Necrotizing fasciitis of the thigh-a devastating sign of intestinal perforation.

BACKGROUND: The incidence of necrotizing soft tissue infections is increasing, and associated mortality remains high. It is important to make an early diagnosis, but diverse atypical presentations occur. PATIENT: An older adult patient was admitted with sepsis syndrome without localizing clues. Necrotizing fasciitis of the right thigh and gluteus developed, and CT revealed an adjacent inflammatory cecum mass which was diagnosed during surgery as perforated adenocarcinoma. CONCLUSION: A review of the literature revealed that this rare type of necrotizing soft tissue infection is characteristi

Case Report: Intestinal mycobacterium abscessus infection in a child.

The diagnosis and treatment of Mycobacterium abscessus infections present significant challenges, especially in the rare cases of extrapulmonary involvement in pediatric patients. These cases are characterized by diagnostic difficulties, limited therapeutic options, scarce clinical experience, and a lack of evidence-based treatment guidelines. This article reports on a 6-year-old child who experienced fever and abdominal pain. Metagenomic next-generation sequencing (mNGS) facilitated the rapid and accurate identification of Mycobacterium abscessus as the causative pathogen. Under a standardize

STING activation promotes intestinal mucin secretion in IBD.

BACKGROUND & AIMS: Goblet cells produce mucus protecting the intestinal epithelium. Depleted goblet cells and a disrupted mucus layer are observed in inflammatory bowel diseases (IBD). Remarkably, both deficiency and overactivation of the cyclic GMP-AMP synthase-stimulator of interferon genes (cGAS-STING) pathway lead to goblet cell depletion in mice. Here, we analyzed the role of cGAS-STING in goblet cell mucus regulation in IBD patients and human crypt-derived colonic organoids (HCO). METHODS: MUC and cGAS-STING pathway gene expression was analyzed in inflamed and non-inflamed mucosa (n = 8

Gastro-intestinal parasites of camels (Camelus dromedarius) - a review.

Domesticated camels (Camelus dromedarius) play a crucial role in the livelihood of people inhabiting arid and semi-arid regions of the world. Despite their adaptation to harsh desert environments, camels harbor a wide diversity of gastrointestinal parasites, including protozoa, helminths, and cestodes, many of which significantly affect animal health, productivity, and public health. This review summarizes the current knowledge on the gastrointestinal parasites of dromedary camels, with emphasis on their prevalence, epidemiology, pathogenicity, zoonotic importance, and control strategies. Multifactorial ecological framework for intestinal-type gastric cancer.

Gastric cancer (GC) remains one of the leading causes of cancer-related mortality worldwide, with projections indicating increasing incidence and marked geographic variability across populations. Although multiple hypotheses have been proposed to explain this heterogeneity, including those related to Helicobacter pylori infection and its management, no comprehensive framework currently integrates the diverse determinants of GC risk. Given the multifactorial nature of gastric carcinogenesis, there is a need for integrative approaches that consider biological, environmental, behavioural, and mic

Gastric Lactobezoar: A Rare Cause of Intestinal Obstruction in Neonates.

Gastric lactobezoar, a pathological collection of milk and mucus in the stomach of milk-fed infants, can present with features of gastric outlet obstruction. A 3-day-old neonate developed abdominal distension with no hemodynamic instability. Abdominal X-ray was suggestive of gastric pneumatosis with heterogeneous opacities in the gastric region indicative of lactobezoar. The patient was managed conservatively with intravenous fluids and gastric lavage at regular intervals. We highlight this case due to its rare presentation, and when diagnosed early using a simple bedside radiological test, it

IL-15 Promotes Inflammatory Th17 Cells in the Intestine.

