Selective fetal termination for preeclampsia treatment in a dichorionic gestation with a triploid fetus: A case report.

BACKGROUND: Triploidy is commonly associated with the development of early-onset preeclampsia. While previable preeclampsia is often a contraindication to prolonging pregnancy, there may be rare circumstances in which an alternative approach may be offered. CASE: A nulliparous patient with a dichorionic twin gestation, recently diagnosed triploidy in one twin, and history of chronic hypertension presented at 18 weeks of gestation with signs and symptoms suggestive of preeclampsia. After symptomatic therapy and laboratory evaluations, selective fetal termination of the affected twin was electe

Apelin/Elabela-APJ: a novel therapeutic target in the cardiovascular system.

Apelin and Elabela (ELA) are endogenous ligands of angiotensin domain type 1 receptor-associated proteins (APJ). Apelin/ELA-APJ signal is widely distributed in the cardiovascular system of fetuse and adult. The signal is involved in the development of the fetal heart and blood vessels and regulating vascular tension in adults. This review described the effects of apelin/ELA-APJ on fetal (vasculogenesis and angiogenesis) and adult cardiovascular function [vascular smooth muscle cell (VSMC) proliferation, vasodilation, positive myodynamia], and relative diseases [eclampsia, hypertension, pulmona

Maternal infections.

Congenital infections are infections transmitted from mother to child during pregnancy (transplacentally) or delivery (peripartum). They have the potential to adversely affect fetal development and long-term neurodevelopmental outcome through inflammatory, destructive, developmental, or teratogenic lesions of the brain. Because the fetal/neonatal brain has a limited capacity to respond to injury, early inflammatory changes may be difficult to visualize and only manifest as neurocognitive disability later in life. Teratogenic effects, which may include aberrations of neuronal proliferation and …

IFITM proteins inhibit placental syncytiotrophoblast formation and promote fetal demise.

Elevated levels of type I interferon (IFN) during pregnancy are associated with intrauterine growth retardation, preterm birth, and fetal demise through mechanisms that are not well understood. A critical step of placental development is the fusion of trophoblast cells into a multinucleated syncytiotrophoblast (ST) layer. Fusion is mediated by syncytins, proteins deriving from ancestral endogenous retroviral envelopes. Using cultures of human trophoblasts or mouse cells, we show that IFN-induced transmembrane proteins (IFITMs), a family of restriction factors blocking the entry step of many vi

Prenatal diagnosis of isochromosome 20q in a fetus with vertebral anomaly and rocker-bottom feet.

OBJECTIVE: Isochromosome of the long arm of chromosome 20 (i(20q)) is a rare structural abnormality in prenatal diagnosis. Thirty prenatal cases of mosaic i(20q) have been reported, among which only four are associated with fetal malformations. We describe a new prenatal case of i(20q) with fetal malformations. MATERIALS AND METHODS: We also observed a discrepancy between uncultured and cultured amniotic fluid cells by using conventional cytogenetic, fluorescence in situ hybridization and array-SNP analysis. RESULTS: The short arm deletion of chromosome 20 arising from the isochromosome enco

Can maternal microchimeric cells influence the fetal response toward self antigens?

The origins of autoimmunity are still elusive despite significant advances in immunology. There is cumulative evidence that, beyond simple genetics, the maternal environment plays a critical role in the development of common autoimmune disorders, such as multiple sclerosis or diabetes. In recent years, the trafficking of maternal cells to the offspring has been clearly demonstrated. This microchimerism represents the very first immunological event in fetal life. The number of persisting maternal cells has been associated with several autoimmune disorders such as systemic sclerosis, juvenile de

SOX2 gene expression in normal human thymus and thymoma.

The SOX gene family encodes a large group of transcription factors that are strongly involved in the normal human development and malignancies. Human tumors, like small cell lung carcinoma, meningioma, gastric, and pancreatic cancer, have been found to be immunogenic for SOX2 protein. In this study, we have investigated for the first time the expression and distribution of SOX2 immunoreactive cells in five normal human thymuses (2 fetal and 3 adult) and 10 thymomas bioptic specimens. Results demonstrated the presence of few positive cells in the fetal and postnatal normal human thymus with a s

Thyroid dysfunction in pregnancy: optimizing fetal and maternal outcomes.

Recent decades have seen a growing awareness of the threat posed by thyroid dysfunction in pregnancy on maternal, fetal and neonatal well-being. Uncontrolled hypothyroidism and hyperthyroidism are associated with adverse obstetrics and fetal outcomes and also affect neurointellectual development in the offspring. Excellent outcomes are, however, achievable with appropriate management. An understanding of the changes in thyroid hormone economy in pregnancy is crucial to the evaluation of thyroid status in pregnant patients with thyroid dysfunction. Furthermore, a balance must be maintained betw

Survival of a profoundly hydropic fetus with a sacrococcygeal teratoma delivered at 27 weeks of gestation for maternal mirror syndrome.

The development of hydrops fetalis and maternal mirror syndrome in the setting of fetal sacrococcygeal teratoma (SCT) carries a grave prognosis. Fetal surgery is not typically offered for hydropic fetuses beyond 26 weeks of gestational age. Few options exist for these families, and they are typically counseled to continue pregnancy until delivery is necessary for maternal indications or the fetus is 30 weeks old. Although a number of series report the survival of hydropic SCT infants delivered after 30 weeks of gestation, a paucity of information is available regarding the outcome of hydropic

Acute fatty liver of pregnancy: an update on pathogenesis and clinical implications.

