[Rupture of the anterior cruciate ligament. Current status of treatment].

This article summarizes the present knowledge on the diagnosis of and treatment rationales for ruptures of the anterior cruciate ligament (ACL) of the knee. There is an increasing incidence of this injury due to the high number of persons involved in dynamic sports. The most significant diagnostic criterion is a positive pivot shift associated with a pathological anterior translation of the tibia in slight flexion of the knee. Instrumented testing of the knee is becoming increasing important and is standard in follow-up studies. A survey of the literature at present delineates very clearly the

Ultrasonographic scan in knee pain in athletes.

Fifty-two knees were examined using real-time high-definition ultrasonography with a 7.5 MHz probe. The extra-articular structures were easily visualized and diagnosis of patellar tendon lesions and Baker's cysts formulated. While the meniscal cartilages were shown as a homogeneous triangular structure between the femoral condyle and the tibial plateau, no lesions were detected. Deeper intra-articular structures, such as the cruciate ligaments, were not shown by the scan, thus their evaluation was not possible. Given its low cost, wide availability, non-invasiveness and patients' acceptability

Robotic assessment of proprioception in individuals with patellar tendinopathy: A feasibility and pilot study.

BACKGROUND: Individuals with patellar tendinopathy often exhibit impaired proprioception, which may contribute to symptom persistence and injury recurrence. A parallel robotic system based on a musculoskeletal knee model has shown valid and reliable performance for assessing joint position sense (JPS). This study aims to evaluate the feasibility of a protocol for the robotic assessment of knee proprioception in individuals with patellar tendinopathy and to compare their proprioceptive performance with that of healthy controls. METHODS: Twenty-one participants with patellar tendinopathy (PTG)

The Combination of Immunomodulatory Secretome and Liposome-Bound TRAIL Improves Knee Osteoarthritis Symptoms in an Ovine Model.

Background/Objectives: Knee osteoarthritis stands as the highest prevalent joint disease worldwide, affecting millions of adults and significantly impairing mobility and quality of life. Pro-inflammatory cells and cytokines are considered key players in the pathophysiology of the disease. In previous work, two anti-inflammatory therapeutic approaches were developed: a secretome enriched in anti-inflammatory cytokines, and nanoliposome-bound TRAIL (LUV-TRAIL), with proven efficacy against rheumatoid arthritis in rabbits. Methods: In this work, we evaluated the ability of these treatments to pre

Effect of Adapted Ergometer Setup and Rowing Speed on Lower Extremity Loading in People with and Without Spinal Cord Injury.

BACKGROUND: Functional electrical stimulation-assisted rowing (FES rowing) is a rehabilitation exercise used to prevent disuse osteoporosis, which is common in people with spinal cord injury (SCI). However, its effect on bone loss prevention varied in SCI patients, potentially due to inconsistent loading. This study investigates the effect of ergometer setup and rowing speed on lower extremity loading during rowing. METHODS: Twenty able-bodied participants and one participant with SCI rowed on an adapted ergometer with different speeds and setups. We calculated foot reaction force and knee mo

Increased Degenerative Biomarkers in Females with Patellofemoral Pain: A Cross-Sectional Analysis with 6-Month Progression.

BACKGROUND/OBJECTIVES: Patellofemoral pain (PFP) is considered a risk factor for knee osteoarthritis (OA) onset. The purpose of this study was to compare degenerative biomarkers in females with and without PFP and to determine changes in these levels, along with pain and function, over 6 months. METHODS: All subjects received a knee x-ray to ensure that none had degenerative changes. Urine and serum were collected and analyzed for C-telopeptide fragments of type II collagen (CTX-II) and C-propeptide II (CP-II); these were then expressed as a cartilage degradation: cartilage synthesis ratio (C

Prepatellar bursa mucosa: An unreported postoperative complication of patellar fracture.

BACKGROUND: Patellar fracture is a common injury caused by overstrain of the extensor mechanism or a direct trauma to the knee. The most common complications after patellar fracture are nonunion, infection, post-traumatic arthritis, joint fibrosis, symptomatic hardware, and extensor mechanism failure. These are attributed to the post-fracture flexion and extension movements, the primary damage to the articular cartilage, and the thin soft tissue coverage over the knee. To our knowledge, prepatellar bursa mucosa is an unreported postoperative complication of patellar fracture. METHODS: We desc

[Medial Hoffa fracture: a case report].

INTRODUCTION: knee femoral condyle coronal fractures or Hoffa fractures are among the rarest fractures of the pelvic limb, being even more rare those of the medial condyle. The success in the management of these patients lies in the timely diagnosis and the consequent anatomical reduction of the femoral articular surface. CLINICAL CASE: the objective of this article is to present a patient with right medial Hoffa fracture, registered as a single case in the archives of our hospital. The patient was referred to us on November 2011, with a 10-day course after suffering a fall from horse ridding

Bilateral Total Knee Arthroplasty in a 15 Year Old With Skeletal Dysplasia and Open Physes.

There are limited reports of total knee arthroplasty (TKA) in the pediatric population. The use of constrained, stemmed components is particularly unusual in the setting of open physes due to concerns with growth arrest and implant survivorship. The current authors describe a 15-year-old boy with open growth plates and an epiphyseal dysplasia-like condition affecting the knees bilaterally. He had no other significant medical or orthopedic conditions and his genetic workup was negative. He had severe knee pain and a bilateral 40° valgus with severe lateral compartment arthritis. Conservative tr

Insulin growth factors may explain relationship between spasticity and skeletal muscle size in men with spinal cord injury.

The major objectives of this cross-sectional study were to (1) measure insulin-like growth factor-1 (IGF-1) in individuals with complete spinal cord injury (SCI) and spasticity and (2) determine the relationships between IGF-1 and cross-sectional areas (CSAs) of thigh skeletal muscle groups. Eight individuals with motor complete SCI underwent magnetic resonance imaging to measure the CSA of the whole thigh, knee extensor, and knee flexor skeletal muscle groups and dual-emission X-ray absorptiometry to measure fat-free mass. After participants fasted for 12 h, we measured their IGF-1 levels and

Heightened flexor withdrawal responses following ACL rupture are enhanced by passive tibial translation.

OBJECTIVE: Hyperexcitability of nociceptive pathways has been demonstrated with several musculoskeletal conditions but not anterior cruciate ligament (ACL) injury. The purpose was to investigate flexor withdrawal reflex (FWR) excitability following ACL rupture and determine if painless stretch of knee joint structures enhanced reflexive responses. METHODS: Ten subjects with and 10 subjects without unilateral ACL rupture were compared. FWRs were induced through sural nerve stimulus in symmetrical stance and recumbent positions, with the knee in relaxed and stressed condition. Latencies and amp

The use of selective electrical stimulation of the quadriceps to improve standing function in paraplegia.

Persons with spinal cord injury (SCI) can benefit significantly from functional neuromuscular stimulation (FNS) systems for standing if manual tasks can be performed while upright. Using FNS to sufficiently activate the knee extensors to rise from a sitting position often results in inadvertent activation of the rectus femoris and/or sartorius, which flex the hip. In this study, intramuscular electrodes implanted in the vastus lateralis and medialis of four subjects with SCI were used to activate these muscles individually and simultaneously to measure knee extension moment. Support forces app
