Motor/vocal tics and compulsive behaviors on stimulant drugs: is there a common vulnerability?

The occurrence of abnormal movements or perserverative/compulsive behaviors was noted in 34 (76%) of a group of 45 hyperactive boys during a double-blind crossover treatment trial of methylphenidate and dextroamphetamine given in a wide range of doses. These adverse effects were often subtle and transient, and they usually occurred only on one drug. There was only one case where treatment was discontinued due to the severity of the tic the subject developed during his initial treatment phase. Dextroamphetamine tended to produce more compulsive behaviors, which were also more likely to resemble

[Neurophysiological studies on group A xeroderma pigmentosum in early childhood].

Neurophysiological studies were performed on 8 patients with group A xeroderma pigmentosum during early childhood. EEG, ABR and NCV were normal during this period. In contrast, various sleep parameters detected by polysomnography showed abnormal findings even in the neurologically normal patient. Decreased % sleep REM was seen in a case, and decreased frequency of REMs were seen in another. Body movements were extremely high or low in frequency in 3 cases in whole night sleep. The distribution of body movements were abnormal; in control subjects, the frequency was higher in SREM and stage 1 th

An incident case-referent study of febrile convulsions in children: genetical and social aspects.

In a prospective community-based study of 103 children with febrile convulsions (FC), social and genetical factors were compared with 193 age and sex matched referents sampled from the community. FC were found more often among parents of cases than referents (odds ratio 21.0; p less than 0.001). A history of FC in any type of relative was found in 39.8% of cases and 10.4% of referents (odds ratio 5.8; p less than 0.001). The familial occurrence of FC is best described by a multifactorial (polygenic) model. No difference was found between groups in the occurrence of other diseases than FC affec

Cervicomedullary compression in young patients with achondroplasia: value of comprehensive neurologic and respiratory evaluation.

We studied prospectively 26 young patients with achondroplasia to test two hypotheses: that respiratory problems may be the result of occult spinal cord compression, and that achondroplastic patients with cord compression might have occult respiratory abnormalities. Respiratory abnormalities were present in 85%, the majority caused by a primary problem of the pulmonary system, such as small thoracic cage or obstructed airway. Three patients had hypoxemia, recurrent cyanotic spells, and episodes of respiratory distress explainable only by cervicomedullary cord compression; in each patient, resp

Complications after subclavian-cortical middle cerebral artery bypass.

Subclavian-cortical middle cerebral artery bypass by saphenous vein interposition is an uncommon recent revascularization technique used for immediate volume flow to the brain or when donor arteries are unavailable or inadequate. We used this technique in four cases of symptomatic common carotid artery occlusion. In the last two cases, severe, reversible neurological complications occurred. We think that these complications are due to this particular technique, and more cautious indications than for traditional extra-intracranial arterial bypass are recommended.

Multimodality evoked potentials and neurophysiological tests in multiple sclerosis. Effects of hyperthermia on test results.

Data on critical frequency of photic driving (CFPD), frequency following response (FFR), and visual, somatosensory (peroneal nerve), and brain-stem auditory evoked potentials (EPs) were obtained from 20 patients who had clinically definite multiple sclerosis and ten healthy normal subjects in a controlled, balanced study under normothermic and hyperthermic (+1 degrees C) conditions with a test-retest interval of one week. Normal subjects' test results showed no changes during hyperthermia. Patients' EP and CFPD data correlated well with history, clinical signs, and symptoms during both normoth

Prognosis of neonates with symptomatic respiratory insufficiency surviving with the aid of ventilator therapy.

Twenty-one survivors representing all survivors of neonatal symptomatic respiratory distress treated with intermittent positive pressure ventilation have been follow-up at an age of 2.9 to 7.0 years. The chance of survival proved to be relatively favourable in infants ventilated for perinatal pneumonias and unfavourable in infants ventilated for haemorrhagic diseases and respiratory insufficiency secondary to surgical conditions. At the follow-up half of the children presented with neurological symptoms but only 10% were found severely handicapped. One infant had a tracheostomy due to a laryng

Clinical testing of the radiosensitiser Ro-07-0582. I. Dose tolerance, serum and tumour concentrations.

Patients have been given single oral doses of the radiosensitiser Ro-07-0582 and serum concentrations have been attained in the range expected to be of value in radiosensitisation. Immediate gastrointestinal side effects appear to limit the dose to below 140 mg/kg but radiosensitising serum levels of approximately 200 mug/ml can be attained. Tolerance may be improved by giving anti-emetics and a more palatable preparation. Estimates of the concentration of Ro-07-0582 in tumour have ranged from 12 to 92% of the serum concentration at the time of determination.

Brain scanning in cerebral vascular disease: a reappraisal.

