgery. This study was done to assess the effectiveness and acceptability by patients of open carpal tunnel release under local anaesthetic. We compare our results with both the literature and those of our previous study, after which were made alterations to improve the surgical and anaesthetic technique. These alterations included (a) injecting local anaesthetic with a dental syringe and a 27G needle in two stages along the proposed line of the incision and (b) omitting the use of a tourniquet, relying on the local

En bloc resection for primary non-Hodgkin's lymphoma of the descending colon with massive extension into the neighboring organs.

A 79-year-old female who was surgically treated and received adjuvant chemotherapy for primary non-Hodgkin's lymphoma of the descending colon with massive extension into the pancreatic tail, spleen, and left kidney is herein reported. The patient had acute colonic obstruction and an urgent laparotomy was performed because endoscopic decompression using a transnasal ileus tube could not be done. We performed a left colectomy with an en bloc resection of the neighboring organs, followed by the creation of an end colostomy. The patient had no major postoperative complications and is well without

[Problems in the diagnosis and surgical treatment in an osseous tuberculosis hospital].

The diagnosed patients treated at an osseous tuberculosis hospital are analyzed. In the reporting period, the diagnosis of osseous tuberculosis has not been verified. Late detection of osseous tuberculosis, as its prehospital hyperdiagnosis is one of the main reasons of a grave condition in this group of patients. The paper characterizes plastic reparative versus palliative decompressive operations for tuberculous spondylitis with spinal-cord abnormalities and shows that radical surgery has a higher efficiency (89%) in the treatment of an inflammatory process and in the regression of neurologi

Endoscopic carpal tunnel release for recurrent carpal tunnel syndrome after previous open release.

Recurrent carpal tunnel syndrome from various causes has been shown to occur in up to 19% of patients. Endoscopic carpal tunnel release has been used to decompress the median nerve in carpal tunnel syndrome for many years. However, endoscopic release for recurrent carpal tunnel syndrome after previous surgical release has not been reported. Nine hands in six patients had recurrent carpal tunnel syndrome five to 20 years after previous open carpal tunnel release. All the cases were successfully treated with endoscopic release.

Pediatric neuroanesthesia. Hydrocephalus.

Hydrocephalus, one of the most common adult and pediatric neurosurgical disorders, is an abnormal accumulation of cerebrospinal fluid in the ventricular system as a result of obstruction to the flow of cerebrospinal fluid. Causes of hydrocephalus include congenital obstruction, hemorrhage, infection, cysts and tumors, and associated neural tube deformities (i.e., myelomeningocele, Arnold-Chiari malformation). Treatment of hydrocephalus involves surgical implantation of shunt systems to drain the cerebrospinal fluid. Anesthetic considerations involve attention to the possibility of increased in

Modified sacrospinalis muscle pedicle bone graft for fusion of isthmic spondylolisthesis.

We treated 20 patients with symptomatic L4-5 isthmic spondylolisthesis (ten grade I, nine grade II and one grade III) surgically. All patients complained of low back pain and nine had additional radicular pain. Bilateral posterolateral fusion using the sacrospinalis muscle as a muscle pedicle bone graft was carried out in all patients. Decompression was added in patients with radiculopathy. Follow-up ranged from 12 to 28 months. Pain relief was graded as excellent in 18 patients, radiographic fusion as excellent in 17 patients and functional outcome as excellent in 19 patients.
