Fetal alcohol spectrum disorders in children residing in Russian orphanages: a phenotypic survey.

BACKGROUND: Alcohol use in Russia is among the highest in the world. Over 600,000 children reside in institutional care in Russia, most of them in baby homes and orphanages. The actual prevalence of fetal alcohol spectrum disorders (FASD) among these children is unknown. Therefore, we performed a systematic survey of phenotypic features associated with prenatal alcohol exposure among institutionalized Russian children and related these findings to their growth, development, medical, and social histories. METHODS: Phenotypic screening was conducted of all 234 baby home residents in the Murmans

Growth hormone and bone health.

Growth hormone (GH) and insulin-like growth factor-I have major effects on growth plate chondrocytes and all bone cells. Untreated childhood-onset GH deficiency (GHD) markedly impairs linear growth as well as three-dimensional bone size. Adult peak bone mass is therefore about 50% that of adults with normal height. This is mainly an effect on bone volume, whereas true bone mineral density (BMD; g/cm(3)) is virtually normal, as demonstrated in a large cohort of untreated Russian adults with childhood-onset GHD. The prevalence of fractures in these untreated childhood-onset GHD adults was, howev

Endogenous erythroid and megakaryocytic colony formation in serum-free, cytokine-free collagen gels.

We studied the suitability of collagen-based semisolid medium for assay of endogenous erythroid colony formation performed in myeloproliferative disorders. Bone marrow (BM) mononuclear cells (MNC) from 103 patients suspected of having polycythemia vera (PV, 76 patients) or essential thrombocythemia (ET, 27 patients) were grown in collagen-based, serum-free, cytokine-free semisolid medium. Colony analysis at day 8 or 10 showed that this collagen assay is specific, as endogenous growth of erythroid colonies was never observed in cultures of 16 healthy donors and 6 chronic myelogenous leukemia (C

Bartter's syndrome in Arabic children: review of 13 cases.

BACKGROUND: Bartter's syndrome (BS) is an inherited disease of renal potassium wasting characterized by hypokalemic alkalosis, normal blood pressure, vascular insensitivity to pressor agents and elevated plasma concentrations of renin and aldosterone. It is caused by generalized hyperplasia of the juxtaglomerular apparatus at the site of renin production caused by mutations in the Na-K-2Cl cotransporter gene, NKCC2. The objective of our study is to establish the prevalence and incidence of BS in Kuwait and to assess treatment modalities for it. METHODS AND RESULTS: Bartter's syndrome was diag

Celiac disease and Turner syndrome.

BACKGROUND: Short stature is one of the features of Turner syndrome and a form of presentation of monosymptomatic celiac disease. METHODS: The recognition of celiac disease in two antiendomysium antibody-positive Turner syndrome girls who did not respond to growth hormone treatment led us to perform as a screening for celiac disease IgA and IgG antigliadin antibodies and antiendomysium antibodies determination in other 35 Turner syndrome patients. Intestinal biopsy was proposed to the antiendomysium antibodies-positive girls; in the former, subtotal villous atrophy was found; in the latter, o

Lamellar bone turnover system and its effector organ.

Juxtaposition of cell kinetics in the asymmetric, self-renewing osteoclast and osteoblast populations in the evolving secondary Haversian systems, which are in neutral balance, disclosed their genetically determined fixed and flexible or variable properties, the latter controllable. These jointly controlled cell populations (coupling) at local and organ level, constitute the Effector Organ of Lamellar Bone Turnover Systems (EO LBTS). In part genetically preprogrammed, this system also responds during growth and maturity to environmental (mainly biomechanical) factors that adjust structure to f

Clinical Features and Short-Term Growth Hormone Outcomes in Maternal UPD of Chromosome 20: Five Chinese Patients and a Literature Review.

BACKGROUND: Maternally derived uniparental disomy of chromosome 20 [UPD(20)mat], also known as Mulchandani-Bhoj-Conlin syndrome (MBCS), is a rare imprinting disorder with fewer than 30 cases documented to date. It is characterized by prenatal and postnatal growth failure and feeding difficulties. Herein, we aimed to summarize the clinical and growth characteristics of UPD(20)mat and describe short-term growth outcomes during recombinant human growth hormone (rhGH) exposure in affected individuals. METHODS: We retrospectively analyzed five Chinese children with UPD(20)mat genetically evaluated

Cardiac myocyte proliferation and maturation near term is inhibited by early gestation maternal testosterone exposure.

