A survey study of resistance to discharge from speech-language therapy.

A survey was designed to investigate the previously unaddressed issue of resistance to discharge from speech therapy. One hundred and eighty-seven speech-language clinicians in clinical and hospital settings were sent a questionnaire based on research from the medical profession indicating that patients may become overly dependent on their physicians. Eighty-six percent of 159 usable respondents reported their clients resisted discharge. Many speech-language pathologists suggested techniques they have used to manage such clients.

Acne vulgaris. Current concepts of pathogenesis and treatment.

Although the basic cause of acne vulgaris remains unknown, considerable data that concern its pathogenesis and that have been accumulated in recent years allow a rational and therapeutically successful approach to the management of this disorder. To date, there is no single treatment for acne. Therapy must be individualized, with appropriate variations and modifications as the degree or severity of this disorder fluctuates. Today, this goal can be achieved by proper selection of available medications, coupled with the cooperation of the patient, and the knowledge, continued interest, and enthu

Congenital tricuspid incompetence simulating pulmonary atresia with intact ventricular septum: a report of two cases.

Two infants with isolated congenital tricuspid incompetence appeared to have associated right ventricular outflow tract obstruction at diagnostic evaluation, including catheterization and cineangiography. One infant died during surgery; the other improved rapidly and survived with medical therapy. We discuss the problem of establishing the presence of patency of the right ventricular outflow tract in infants with massive tricuspid incompetence and outline an approach to the management of infants whose diagnosis remains unclear even after careful evaluation.

Trauma and the child.

Despite better prevention programs and emergency medical care, trauma continues to be the leading cause of death in children. Children present very special anatomic, physiologic, and psychological problems to the emergency room physician, and the spectrum of injury in these young patients may be significantly different from that seen in adult trauma victims. These factors make immediate diagnosis and early appropriate therapy for these young patients imperative to a successful outcome. A few unique forms of injury have been reviewed in this article and their diagnosis and management discussed.

Systematic review: nutritional therapy in paediatric Crohn's disease.

BACKGROUND: At least 25% of individuals diagnosed with Crohn's disease (CD) have onset of disease in childhood. Almost all children with CD have nutritional impairments, such as weight loss or stunting, at diagnosis or subsequently. Nutritional therapy (exclusive enteral nutrition) is established as a valid and effective treatment in paediatric CD. The advantages of this approach are induction of remission and control of inflammatory changes, mucosal healing, positive benefits to growth and overall nutritional status, and avoidance of other medical therapies. AIM: To provide a comprehensive u

Acute migraine treatments in patients above age 65.

BACKGROUND: Migraine remains a prevalent and disabling condition in older adults, yet there are limited data guiding acute treatment in patients over the age of 65 years. This population is often underrepresented in clinical trials and there are special considerations to take with management in this population including age-related pharmacokinetic changes, comorbidities, and increased risk of medication-related adverse effects. OBJECTIVE: In this narrative review, we aim to highlight the current acute treatments for migraine and considerations that should be taken when choosing medications in

Spinal Cord Stimulation - Device Revision After Weight Loss in a Patient on Chronic Semaglutide - A Case Report.

BACKGROUND: Spinal cord stimulation (SCS) is an effective treatment option for patients suffering from chronic intractable pain of the trunk and/or limbs. One of the potential adverse effects of SCS is discomfort or pain at the site of the implantable pulse generator (IPG). It is a known phenomenon that patient weight loss may exacerbate or increase discomfort or pain at the IPG implantation site. OBJECTIVE: This case report aims to educate neuromodulation clinicians on the potential impact of glucagon-like peptide-1 receptor agonists (GLP-1RA) medications on patient weight loss and decreased

Evolution from electrophysiologic to hemodynamic monitoring: the story of left atrial and pulmonary artery pressure monitors.

Heart failure (HF) is a costly, challenging and highly prevalent medical condition. Hospitalization for acute decompensation is associated with high morbidity and mortality. Despite application of evidence-based medical therapies and technologies, HF remains a formidable challenge for virtually all healthcare systems. Repeat hospitalizations for acute decompensated HF (ADHF) can have major financial impact on institutions and resources. Early and accurate identification of impending ADHF is of paramount importance yet there is limited high quality evidence or infrastructure to guide management

Phenotype-driven management in ileocecal Crohn's disease: a narrative review with a proposed research framework.

The management of localized ileocecal Crohn's disease (CD) requires balancing escalation of medical therapy against early surgical resection. While clinical phenotyping guides initial management, standard assessments often fail to distinguish reversible inflammation from irreversible fibrosis. This limitation remains a major barrier to personalized care. This narrative review aims to review current evidence on clinical features, imaging modalities, and biomarkers relevant to intestinal fibrosis and to propose a phenotype-driven framework for treatment selection in localized ileocecal CD. Medical management of patients after bariatric surgery: Principles and guidelines.

Obesity is a major and growing health care concern. Large epidemiologic studies that evaluated the relationship between obesity and mortality, observed that a higher body-mass index (BMI) is associated with increased rate of death from several causes, among them cardiovascular disease; which is particularly true for those with morbid obesity. Being overweight was also associated with decreased survival in several studies. Unfortunately, obese subjects are often exposed to public disapproval because of their fatness which significantly affects their psychosocial behavior. All obese patients (BM

Cognitive-behavioral treatment of gender dysphoria in youth: Opportunities, challenges, and responsibilities.

During the past twenty years, the prevailing clinical approach to child and adolescent gender dysphoria changed dramatically. Before then, most clinicians considered youth gender dysphoria a mental health problem best addressed through psychological management. Typically, childhood gender dysphoria resolved prior to adulthood. Recently, however, the gender-affirming approach has prioritized facilitating social and medical gender transition of dysphoric youth. The ascendancy of gender-affirming care did not arise because of empirical advances. Rather, gender-affirming care triumphed because of

Septal Perforation Repair in a Patient With Medically Managed Granulomatosis With Polyangiitis Using Temporoparietal Fascia With Polydioxanone Sheet.

Granulomatosis with polyangiitis (GPA) is a systemic vasculitis with frequent otolaryngologic manifestations, including nasal septal perforation (NSP) secondary to ischemic necrosis of the septal cartilage. While conservative therapy is often first-line, surgical repair may be indicated in patients with symptomatic or refractory perforations. Limited literature exists regarding septal perforation repair in patients with GPA. We report 1-year postoperative results on the case of a 19-year-old male with a 5-year history of GPA, medically controlled on immunosuppressive therapy, who presented wit
