Public Health Nursing as Connective Infrastructure: A Health Policy Perspective on Family Homelessness and Health Equity.

Family homelessness is shaped by social and structural determinants of health, including poverty, housing insecurity, fragmented services, and unequal access to support. Families with infants and young children are particularly vulnerable to the health and developmental effects of housing instability. Public Health Nurses (PHNs) are uniquely positioned at the interface of health services, family life, and community systems. Through universal reach, relational continuity, developmental surveillance, safeguarding, advocacy, and interagency coordination, PHNs help families remain connected to car

Is Papaya Leaf Extract the Sweet Remedy for Dengue-Induced Thrombocytopenia?

Dengue fever, caused by the dengue virus and transmitted by Aedes mosquitoes, poses a significant health challenge in tropical regions like Trinidad and Tobago, with severe cases leading to life-threatening complications. This case study presents a 65-year-old female with dengue exhibiting warning signs, characterized by persistent fever, abdominal pain, and thrombocytopenia. Despite supportive care, including intravenous fluids and symptomatic treatment, her platelet count remained critically low at 12.1 x 109/L. Following the introduction of diluted papaya leaf extract into her treatment reg

Children with medical complexity and paediatric palliative care: a retrospective cross-sectional survey of prevalence and needs.

BACKGROUND: Children with medical complexity (CMC) have been defined (Cohen et al., Pediatrics 127: 529-538, 2011.) as an emerging population potentially eligible for PPC. The current study investigated the prevalence of children with medical complexities eligible for a local palliative care network, including a paediatric hospice. METHODS: A retrospective cross-sectional survey has been conducted using children clinical charts from 14 local health authorities of our region (Emilia Romagna, Italy). RESULTS: The total number of children with life-limiting conditions was 601, with a mean age o

Extracorporeal Membrane Oxygenation as Salvage Therapy in the Peripartum Period: A Case Series.

Despite considerable advances in maternity care, maternal death rates remain unacceptably high. Even with optimal care, unexpected complications can result in catastrophic consequences. Hemorrhage, cardiovascular and coronary conditions, and cardiomyopathy make up the three most common causes of pregnancy-associated deaths, followed by sepsis and thromboembolic disease. Although a number of deaths may be deemed to be potentially avoidable with appropriate education and infrastructure, others such as refractory hypoxia and peripartum cardiomyopathy are not. All possible interventions should be

Ethical Considerations in Premortem Heparin Use for Organ Donation after Circulatory Death: A Case Study.

Donation after circulatory death (DCD) has become a key avenue for expanding the organ donor pool. Many DCD protocols include premortem intravenous heparin to reduce microthrombus formation and preserve graft viability. This presents the plausible risk of premortem bleeding from an intervention that offers no direct therapeutic benefit to the donor. Ethical concerns and moral distress can arise when potential donors have a catastrophic intracranial hemorrhage with ongoing physiological concern for bleeding risk that is not survivable, but do not currently meet brain death criteria. While prior

Perceived Impact on the Daily Lives of Patients With Spinal Muscular Atrophy Treated With Nusinersen: A Natural Language Processing Approach.

OBJECTIVE: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder caused by SMN1 mutations, leading to progressive muscle weakness. Although nusinersen improves motor function, its broader impact on daily life remains insufficiently understood. This study explored caregivers' perceptions of nusinersen treatment and identified themes reflecting its influence on everyday functioning. METHODS: An observational study included 23 children with SMA (10 type 2, 7 type 3, 6 type 1) treated with nusinersen for up to 36 months. The mean age at interview was 6.3 years (SD: 2.3). Caregivers compl

Molar Incisor Hypomineralization and Morphological Defects Following Pediatric Septicemia: A Case Report.

INTRODUCTION: Early-childhood septicemia may disrupt odontogenesis, yet documented dental sequelae remain limited. This report outlines the clinical and radiographic features of two pediatric septicemia survivors who later developed molar-incisor hypomineralization (MIH) and morphological tooth defects. METHODS: Two pediatric patients (ages 12 and 6) with documented early-life methicillin-sensitive Staphylococcus aureus (MSSA) and Neisseria meningitidis were evaluated. Both exhibited MIH, limb amputations, chronic lifelong medical comorbidities, limb amputations, and dental morphological anom

Progress in the pharmacological management of vasovagal syncope.

INTRODUCTION: Vasovagal syncope is a frequent problem affecting nearly half the population. The symptom burden is wide, ranging from once to hundreds of times. It reduces quality of life and causes injuries. Physicians, particularly front-line physicians such as family doctors, need a simple, practical approach. AREAS COVERED: We provide context on syncope frequency, competing diagnoses, and natural history, usually characterized by improvement and remissions. Quality of life is reduced and accompanied by anxiety and improves with care. Injuries are common but serious injuries are rare. Treat

Factors influencing quality of life in patients with temporal lobe epilepsy.

BACKGROUND: Temporal lobe epilepsy (TLE) predisposes individuals to cognitive difficulties and psychosocial consequences. Evaluating its impact on quality of life (QOL) is essential for patient care. OBJECTIVE: To identify factors influencing QOL in low-income patients with TLE. METHODS: An institution-based cross-sectional study was conducted on 40 patients with TLE during neurological consultations at a day clinic in Agadir, Morocco. The Quality of Life in Epilepsy Inventory-31 (QOLIE-31) was used to measure QOL. Multivariate linear regression analysis was performed to assess the associati

Exploring the Connection between Social Housing and Employment: A Scoping Review.