The frequency of abnormal brain scans in patients with cerebral vascular disease admitted to a stroke intensive care unit has been evaluated in relation to diagnosis, time after onset of symptoms, effect of delayed imaging and the degree of clinical neurological recovery. In patients with completed thromboembolic infarction, 33% had abnormal scans including 39% of those with hemispheric lesions and 14% with posterior fossa lesions. Completed hemorrhagic infarction occurred in seven patients, and three (43%) had abnormal brain scans. Of 14 patients with either transient ischemic attacks or reve

Selection for treatment in spina bifida cystica.

This paper discusses (1) the mortality in 295 patients with myelomeningocele born 1961-9, (2) the "quality of life" of 88 surviving patients born 1961-5, and (3) their implications for selection for treatment in spina bifida cystica. Evidence is presented that high neurological levels, the presence of severe hydrocephalus, especially if present at birth, meningitis and ventriculitis, and gross renal disease are very adverse factors both in mortality and in quality of life. In selection for treatment we recommend immediate repair of the sac in most low lesions, deferring treatment in high lesio

Induced hypothermia during emergency department thoracotomy: an animal model.

BACKGROUND: Induced hypothermia is used clinically to prevent ischemic injury during elective procedures. We present an animal model of asanguinous hypothermic (10 degrees C) circulatory arrest, induced through a left anterior lateral thoracotomy after exsanguinating uncontrolled hemorrhage. METHODS: Through a left anterior thoracotomy, 26 swine (45-70 kg) sustained a laceration of the descending thoracic aorta, producing exsanguinating uncontrolled hemorrhage. After 5 minutes of severe hypotension (systolic BP <20 mm Hg), a 22 French Foley catheter was directed cephalad through the enlarged

Deconstructing Poststroke Delirium in a Prospective Cohort of Patients With Intracerebral Hemorrhage.

OBJECTIVES: Poststroke delirium may be underdiagnosed due to the challenges of disentangling delirium symptoms from underlying neurologic deficits. We aimed to determine the prevalence of individual delirium features and the frequency with which they could not be assessed in patients with intracerebral hemorrhage. DESIGN: Prospective observational cohort study. SETTING: Neurocritical Care and Stroke Units at a university hospital. PATIENTS: Consecutive patients with intracerebral hemorrhage from February 2018 to May 2018. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: An attending ne

[Prognostic value of VZV PCR in cerebrospinal fluid in neurological manifestations of varicella].

Varicella-zoster virus (VZV) is, like other alphaherpes viruses, neurotropic. Of the admissions to hospital for varicella, 7.6-25% are due to neurologic manifestations. Most of the time, the concomitant skin lesions facilitate the diagnosis. The justification of qualitative PCR (polymerase chain reaction) for VZV DNA in cerebrospinal fluid in the management of these patients is not clear. The aim of this study was to evaluate the prognostic value of qualitative PCR for VZV DNA in cerebrospinal fluid and to assess its impact on patient management. We conducted a retrospective monocentric study

Spinal cord compression in pseudohypoparathyroidism.

BACKGROUND CONTEXT: Spinal cord compression associated with pseudohypoparathyroidism (PHP) is an increasingly reported sequelae of the underlying metabolic syndrome. The association of neurologic dysfunction with PHP is not well appreciated. We believe this to be secondary to a combination of underlying congenital stenosis, manifest by short pedicles secondary to premature physeal closure, and hypertrophic ossification of the vertebral bony and ligamentous complexes. PURPOSE: The purpose of this case report is to review the case of spinal stenosis in a child with PHP Type Ia. We are aware of

Prospective neurocognitive evaluation of patients undergoing carotid interventions.

OBJECTIVE: Distal cerebral embolization is a known complication of carotid interventions. We prospectively investigated whether subclinical microembolization seen on postoperative magnetic resonance imaging (MRI) leads to cognitive deficits in patients undergoing carotid revascularization procedures. METHODS: Patients undergoing carotid interventions and eligible for MRI scanning were recruited. Among 247 patients who received preoperative and postoperative MRI evaluations, 51 also completed neuropsychologic testing before and at 1 month after their procedure. Cognitive evaluation included th

Dynamics of Wakeful EEG Correlated with Serum Lithium Concentration Before and After Treatment of Lithium Carbonate Toxicity: A Case Report and Literature Review.

Lithium carbonate is a first-line psychopharmacological agent for the treatment of mood disorders, demonstrating significant therapeutic and prophylactic efficacy in the management and prevention of relapses in bipolar affective disorders characterized by manic and depressive episodes. However, chronic lithium therapy or acute overdose can lead to toxicity, with symptoms predominantly observed in individuals with a psychiatric history. Clinical manifestations of lithium toxicity are primarily neurologic and gastrointestinal. This article presents the diagnosis and treatment of a case of lithiu
