Pain surgery is one of the historic foundations of neurological surgery. The authors present a review of contemporary concepts in surgical pain management, with reference to past successes and failures, what has been learned as a subspecialty over the past 50 years, as well as a vision for current and future practice. This subspecialty confronts problems of cancer pain, nociceptive pain, and neuropathic pain. For noncancer pain, ablative procedures such as dorsal root entry zone lesions and rhizolysis for trigeminal neuralgia (TN) should continue to be practiced. Other procedures, such as medi…

Background: Trigeminal neuralgia (TN) is the most common cranial neuralgia in adults, with a slightly higher incidence in women than in men. This chronic pain condition affects the trigeminal nerve, also known as the 5th cranial nerve. It is one of the most deeply distributed nerves in the head. Antiseizure drugs are the main biomedical treatment of TN. However, TN is not the only source of facial pain. Background persistent idiopathic facial pain (PIFP) is also a chronic disorder, recurring daily for more than 2 hours per day over more than 3 months. PIFP occurs in the absence of a neurologic…

OBJECTIVE: This is a case report and review of the literature of aneurysm formation after stereotactic radiosurgery (SRS) in the posterior fossa. Cerebral aneurysm formation is not a commonly recognized complication of SRS. We present the first case of an unruptured anteroinferior cerebellar artery aneurysm incidentally found at surgery in a patient with trigeminal neuralgia secondary to a vestibular schwannoma (VS) first treated with Gamma Knife radiosurgery. Other cases of posterior fossa aneurysms associated with SRS and the pathogenesis of vascular injury by radiation are discussed.

OBJECT: Value of care is emerging as a promising framework to restructure health care, emphasizing the importance of reporting multiple outcomes that encompass the entire care episode instead of isolated outcomes specific to care points during a patient's care. The authors assessed the impact of coordinated implementation of processes across the episode of surgical care on value of neurosurgical care, using microvascular decompression (MVD) as an example.
METHODS: This study is a retrospective review of consecutive cases involving patients with either trigeminal neuralgia or hemifacial spasm …

BACKGROUND: Percutaneous radiofrequency trigeminal rhizotomy offers high rates of complete pain relief for classic idiopathic trigeminal neuralgia. When performed under fluoroscopy, it may cause anxiety for an inexperienced neurosurgeon owing to lack of familiarity with the regional anatomy, lack of reliable landmarks on fluoroscopy, and perceived risks associated with inadvertent puncture of neurovascular structures near the foramen ovale. The purpose of this study is to describe a new procedure to maximize patient security and shorten operative time.
METHODS: From January 2006 to May 2009, …

UNLABELLED: Study of humans with shingles or postherpetic neuralgia (PHN) is providing insights into pain mechanisms. Shingles pain is a combination of normal and neuropathic pain that reflects acute tissue and neural injury. PHN pain, which lasts after tissues have healed, is caused by persistent neural injuries. Spontaneous C-nociceptor activity has been documented in painful polyneuropathies and probably occurs in shingles as well, although there are no microneurographic studies of either shingles or PHN. It is uncertain if this persists in PHN since pathological examination of PHN-affected…

Herpes zoster or shingles is a frequent occurrence in both elderly individuals and immunocompromised hosts. The pain associated with herpes zoster is the most debilitating complication of the disease. It can be described as acute pain and post-herpetic neuralgia or zoster associated pain (ZAP). The latter definition encompasses pain from the onset of disease through its resolution and provides a convenient analytic tool for evaluation of antiviral therapy. A heuristic examination of ZAP historical data suggests the existence of three phases of pain resolution: the acute, subacute and chronic p…

The cellular distribution and changes in CX3CL1/fractalkine and its receptor CX3CR1 protein levels in the trigeminal subnucleus caudalis (TSC) of rats with unilateral infraorbital nerve ligation (IONL) were investigated on postoperation days 1, 3, 7, and 14 (POD1, POD3, POD7, and POD14, respectively) and compared with those of sham-operated and naïve controls. Behavioral tests revealed a significant increase in tactile hypersensitivity bilaterally in the vibrissal pads of both sham- and IONL-operated animals from POD1 to POD7, with a trend towards normalization in sham controls at POD14. The therapy of chronic facial pain still poses an important challenge. A therapeutical scheme in four steps has been developed. This scheme enables modification of the pain therapy depending on localisation, intensity and former treatment. It consists of the following steps: 1. Transcutaneous electrical nerve stimulation. 2. Medication 3. Extracranial glycerol blocking of the trigeminal nerve 4. Neurosurgical treatment with thermocoagulation, chemical glycerol-rhizotomy and microsurgical decompression. The different therapeutical steps have no influence on each other.

The authors present the clinico-pathological correlation in a women who developed at the age of 43 years progressive pain in the left hemiface with motor and sensitive disorders of the left trigeminal nerve. Progressive involvement of the left IIIrd, IVth and Vth cranial nerves appeared with persistence of intractable facial pain which was not responsive to treatment by Tegretol, nor by thermocoagulation of the left Gasserian ganglion. Partial resection of a tumor was performed. The biopsy revealed its cylindromatous nature. The patient died after eight years of evolution. Autopsy confirmed an

OBJECTIVE: The study aimed to assess the long-term durability of pain relief among patients with cervicogenic headache (CGH) or occipital neuralgia (ON) following percutaneous 60-day peripheral nerve stimulation (PNS) treatment.
BACKGROUND: Cervicogenic headache (CGH) and occipital neuralgia (ON) are challenging headache conditions with significant impact on function and quality of life. The present feasibility study initially reported that percutaneous 60-day PNS can reduce headache, facilitating functional improvements such as improved quality of life and reduced neck disability. Neuropathic pain (NP) is a type of pain resulting from damage or dysfunction of the somatosensory system. Emerging evidence suggests that lipid-lowering drugs may influence the development or progression of NP. This study's objectives are to examine the association between statin use and diabetic peripheral neuropathy (DPN) using NHANES cross-sectional data, and to evaluate whether genetically proxied inhibition of HMGCR, PCSK9, and NPC1L1 is associated with DPN, trigeminal neuralgia (TN), and postherpetic neuralgia (PHN). In the NHANES component, we conducted a cross-sectional analysis of adu…
