The results from an ultrasound study performed on an 11-year-old boy, who had several weeks of intermittent abdominal pain, showed a nonperistaltic loop of bowel near the umbilicus, which suggested either an internal hernia or a diverticulum. Tc-99m pertechnetate imaging showed a focal collection near the umbilicus, which was consistent with a Meckel's diverticulum. No evidence was found of gastrointestinal bleeding. Gallbladder agenesis is a rare condition that results from the failure of the cystic bud to develop in the 4th wk of intrauterine life. OBJECTIVE: To report the case of an unusual form of protein-losing enteropathy.
PATIENT: A 54-year-old patient with hypoechoic tumours resembling lipomas in the mesentery.
INTERVENTIONS: Following examination by ultrasound and computed tomography, the well-known intestinal and extra-intestinal forms of protein-losing enteropathy were excluded and a diagnostic laparotomy was performed. The whole mesentery was infiltrated by a large unresectable tumour. Histological examination showed that it was a sclerosing mesenteritis.

The surgical management of fifteen cases of duodenal trauma is reported. Blunt injury of the abdomen was the commonest mode of injury. Thirteen patients (86.6%) had associated intra-abdominal and/or extra-abdominal injuries. Diagnosis of duodenal injury was made at laparotomy in all patients. In the majority of patients (n = 11), either duodenal repair, or anastomosis with the pylorus or jejunum was done. Danish patients with Peutz-Jeghers syndrome are registered in the Danish Polyposis Register. We have initiated a new follow-up programme based on recent literature including our own cases. This has been adjusted to the regrowth rate of the polyps whereby patients with no symptoms and low regrowth rate are followed with few examinations. The medical records of 15 patients found to have pneumoretroperitoneum in a 10-year period were reviewed. The cause of retroperitoneal emphysema was infection in six patients, trauma in five and iatrogenic in four. Emphysema was confined to the retroperitoneum in 11 patients, and extended to the mediastinum in four and to the soft tissue of the neck in three. Spontaneous expulsion of a small portion of the intestinal mucosa per rectum, in an infant, associated with ileocolic intussusception is an extremely rare condition. This appears to be the first report of such a clinical presentation. The initial diagnosis was raised by the histopathologist following examination of the eliminated piece of tissue. A case of malakoplakia of the small intestine presenting as an abscess of the anterior abdominal wall is described in a 28-year-old black Zimbabwean man. The patient developed an enterocutaneous fistula after incision and drainage of the abscess. Light and electronic microscopy revealed Michaelis-Gutmann bodies in some von Hansemann cells. He did well for eight months following laparotomy and resection of the fistula, but later died of a severe chest infection and septicaemia. A study was carried out on the presence of antibodies to C. trachomatis in women with ectopic pregnancy, established during laparotomy. Appendicitis can occur rarely in association with carcinoma of the caecum, particularly in the elderly. If this possibility is not appreciated and thorough laparotomy is not carried out at the time of initial surgery, an early malignant lesion may go unrecognised. Fallopian tube segments were removed from 20 women undergoing sterilization by laparoscopy or laparotomy. Ten of these patients used an intrauterine contraceptive device (IUCD). Five patients with a hysterosalpingographic diagnosis of unicornuate uterus underwent magnetic resonance imaging (MRI) and subsequently laparoscopy/laparotomy to evaluate the ability of MRI to identify the various subclasses of this malformation. This study represents our experience with severe blunt gastrointestinal injury in 29 polytrauma and 3 solitary trauma patients. The primary diagnosis was missed in intestinal perforation (4x) and mesenteric disrupture (1x). In 2 other patients contusion of the gastrointestinal wall led to postprimary perforation. The average mortality was 21.8%; only 2 patients died from abdominal sepsis.
