Long-term treatment of involuntary facial spasms using botulinum toxin.

Botulinum toxin, a powerful pre-synaptic neurotoxin produced by Clostridium botulinum, interferes with the release of acetylcholine from nerve terminals. Since September 1985, we have been using this toxin to treat altogether 62 patients with benign facial spasms. Most of the patients had been on drugs or psychotherapy, 2 had received alcohol injections, 2 had undergone surgery of the orbicular branch, and 2 electrocoagulation of the facial nerve. In essential blepharospasm the duration of the beneficial effect after each treatment with botulinum toxin was about 3 1/2 months. In patients with

Training skills in the psychiatrically disabled: learning coping and competence.

Social skills training methods represent a major strategy for psychiatric rehabilitation. Building skills in patients with schizophrenic and other major mental disorders is based on the assumption that coping and competence can override stress and vulnerability in reducing relapses and improving psychosocial functioning. For maximum efficiency, skills training needs to incorporate procedures and principles of human learning and information processing. Several models for skills training have been designed and evaluated, each of which has proved to be effective in raising the social competence o

Implications of nutritional status on human biochemistry, physiology, and health.

Optimum nutrition is the level of intake that should promote the highest level of health. Although excess caloric intake will lead to obesity, a deficit in nutrition may result in a tissue depletion of essential nutrients that can lead to biochemical changes and eventually to clinical signs and symptoms. Nutrition requirements may differ according to sex, age, activity, or physiological state and can be influenced by drugs, smoking, alcohol, and other factors. With ever-increasing sedentary life styles and less physically demanding jobs, the resulting reduced caloric requirements have made it

Approaches to the treatment of chronic hepatitis B viral infection.

In HBeAg positive patients with a high level of viral replication, antiviral therapy is the treatment of choice. The most promising agents at the moment are ARA-AMP and Interferon and both are being assessed in controlled clinical trials. In the anti-HBe positive patients in whom continued HBs antigenaemia is due to the presence of clones of cells containing integrated virus, some form of immune manipulation may be necessary. In rare cases in whom continuing inflammatory activity is related to an autoimmune reaction, low dose prednisolone may be beneficial. Treatment for delta infection has ye

Symptoms and early management of germinal tumours of the testis.

The symptomatology and delay in diagnosis in a series of 502 germinal tumours of the testis were surveyed. Pain is much more commonly a feature than is usually appreciated and often leads to the condition being treated as inflammatory for much longer than it should be. Most surgical textbooks are not helpful in this regard. Delay in exploring the testis, and, particularly, delay in initiating follow-up therapy after orchidectomy, were associated with a higher stage of disease and poorer results, most clearly seen in nonseminomatous tumours. As the incidence of testicular tumours seems to be ri

MDCL: Multi-source Information Fusion and Collaborative Contrastive Learning for Inferring miRNA-Drug Sensitivity and Resistance Associations.

A growing body of evidence indicates that microRNAs (miRNAs) play essential roles in cancer progression, therapy, and drug resistance. They are increasingly recognized as promising therapeutic targets for overcoming cancer drug resistance. However, existing computational methods for predicting miRNA-drug associations are often limited by sparse labeled associations, reliance on single-modality data, and insufficient ability to capture long-range dependencies in sequences. In this study, we propose a novel deep learning framework, named MDCL, that integrates multi-source information fusion with

Is Short Therapy an Appropriate Regimen for Children and Young Adolescents with Drug-Susceptible Tuberculosis?

Background: Tuberculosis (TB) remains a major cause of morbidity and mortality among children worldwide, with approximately one million new pediatric cases annually. The conventional treatment for drug-susceptible TB has long relied on a 6-month multidrug regimen, which is highly effective but associated with challenges in adherence, toxicity, and healthcare burden. Objectives: To evaluate whether short-course therapy is an appropriate regimen for children and young adolescents with drug-susceptible TB, with particular focus on its efficacy, safety, and applicability in different clinical cont

Target-controlled dialysis for antibiotics (TCD-ABx).

BACKGROUND: Effective antimicrobial therapy is an essential part of intensive care medicine and renal replacement therapy is an important and common intervention which significantly affects the pharmacokinetics of many antimicrobials. This is especially critical for substances with a narrow therapeutic range, creating a dilemma of weighing the risk of toxicity from increased drug exposure against risk of ineffective treatment and promotion of antimicrobial resistance. To address this problem, we investigate a target-controlled dialysis by in vitro experiments - a novel technique in which drug

Advances in Computational Methods to Discover New NS2B-NS3 Inhibitors Useful Against Dengue and Zika Viruses.

The Flaviviridae virus family consists of the genera Hepacivirus, Pestivirus, and Flavivirus, with approximately 70 viral types that use arthropods as vectors. Among these diseases, dengue (DENV) and zika virus (ZIKV) serotypes stand out, responsible for thousands of deaths worldwide. Due to the significant increase in cases, the World Health Organization (WHO) declared DENV a potential threat for 2019 due to being transmitted by infected travelers. Furthermore, ZIKV also has a high rate of transmissibility, highlighted in the outbreak in 2015, generating consequences such as Guillain-Barré sy

In Vivo Gene Expression Profile of Human Intestinal Epithelial Cells: From the Viewpoint of Drug Metabolism and Pharmacokinetics.

