Association of in vivo β-adrenergic receptor sensitivity with inflammatory markers in healthy subjects.

OBJECTIVE: Several stress-related states and conditions that are considered to involve sympathetic overactivation are accompanied by increased circulating levels of inflammatory immune markers. Prolonged sympathetic overactivity involves increased stimulation of the β-adrenergic receptor (β-AR). Although prior research suggests that one mechanism by which sympathetic stimulation may facilitate inflammation is via β-AR activation, little work has focused on the relationship between circulating inflammatory immune markers and β-AR function within the human body (in vivo). We examined whether dec

Effects of maternal supplementary oxygen on the newborn for elective cesarean deliveries under spinal anesthesia.

PURPOSE: The aim of this investigation was to determine whether supplementary oxygen provided by either nasal cannula or face mask versus room air might affect fetal oxygenation during elective cesarean section under spinal anesthesia by assessing maternal and neonatal regional cerebral oxygenation (rSO(2)) with a cerebral oximeter. METHODS: Ninety parturients were randomly allocated into three groups: two groups received 5 L/min oxygen by either nasal cannula (Group NC, n = 30) or face mask (Group FM, n = 30), respectively, and the third group was allowed to breathe room air (Group RA, n = 3

Changes in cortisol release and heart rate and heart rate variability during the initial training of 3-year-old sport horses.

Based on cortisol release, a variety of situations to which domestic horses are exposed have been classified as stressors but studies on the stress during equestrian training are limited. In the present study, Warmblood stallions (n=9) and mares (n=7) were followed through a 9 respective 12-week initial training program in order to determine potentially stressful training steps. Salivary cortisol concentrations, beat-to-beat (RR) interval and heart rate variability (HRV) were determined. The HRV variables standard deviation of the RR interval (SDRR), RMSSD (root mean square of successive RR di

Differential effects of beta-adrenoreceptor antagonists on central and peripheral blood pressure at rest and during exercise.

BACKGROUND: Differential effects of beta-adrenoreceptor antagonists (beta-ARB) on central and peripheral blood pressure may relate to change in heart rate and/or vasodilator tone and thus be exaggerated during exercise. AIMS: To examine acute effects of selective and nonselective beta-ARB on central and peripheral blood pressure, cardiac output and peripheral vascular resistance during exercise. METHODS: Healthy volunteers (n= 20, 18 men, 19-54 years) received propranolol 80 mg, bisoprolol 20 mg, and placebo 1 h before bicycle ergometry (50, 75 and 100 W each for 3 min) in a randomized, cros

Prediction of early secondary complications in patients with spontaneous subarachnoid hemorrhage based on accelerated sympathovagal ratios.

PURPOSE: The development of secondary complications following spontaneous subarachnoid hemorrhage (SAH) largely depends on sympathetic overexcitation. The roles of vagal activities, however, are poorly defined. Because both components of the autonomic nervous system can be explored in the frequency domain of heart rate variability (HRV), the present study aimed to determine the dynamic evolution of autonomic activities and to identify patients at high risk for complications following hemorrhage. METHODS: Thirty patients with SAH were enrolled in our study. Those who suffered from symptomatic

Arginine vasopressin to manage hypoxemic infants after stage I palliation of single ventricle lesions.

OBJECTIVE: Management of patients with single ventricle physiology following stage I palliation procedures is often challenging, with optimization of the ratio of pulmonary-to-systemic blood flow as an important goal. Persistent hypoxemia may be a manifestation of elevated pulmonary vascular resistance and therefore decreased blood flow to the lungs. In such situations, the use of arginine vasopressin to increase systemic vascular resistance may be an effective strategy to improve pulmonary blood flow and maintain adequate pulmonary-to-systemic blood flow ratio. We describe three infants in wh

Determinants of arterial wall stiffness and peripheral artery occlusive disease in nondiabetic hemodialysis patients.

Aortic pulse wave velocity (Ao-PWV) and ankle-brachial blood pressure index (ABPI) are significant prognostic factors in patients with end-stage renal disease (ESRD). Diabetes mellitus (DM) promotes changes in arterial walls, including marked increases in Ao-PWV and decreases in ABPI. To determine the prevalence of peripheral arterial occlusive disease (PAOD) as well as the clinical variables useful in predicting these changes in nondiabetic patients with ESRD undergoing hemodialysis (HD), we performed a cross-sectional study in a cohort of 143 patients. Ao-PWV and ABPI were measured simultane

Rate, predictors, and consequences of hemodynamic depression after carotid artery stenting.

OBJECTIVES: We sought to determine the frequency, predictors, and consequences of hemodynamic depression (HD) after carotid artery stenting (CAS). BACKGROUND: Hemodynamic depression has been reported after carotid artery stenting CAS and carotid endarterectomy (CEA). METHODS: We retrospectively analyzed data on 500 consecutive CAS procedures performed over a 5-year period. Hemodynamic depression was defined as periprocedural hypotension (systolic blood pressure <90 mm Hg) or bradycardia (heart rate <60 beats/s). Univariate and multivariate binary logistic regression models were used to deter

Risk factors for heat related death during the August 2003 heat wave in Paris, France, in patients evaluated at the emergency department of the Hôpital Européen Georges Pompidou.

OBJECTIVES: This study sought to determine the risk factors for short term mortality in the victims of the heat wave of August 2003 in France from among patients evaluated in our emergency department (ED). It was hypothesised that age, temperature, and some long term therapies and pre-existing pathologies were factors associated with short term mortality. METHODS: A retrospective analysis of a seven day period. Four experts decided blindly, in pairs, whether a patient had presented with a heat related problem. Inclusion criteria were: core temperature > or =38 degrees C and/or clinical signs

1993 2003 gender differences in coronary artery revascularization: has anything changed?

BACKGROUND: Gender differences abound in patients undergoing coronary artery bypass graft (CABG) surgery. Most research was conducted in the early 1 990s. It is unknown if gender differences have diminished over time. RESEARCH OBJECTIVES: To determine whether gender differences exist in the current era of CABG surgery by examining preoperative, intraoperative, and postoperative factors known to affect outcomes. SUBJECTS AND METHODS: In this descriptive, correlational study of all patients undergoing primary, isolated CABG at a large, urban Midwestern healthcare center, data in 1993 and 2003

Effect of a perinatal high-salt diet on blood pressure control mechanisms in young Sprague-Dawley rats.

In the present investigation we sought to determine if a perinatal high-salt treatment affects blood pressure at an early age (30 days), and if so, to determine the mechanisms responsible for the hypertension. Pregnant dams were given an 8% NaCl diet [high-salt (HS) rats] during the final one-third of gestation and throughout the suckling period. After weaning, the pups continued to receive the high-salt diet until testing at age 30 days. Control groups received a normal-salt diet (NS rats). In HS rats, mean arterial pressure (MAP) was significantly increased (110 +/- 5 vs. 96 +/- 3 mmHg) comp
