Health promotion in the city: a review of current practice and future prospects in the United States.

To achieve its health goals, the United States must reduce the disproportionate burden of illness and poor health borne by urban populations. In the 20th century, patterns of immigration and migration, changes in the global economy, increases in income inequality, and more federal support for suburbanization have made it increasingly difficult for cities to protect the health of all residents. In the last 25 years, epidemics of human immunodeficiency virus infections and substance abuse and increases in homelessness, lack of health insurance, rates of violence, and concentrations of certain po

Supporting women with perinatal mental health problems: the role of the voluntary sector.

The prevalence of perinatal mental health problems is high with estimates suggesting that around a fifth of women experience anxiety and or depression during the ante- and post-natal period. Mental health problems in pregnancy and the postnatal period have an adverse impact on the development of the foetal and infant nervous system and the parent-infant relationship, with significant long-term consequences for the child. The charity Family Action established a Perinatal Support Project (PSP) underpinned by the Newpin model of working, at four sites across the U.K. The service offered women exp

Declining rates of health problems associated with crack smoking during the expansion of crack pipe distribution in Vancouver, Canada.

BACKGROUND: Crack cocaine smoking is associated with an array of negative health consequences, including cuts and burns from unsafe pipes, and infectious diseases such as HIV. Despite the well-established and researched harm reduction programs for injection drug users, little is known regarding the potential for harm reduction programs targeting crack smoking to reduce health problems from crack smoking. In the wake of recent crack pipe distribution services expansion, we utilized data from long running cohort studies to estimate the impact of crack pipe distribution services on the rates of h

Aligning clinical care with consumer priorities: an adult cystic fibrosis centre experience of co-designing a model of care.

Cystic fibrosis (CF) is a multi-system genetic condition, and CF modulator therapies have transformed health outcomes promising improved longevity. Our aim was to co-design and implement a nested endocrine and metabolic model of care called CF Endocrine for adults with CF that aligned with consumer and healthcare provider priorities and addressed emerging service gaps in the post-modulator era. Using mixed-methods and implementation science methodologies, we designed and conducted surveys, audits and workshops to define service priorities and referral pathways and workflows. In total, 17% of a

The Ties That Bind and Unbound Ties: Experiences of Formerly Homeless Individuals in Recovery From Serious Mental Illness and Substance Use.

This report explores the nature and quality of social ties of formerly homeless individuals in recovery from serious mental illness and substance abuse and how these ties relate to experiences of community. Using grounded theory and cross-case analysis techniques, we analyzed 34 qualitative interviews conducted with predominantly racial/ethnic minority individuals receiving mental health services. Participants described a range of involvement and experiences in the mental health service and mainstream communities indicating a combination of weak or strong ties in these communities. Across part…

Access to Recovery and Recidivism Among Former Prison Inmates.

Access to Recovery (ATR) is a Substance Abuse and Mental Health Services Administration (SAMHSA)-funded initiative that offers a mix of clinical and supportive services for substance abuse. ATR clients choose which services will help to overcome barriers in their road to recovery, and a recovery consultant provides vouchers and helps link the client to these community resources. One of ATR's goals was to provide services to those involved in the criminal justice system in the hopes that addressing substance abuse issues could reduce subsequent criminal behaviors. This study examines this goal …