Acute fatty liver of pregnancy (AFLP) is a serious maternal illness occurring in the third trimester of pregnancy with significant perinatal and maternal mortality. Till recently, it has been considered a mysterious illness. In this editorial, we review the recent advances in understanding the pathogenesis of AFLP and discuss the studies documenting a fetal-maternal interaction with a causative association between carrying a fetus with a defect in mitochondrial fatty acid oxidation and development of AFLP. Further, we discuss the impact of these recent advances on the offspring born to women w

Placental pathology in antiphospholipid syndrome.

One of the major targets of antiphospholipid antibodies (aPL) is the placenta, the evolution of which during pregnancy has been well documented. Histopathological findings are related to gestational age, and several physiologic and pathologic alterations that occur during its development. The major findings in placentae from aPL positive patients are thrombosis, acute atherosis, a decreased number of syncytio-vascular membranes, increased number of syncytial knots and obliterative arteriopathy. These findings are not specific to the antiphospholipid syndrome (APS) and sometimes do not correlat

Fetal immune response following maternal diphtheria during early pregnancy.

A pregnant Sudanese woman contracted pharyngeal diphtheria during the first trimester of gestation. She had a severe diphtheritic toxaemia and paralysis of all four limbs but made a full recovery from these manifestations. Pregnancy, apart from vaginal bleeding, was not interrupted. At term she was delivered of a female baby who was physically normal but had a very high level of IgA in cord blood. Diphtheria in adults is a rare disease and the occurrence of diphtheria in a pregnant woman has not, as yet, been reported. Accordingly it is not known how fetal growth and development progress in th

Fetal and placental responses to artificially induced hyperthermia in rats.

Pregnant rats were utilized to study the effect of maternal hyperthermia on fetal development. Eight groups of six to eight rats were exposed to ambient temperatures of 43-44 degrees C at various stages of pregnancy. All rats were killed on day 20 of gestation. Edema, microencephaly and microphthalmia followed heat treatment on day 4, 6, or 8 and skeletal defects occurred on day 10 of gestation. Apparently heat stress of dams after day 14 of gestation had little or no effect on embryos. Most placentas from day 6-10 treatment groups were significantly heavier than control and exhibited extensiv…

DIABETES AND THE GYNECOLOGIST.

The concept of a metabolic disorder of carbohydrate tolerance that progresses with age is examined. It is considered that acute exacerbations may occur at the time of puberty or during pregnancy or the menopause, leading to the development of manifest clinical diabetes.Pregnancy offers a unique opportunity for early diagnosis of potential diabetes in the mother. Existing screening tests are not satisfactory or reliable in uncovering this inborn error of metabolism that may lead to stillbirth or a grossly overweight baby, the onset of clinical diabetes in the mother occurring only many years la

Expression of thyroid receptor isoforms in the human fetal central nervous system and the effects of intrauterine growth restriction.

BACKGROUND: Congenital hypothyroidism is known to be associated with mental retardation which, if recognized promptly, is largely prevented by thyroid hormone replacement. Intrauterine growth restriction (IUGR) is a major cause of perinatal mortality and morbidity, and is also associated with neurodevelopmental delay. Fetuses with IUGR have reduced circulating concentrations of free thyroxine (T4) and free triiodothyronine (T3), leading to the hypothesis that a reduction in the tissue effects of thyroid hormones in the central nervous system (CNS) may contribute to neurodevelopmental morbidity

FetalSleepNet: A Transfer Learning Framework with Spectral Equalisation Domain Adaptation for Fetal Sleep State Classification.

OBJECTIVE: Fetal sleep state classification is essential for identifying neurodevelopmental complications like hypoxia, but manual annotation is subjective and labor intensive, and fetal EEG (fEEG) data is extremely scarce. METHODS: We propose FetalSleepNet, the first deep learning architecture specifically developed for automated sleep staging from the fEEG. To address the scarcity of labeled data, we implement the first cross-developmental (adult-to fetal) and cross-species (human-to-sheep) transfer learning framework for this task, utilizing Spectral Equalisation (SE) to align the frequenc

Single Nucleotide Polymorphism Typing Going Spatial: In Situ Padlock Probes Targeting mRNA Variants to Identify Haploidentical Cells within the Tissue Environment.

BACKGROUND: Despite almost 3 decades of research, the mechanisms underlying the bidirectional trafficking of cells at the maternal-fetal interface that gives rise to microchimerism remain poorly understood. A major barrier to progress has been the lack of suitable detection methods capable of distinguishing maternal from fetal cells within the spatial context of the human placenta. To address this, we developed a novel detection method based on padlock probe technology to differentiate haploidentical cells in placental tissues. METHODS: Padlock probes were designed to target single nucleotide

Bio-engineering of fetal cartilage for in utero spina bifida repair.

PURPOSE: During in utero surgical spina bifida repair, a multi-layer closure is used to cover the defect. These soft tissues, however, might be not sufficient to protect the spinal cord during the future life. Our goal is to develop a more rigid protective tissue construct consisting of bioengineered cartilage and skin. METHODS: Ovine fetal chondrocytes were tested for their in vitro chondrogenic potential in three-dimensional cultures. Scaffolds based on natural biopolymers (collagen I, fibrin glue) were loaded with varying amounts of fetal chondrocytes and assessed for their ability to supp
