Synpolydactyly (SPD) is a dominantly inherited congenital limb malformation consisting of 3/4 syndactyly in the hands and 4/5 syndactyly in the feet, with digit duplication in the syndactylous web. The condition recently has been found to result from different-sized expansions of an amino-terminal polyalanine tract in HOXD13. A case of a single-stage reconstruction of the left hand of a 32-year-old right-handed construction worker with a shotgun wound by means of a composite osteoarthrotenocutaneous free flap from the foot is presented. The tissue loss caused by the injury included the cutaneous coverage of the dorsum of the left hand, extensor tendons of the second, third, fourth and fifth digits, metacarpals of the same fingers, and the second and third metacarpophalangeal joints. After 8 years of evolution, we have been able to verify a valuable degree of functional capability of the reconstructed hand.

PURPOSE: To determine if runners have an increased prevalence of marrow edema in the foot and ankle compared with nonrunners at magnetic resonance (MR) imaging.

Transient regional osteoporosis of the ankle and foot is an uncommon clinical entity characterized by local tenderness over the involved area. Plain radiographs demonstrate osteopenia of the involved bone and technetium diphosphonate bone imaging shows increased tracer uptake at the same location. This report describes the construction and evaluation of an ankle-foot prosthesis using human cadaveric bones as the endoskeleton inside a fiber-reinforced vulcanized rubber shell. Cadaveric bones and exhumed skeletal bones were used. A literature review of the etiologies and treatments of both hallux varus and brachymetatarsia is presented. An unusual case report of a young girl with bilateral congenital hallux varus and brachymetatarsia of the first metatarsal is then discussed.
