Osteogenesis imperfecta (OI) is often regarded as a form of osteoporosis. However, the bone fragility is the result of defective collagen and earlier work has demonstrated that cortical thickness, in bones not previously fractured, is usually normal. We have now measured the bone mineral content of the distal forearm in 61 adult patients with well characterized OI. Limb lengthening in nine patients was monitored by radiographs and by ultrasound scans. The distraction gap appeared as a sonolucent area within which echogenic foci developed soon after distraction commenced. By seven weeks a new cortex was detected, and medullary canal began to develop between seven and eight weeks. Ultrasound scanning can be used to measure distraction, but it was not as useful as radiographs in detecting angulation. Its use in patients undergoing limb lengthening could reduce their exposure to radiation.

The effects of acute exposure to noise on central and regional (calf and forearm) haemodynamics were studied during placebo therapy and during combined nonselective beta-adrenoceptor blockade and precapillary vasodilatation in 9 patients with essential hypertension. Since May 1972, vein grafts have been used to restore circulation in electrical injuries of the upper extremity when the wrist has been the centre of electrical injury associated with obstructed blood supply. Results in a series of 24 total parathyroidectomies with immediate reimplantation in patients with chronic renal insufficiency are described, and the advantages of this method discussed. Two patients are described in whom symptoms of the carpal tunnel syndrome were provoked by haemodialysis for which an antebrachial arteriovenous fistula had been established. The symptoms in one case were partially relieved by ligation of the radial artery distal to the fistula, and in both they were abolished by decompression of the median nerve by section of the flexor retinaculum at the wrist.