Polycystic ovary syndrome is a complex and common disorder in women, and those affected experience an increased burden of cardiovascular disease. It is an intergenerational syndrome, as affected women with high androgen levels during pregnancy "program" fetal development, leading to a similar phenotype in their female offspring. The effect of excess maternal testosterone exposure on fetal cardiomyocyte growth and maturation is unknown. Pregnant ewes received biweekly injections of vehicle (control) or 100 mg testosterone propionate between 30 and 59 days of gestation (early T) or between 60 an

Developmental challenges in infants who are HIV-exposed uninfected.

INTRODUCTION: The global HIV epidemic continues to affect millions of people and significantly affects children. Approximately 16 million children born to HIV-positive women, categorized as HIV-exposed uninfected (HEU), face unique developmental hurdles despite not contracting the virus. These children are more likely to experience developmental issues due to factors such as exposure to maternal HIV, antiretroviral therapy (ART), nutritional shortages, and adverse social conditions. Research indicates that children who are HIV-exposed but uninfected (HEU) may have subpar neurodevelopmental out

Nephrotic syndrome in a child with neurofibromatosis type 1: A case report and literature review.

Neurofibromatosis type 1 (NF1) is an autosomal dominant genetic disorder that caused by NF1 mutations. NF1 gene encodes neurofibromin (a GTPase-activating protein) and plays a regulatory role in many signalling pathway such as the Ras/MAPK pathway, which is important for regulating cell growth, proliferation and neural development. Therefore, NF1 gene mutations causes the excessive activation of signalling pathways and uncontrolled cell growth. NF1 exhibits complete genetic penetrance and clinical heterogeneity. Glomerular disease has rarely been reported in patients with NF1, especially in ch

Is there clinical consensus in defining weight restoration for adolescents with anorexia nervosa?

Successful outcome for adolescent anorexia nervosa includes achieving weight restoration. Despite the importance of this measurement, there are significant discrepancies in the field on how to quantify expected body weight (EBW). A survey was sent to 113 child and adolescent eating disorder treatment providers inquiring about the methods used to determine weight restoration in their clinical practice. Although 40.7% used growth curve data, the remaining employed a diverse range of approaches. Providers who specialize in Family-Based Treatment were significantly more likely to use an individual

Neurodevelopmental Problems in Non-Syndromic Craniosynostosis.

Craniosynostosis is the premature fusion of calvarial sutures, resulting in deformed craniofacial appearance. Hence, for a long time, it has been considered an aesthetic disorder. Fused sutures restrict growth adjacent to the suture, but compensatory skull growth occurs to accommodate the growing brain. The primary goal for the management of this craniofacial deformity has been to release the constricted skull and reform the distorted shape of the skull vault. However, the intellectual and behavioral prognosis of affected children has also been taken into consideration since the beginning of t

Effect of thanatophoric dysplasia type I mutations on FGFR3 dimerization.

Thanatophoric dysplasia type I (TDI) is a lethal human skeletal growth disorder with a prevalence of 1 in 20,000 to 1 in 50,000 births. TDI is known to arise because of five different mutations, all involving the substitution of an amino acid with a cysteine in fibroblast growth factor receptor 3 (FGFR3). Cysteine mutations in receptor tyrosine kinases (RTKs) have been previously proposed to induce constitutive dimerization in the absence of ligand, leading to receptor overactivation. However, their effect on RTK dimer stability has never been measured experimentally. In this study, we charact

A rare cause of mental motor retardation: recessive congenital methemoglobinemia type II.

Recessive congenital methemoglobinemia (RCM) is a very rare disorder caused by NADH- cytochrome b5 reductase (cytb5r) deficiency. It has been classified into four types. Type I presents with mild cyanosis due to a significant deficiency of cytb5r in erythrocytes only. In type II, the deficiency occurs in all tissues and causes growth and mental retardation and other neurological impairments. RCM types I and II are caused by a defect in a single gene, which is located on chromosome 22 (locus DIA 1: q 13.31-qter). Prenatal diagnosis is possible. Cyanosis can be well treated by 200-500 mg of asco

The impact of resource loss and traumatic growth on probable PTSD and depression following terrorist attacks.

The authors interviewed by phone 2,752 randomly selected individuals in New York City within 6 to 9 months after the attacks of September 11, 2001 on the World Trade Center, and 1,939 of these were reinterviewed at a 12- to 16-month follow-up. It was hypothesized that resource loss would significantly predict probable posttraumatic stress disorder (PTSD) and probable depression since September 11, and that resource loss's impact would be independent of previously identified predictors relating to individuals' demographic characteristics, history of stressful event exposure, prior trauma histor