Orally administered drugs are absorbed and metabolized in the intestine. To accurately predict pharmacokinetics in the intestine, it is essential to understand the intestinal expression profiles of the genes related to drug absorption, distribution, metabolism, and excretion (ADME). However, in many previous studies, gene expression analysis in the intestine has been carried out using specimens from patients with cancer. In this study, to obtain more accurate gene expression profiles, biopsy samples were collected under endoscopic observation from the noninflammatory regions of 14 patients wit

From toxic precursors to safe drugs. Mechanisms and relevance of idiosyncratic drug reactions.

In adverse drug reactions, effects accounted for by the pharmacological profile of the drug may be distinguished from sometimes bizarre, unpredictable events ("idiosyncrasies"). In the majority of cases, the latter are due to reactive intermediates formed by members of the cytochrome P450 family of enzymes. These products are usually formed at very low quantities. However, genetic disposition (e.g. by enhanced expression of the catalyzing enzyme or failure of protective factors such as glutathione) or certain external conditions (for example comedication of inducers of drug metabolism) may lea

Medication Status and Related Factors in Essential Tremor Patients: A Cross-Sectional Study in China.

INTRODUCTION: Essential tremor (ET) is one of the most common movement disorders. Oral drugs play a crucial role in treating ET, with various available options such as propranolol, primidone, and topiramate. However, the medication status and related factors among Chinese ET patients are unknown yet. METHODS: This study used the baseline data from the National Survey of Essential Tremor Plus in China cohort. ET patients with information related to medication intake were included. Medication patients were defined as patients who were taking medication at the time of the survey. We further defi

Regression of left ventricular hypertrophy with moexipril, an angiotensin-converting enzyme inhibitor, in hypertensive patients.

Left ventricular hypertrophy (LVH) is a common complication of essential hypertension and an independent risk factor for the development of cardiovascular disease. Therefore, antihypertensive treatment should decrease blood pressure (BP) and reverse LVH. However, antihypertensive drugs have been shown to have different effects on LVH despite similar effects on BP reduction. Although lowering BP produces a beneficial effect on LVH per se, meta-analyses of clinical trials have indicated that angiotensin-converting enzyme (ACE) inhibitors decrease left ventricular mass (LVM) to a greater extent t

Calpain-GRIP Signaling in Nucleus Accumbens Core Mediates the Reconsolidation of Drug Reward Memory.

Exposure to drug-paired cues causes drug memories to be in a destabilized state and interfering with memory reconsolidation can inhibit relapse. Calpain, a calcium-dependent neutral cysteine protease, is involved in synaptic plasticity and the formation of long-term fear memory. However, the role of calpain in the reconsolidation of drug reward memory is still unknown. In the present study, using a conditioned place preference (CPP) model, we found that exposure to drug-paired contextual stimuli induced the activation of calpain and decreased the expression of glutamate receptor interacting pr

Fluid and Electrolyte Management in Critically Ill Patients with Infections.

Fluid and electrolyte management is a crucial component of care in critically ill patients with severe infections. Patients with sepsis, septic shock, or multiorgan dysfunction syndrome struggle to maintain fluid balance due to increased capillary permeability, inflammation, vasodilation, and organ dysfunction. Inadequate fluid balance can increase complications, morbidity, and mortality. Fluid management is complex because sepsis disrupts vascular permeability, tissue perfusion, and cellular function. Infectious disease care is further challenged by antimicrobial resistance, drug stewardship,

Editorial: future of ureteral stents: what do we need? a pearls opinion.

Ureteral stents, introduced in 1960s, are essential for managing upper urinary tract pathologies, maintaining ureteral patency in conditions like obstruction, trauma and post-surgery. However, stent-related symptoms (SRS) including pain, dysuria, and urinary frequency, remain a significant clinical challenge. Advances in stent design, biomaterials, drug-eluting catheters and biodegradable options aim to reduce SRS and complications like encrustation and infections. Despite this improvements, the ideal stent has yet to be achieved. The future of stent therapy lies in personalized approaches, ai

[Overview of therapeutic strategies for motor disturbances in Parkinson's disease].

Parkinson's disease is an adult neurodegenerative disorder whose prevalence continues to rise. There is no proven therapeutic strategy capable of interfering with its unfavourable evolutionary profile. Current treatments remain symptomatic. This article proposes to guide practitioners through the various therapeutic strategies for motor manifestations available in Belgium, from first-line drug treatments to the more complex options offered at the most advanced stages, such as deep brain stimulation or levo-dopa infusions. Regardless disease duration, maintaining quality of life involves essent

Drug-induced Liver Injury: Pathology Patterns and Common Culprits.

Drug-induced liver injury (DILI) has an incredible range of morphologic presentations, from acute extensive necrosis to resolving injury with ceroid-laden macrophages. The diversity in presentation on biopsy is diagnostically challenging, but DILI is becoming more widely recognized, especially with the aid of resources like LiverTox. Some medications, such as acetaminophen, have well-established patterns of injury. However, newer medications, such as immune checkpoint inhibitors, are continually being developed, and our understanding of their effects on the liver are evolving. In this chapter,

Disparities in Diagnosis of and Management of Narcolepsy and Idiopathic Hypersomnia.

This article examines disparities in the diagnosis, treatment, and lived experience of narcolepsy and idiopathic hypersomnia (IH). It highlights prolonged diagnostic delays, especially in women, children, and marginalized groups, compounded by limited access to essential sleep tests and insurance-related barriers. Treatment inequities stem from high medication costs, restrictive coverage, and a lack of global drug availability, leaving many patients with persistent symptoms. IH poses unique challenges due to ambiguous diagnostic criteria and poor test reliability. Both disorders impose severe