Adequate housing is a social determinant of health and well-being, providing stability from which people can engage in important life activities, including self-care and productivity. Social housing is a system-level intervention that aims to provide affordable housing to people vulnerable to experiencing social and economic marginalisation. Given the importance of employment to social-economic status and overall health and well-being, we sought to better understand the available knowledge and research related to employment and living in a social housing environment. We used scoping review met

Successful on-ECLS Repair of CDH and Omphalocele in a Newborn.

Both congenital diaphragmatic hernias (CDHs) and omphaloceles show relevant overall mortality rates as individual findings. The combination of the two has been described only sparsely in the literature and almost always with a fatal course. Here, we describe a term neonate with a rare high-risk constellation of left-sided CDH and a large omphalocele who was successfully treated on extracorporeal life support (ECLS). Prenatally, the patient was diagnosed with a large omphalocele and a left CDH with a lung volume of ∼27% and an observed to expected lung-to-head ratio of 30%. Due to respiratory i

Aging with spinal cord injury: A narrative review of consequences and challenges.

Given the increase in life expectancy, aging with a pre-existing spinal cord injury (SCI) is becoming more common. This condition is challenging as compromised health status and functional independence can worsen. We aimed to provide an updated overview of the consequences of aging with SCI, highlighting the main challenges facing this population in a narrative review of the current literature we retrieved from the PubMed database from 2000 to 2022 on any aspect related to aging in persons with SCI. Here we address adverse circumstances that increase disability and hinder an active lifestyle,

Chronic intestinal failure and short bowel syndrome in Crohn's disease.

Chronic intestinal failure (CIF) is a rare but feared complication of Crohn's disease. Depending on the remaining length of the small intestine, the affected intestinal segment, and the residual bowel function, CIF can result in a wide spectrum of symptoms, from single micronutrient malabsorption to complete intestinal failure. Management of CIF has improved significantly in recent years. Advances in home-based parenteral nutrition, in particular, have translated into increased survival and improved quality of life. Nevertheless, 60% of patients are permanently reliant on parenteral nutrition.

Addressing unmet needs in rare bleeding disorders: selected poster extracts of recent research in hemophilia A and von Willebrand disease presented at the 14th Annual Congress of the European Association for Haemophilia and Allied Disorders (EAHAD) (Feb 3-5, 2021; virtual congress).

Hemophilia A and von Willebrand disease (VWD) are inherited rare bleeding disorders affecting normal hemostasis. Patients with VWD, especially those with severe disease types, share some similarities to patients with hemophilia A in their burden of disease: they suffer from an increased risk of potentially severe and life-threatening bleeds and associated long-term consequences, such as impaired joint health and overall lower quality of life. However, the two bleeding disorders differ in their primary cause and affected patient population, and comprise a range of different bleeding phenotypes

Effect of a surfactant-based gel on patient quality of life.

The characteristic clinical signs of chronic wounds, which remain in a state of prolonged inflammation, include increased production of devitalised tissue and exudate, pain and malodour. The presence of necrotic tissue, slough and copious exudate encourages microbial proliferation, potentially resulting in planktonic and/or biofilm infection. For patients, the consequences can include leakage of exudate, pain and reduced mobility, which can impair their ability to socialise and perform activities of daily living. This can severely reduce their quality of life and wellbeing. Concentrated surfac

Concealed concern: fathers' experiences of having a child with juvenile idiopathic arthritis.

Despite increased research into families of chronically ill children, more needs to be known about the father's experience. We address this issue through asking: 'What is it like to be the father of a child with Juvenile Idiopathic Arthritis?' (JIA). Four members of eight families with an adolescent diagnosed with JIA, including seven fathers, were interviewed and transcripts analysed using grounded theory. This study suggests that fathers of children with JIA experience several severe losses which are exacerbated through comparisons they make between their own situation and that of fathers of

Impact and management of skin toxicity associated with anti-epidermal growth factor receptor therapy: survey results.

Biologic agents targeting the epidermal growth factor receptor (EGFR) have emerged as a robust treatment option for various solid tumors. Despite lower systemic side effects than conventional chemotherapy, the majority of patients treated with these agents experience cutaneous toxicities, including papulopustular rashes, hair and nail changes, xerosis and pruritus, which have a significant impact on health and quality of life. Currently no consensus or management guidelines exist for these untoward events. Therefore, a retrospective survey was carried out across 110 oncology practioners in the

Use of the MQoL-HIV with asymptomatic HIV-positive patients.

The purpose of this study was to determine the appropriateness of the recently developed Multidimensional Quality of Life Questionnaire for HIV/AIDS (MQoL-HIV) as a measure of quality of life (QoL) in cases of asymptomatic human immunodeficiency virus (HIV) infection. The MQoL-HIV is a 40-item instrument measuring ten domains (mental health, physical health, physical functioning, social functioning, social support, cognitive functioning, financial status, partner intimacy, sexual functioning and medical care) relevant to HIV infection. An overall QoL score, the MQoL-HIV Index, is a weighted co
