The treatment of reverse obliquity intertrochanteric fractures with the intramedullary hip nail.

BACKGROUND: Reverse obliquity intertrochanteric fractures have been recognized as having unique anatomic and mechanical characteristics. Even though some clinical reports regarding intramedullary hip nailing for reverse obliquity intertrochanteric fracture show favorable results, there has been no clinical report of intramedullary hip nailing regarding the clinical significance of the lesser trochanteric fragment which differentiates Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) 31-A3.3 from A3.1 and A3.2. METHODS: We retrospectively reviewed the clinical

Robotic-Assisted Single-Position Prone Lateral Lumbar Interbody Fusion: Indications, Techniques, and Outcomes.

BACKGROUND: Lateral lumbar interbody fusion (LLIF) is a widely utilized minimally invasive surgical procedure for anterior fusion of the lumbar spine. However, posterior decompression or instrumentation often necessitates patient repositioning, which is associated with increased operative time and time under anesthesia1-3. The single-position prone transpsoas approach is a technique that allows surgeons to access both the anterior and posterior aspects of the spine, bypassing the need for intraoperative repositioning and therefore optimizing efficiency4. The use of robotic assistance allows fo

Surgical versus non-surgical management of lateral compression type-1 pelvic fracture in adults 60 years and older: the L1FE RCT.

BACKGROUND: Lateral compression type-1 pelvic fractures are a common fragility fracture in older adults. Patients who do not mobilise due to ongoing pain are at greater risk of immobility-related complications. Standard treatment in the United Kingdom is provision of pain relief and early mobilisation, unlike fragility hip fractures, which are usually treated surgically based on evidence that early surgery is associated with better outcomes. Currently there is no evidence on whether patients with lateral compression type-1 fragility fractures would have a better recovery with surgery than non-

Effectiveness of home-based and remotely supervised aerobic exercise in Parkinson's disease: a double-blind, randomised controlled trial.

BACKGROUND: High-intensity aerobic exercise might attenuate the symptoms of Parkinson's disease, but high-quality evidence is scarce. Moreover, long-term adherence remains challenging. We aimed to evaluate the effectiveness of aerobic exercise-gamified and delivered at home, to promote adherence-on relieving motor symptoms in patients with Parkinson's disease with mild disease severity who were on common treatment regimes. METHODS: In this single-centre, double-blind, randomised controlled trial (Park-in-Shape), we recruited sedentary patients with Parkinson's disease from the outpatient clin

Municipality-based physical rehabilitation after acute hip fracture surgery in Denmark.

INTRODUCTION: Patients who are surgically treated for an acute hip fracture in Denmark commence early in-hospital physical rehabilitation (PR) with more than 95% of patients referred to further PR following discharge. However, the specifics of the PR services after discharge are unknown. Thus, the aim of the present paper was to describe the specifics of PR provided to patients following discharge after hip fracture (HF) surgery in Denmark to evaluate the need for future interventions or guidelines. METHODS: This was a national, cross-sectional questionnaire survey including 56 randomly selec

Minimally invasive dynamic hip screw for fixation of hip fractures.

We compared a minimally invasive surgical technique to the conventional (open approach) surgical technique used in fixation of hip fractures with the dynamic hip screw (DHS) device. Using a case-control design (44 cases and 44 controls), we tested the null hypothesis that there is no difference between the two techniques in the following outcome measures: duration of surgery, time to mobilisation and weight bearing postoperatively, length of hospital stay, mean difference of pre- and postoperative haemoglobin levels, position of the lag screw of the DHS device in the femoral head, and the tip-

Medial Meniscus Posterior Root Shows More Severe Histologic Degeneration in Female Patients With Advanced Osteoarthritis and Varus Alignment: A Histopathologic Analysis of Human Menisci.

BACKGROUND: Medial meniscus posterior root (MMPR) tears are more commonly observed in female patients with preexisting osteoarthritis (OA); however, the histopathologic basis for medial root predilection and female predominance remains unclear. QUESTIONS/PURPOSES: (1) Is histopathologic degeneration associated with knee OA that selectively affects the MMPR? (2) Is greater histologic degeneration of the meniscus root insertion observed in female patients compared with male patients? (3) Do patients with varus alignment and severe OA exhibit greater tissue degeneration at the meniscus root inse

[Conservative treatment of femoral fractures in children in data from the Orthopedic Clinic of the 2nd Medical Faculty of Charles University].

PURPOSE OF THE STUDY: Conservative therapy is the method of choice for treating femoral fractures in younger children all over the world. At the Department of Orthopedic Surgery of the Second Faculty of Medicine, Charles University in Prague, this approach has had a long tradition and has only partly been replaced by other, more recent methods. It is not always necessary to abandon well-established and reliable techniques because of new achievements and this is demonstrated by the results of our study. MATERIAL: In a period of 22 years, 112 boys and 97 girls were treated by Bryant's traction

[Bioimpedance analysis parameters and metabolic profile in children with non- alcoholic fatty liver disease].

UNLABELLED: Non-alcoholic fatty liver disease (NAFLD) in children is considered a manifestation of metabolic disturbances, that is a complex of interrelated alterations in carbohydrate and lipid metabolism associated with insulin resistance and an increased proportion of visceral fat. In this context, a comprehensive assessment of metabolic parameters combined with body composition analysis in children with NAFLD and without it is relevant for clarifying the role of visceral obesity in the development and progression of the disease, as well as for improving the accuracy of clinical assessment.

Association Between Preoperative Joint Line Convergence Angle and Clinical Outcomes After Isolated Medial Meniscus Posterior Root Tear Repair.

BACKGROUND: While coronal malalignment influences outcomes after medial meniscus posterior root tear (MMPRT) repair, alignment parameters such as the hip-knee-ankle angle (HKAA) may not fully capture the biomechanical setting of the medial compartment. The joint line convergence angle (JLCA) reflects the intra-articular environment but has not been well studied as a prognostic factor after MMPRT repair. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the association between the preoperative JLCA and patient-reported outcomes (PROs) after MMPRT repair and to determine whether the

Is the Lind or LaPrade Technique Able to Control Anterior and Rotational Laxity in Medial-Side Knee Injuries? A Controlled Laboratory Study of 18 Knees.

BACKGROUND: Reconstruction of the medial collateral ligament (MCL) and posterior oblique ligament (POL) is necessary to restore stability in chronic lesions on the medial side of the knee. The Lind technique uses a single-strand, pedicled semitendinosus tendon autograft to perform continuous reconstruction of the MCL and POL. The LaPrade technique uses 2 free grafts: 1 graft for the MCL (double-strand semitendinosus tendon graft) and 1 graft for the POL (double-strand gracilis tendon graft). PURPOSE: To investigate the LaPrade and Lind techniques in restraining tibial translation and rotation