Ulcerative colitis (UC) is a chronic gastrointestinal condition with high morbidity. Modern therapies have revolutionized the care of UC, but 10% to 25% of patients do not respond to treatments and many progress to surgery. Thus, developing new treatments remains an important goal in UC. T cells, especially T helper 17 (Th17) cells, are linked with the pathogenesis of UC and are thought to be major targets of medications in UC. Therefore, we considered cytokines that may regulate pathogenic T cells in UC, focusing on cytokines that regulate T cells in tissue, as the intestinal microenvironment

Elderly Onset Primary Intestinal Lymphangiectasia-A Rare Case.

Primary intestinal lymphangiectasia (PIL) is a rare protein-losing gastroenteropathy characterized by diffuse or localized ectasia of the enteric lymphatics, which can be accompanied by lymphatic abnormalities in other parts of the body. This condition results in hypoalbuminemia, hypogammaglobulinemia, and lymphopenia due to the abnormal leakage of lymphatic fluid into the gastrointestinal tract. As there are no specific serological or radiological tests available, the gold standard for diagnosing intestinal lymphangiectasia is endoscopic examination with histopathological examination of intes

Effects of methylglyoxal on intestine and microbiome composition in aged mice.

BACKGROUND: Methylglyoxal (MGO), a highly reactive precursor of advanced glycation end products (AGEs), is endogenously produced and prevalent in various ultra-processed foods. MGO has emerged as a significant precursor implicated in the pathogenesis of type 2 diabetes and neurodegenerative diseases. To date, the effects of dietary MGO on the intestine have been limited explored. Thus, this study investigates the impact of prolonged oral administration of MGOs on gut health in aged mice. METHODS: Aged mice received MGO chronically (100 mg/kg/day) for 4 weeks Intestinal samples were analyzed u

Mucosal Macrophages Govern Intestinal Regeneration in Response to Injury.

BACKGROUND & AIMS: Radiation-induced enteritis develops in cancer patients treated with radiotherapy in the abdominal and pelvic cavity, a condition that impairs their quality of life. Radiation injury depletes proliferative intestinal stem cells; in response to this, the epithelium activates a regenerative program that facilitates the healing of the intestine. However, the mechanisms that induce the activation of the intestinal regenerative program are poorly characterized. METHODS: In this study, we induced radiation-induced enteritis in mice through abdominal irradiation, mimicking clinica

Loss of NKCC1 Activates the NLRP3 Inflammasome in Intestinal Epithelia.

BACKGROUND & AIMS: Potassium and chloride efflux have been reported to regulate nucleotide-binding oligomerization domain, leucine-rich repeat, and pyrin domain-containing protein 3 (NLRP3) inflammasome activation. Aberrant activation of NLRP3 inflammasome causes autoimmune and chronic inflammatory diseases. The Na+-K+-2Cl- cotransporter (NKCC1) maintains the intracellular concentrations of sodium, potassium, and chloride. Here we ask whether NKCC1 modulates NLRP3 inflammasome activation. METHODS: Mice with intestinal epithelial-specific deletion of NKCC1, CRISPR-Cas9 NKCC1-deleted Caco-2 and

Crosstalk between the microbiota and intestinal dendritic cells in IBD.

Conventional dendritic cells (cDCs) play a pivotal role in orchestrating the delicate balance between immunity and tolerance within the gastrointestinal tract by interacting with other cell types, particularly T cells. Meanwhile, the microbiota is critical for the induction and modulation of the immune system in the gut and plays a key role in the function of cDCs. So far, the study of intestinal cDCs has been encumbered by their limited numbers and phenotypic overlap with other myeloid cells. Recent advancements in single-cell sequencing technology have helped define cDCs and their subsets, w

Clonal persistence dominates homeostatic intestinal IgA responses.

Immunoglobulin A (IgA) is the most abundantly produced antibody isotype and mediates protection and homeostatic regulation at mucosal surfaces. Steady-state IgA production is supported by multiple pathways, including chronic germinal centers in gut inductive lymphoid tissues. However, we lack a detailed understanding of how IgA responses are temporally integrated across inductive and effector sites. Here, we dissect homeostatic IgA responses from the perspective of clonal repertoires in inductive compartments and the gut lamina propria as the main effector compartment. We show that unique clon
