Severe corneal infections associated with contact lens wear.

An analysis of acute contact lens-related complications, with particular emphasis on severe microbial keratitis, was undertaken at the Department of Ophthalmology, Sahlgren's Hospital in Gothenburg. A total of 224 contact lens wearers presented with acute complications over a 2-year period (1981-1982). The lowest incidence of acute complications was found with hard contact lens wear. Similar numbers of daily wear and extended wear soft contact lens wearers presented with acute complications. The highest incidence of severe microbial keratitis was found in patients wearing soft contact lenses o…

Ocular manifestations of systemic diseases.

The purpose of this review is to keep the optometrist informed as to the latest developments in the field of Ophthalmology. Papers reviewed are, almost exclusively, those published between Oct. 1st, 1973 and Sept. 30th, 1974. Emphasis will be placed on signs and symptoms of the disorders which are helpful in their detection or confirmation. No rare disorders will be discussed and although the literature has been especially rich in single cases this past year, they have been, for the most part, omitted from this review. Thus, to an ophthalmologist, the selection may appear arbitrary and incompl…

Mirvetuximab Soravtansine and its TreatmentAssociated Ocular Adverse Effects. A Review of the Current Knowledge.

Mirvetuximab soravtansine is an antibody-drug conjugate targeting folate receptor alpha (FRα), which is used in the treatment of platinum-resistant ovarian cancer. Its administration is associated with a distinct spectrum of ocular adverse events, representing a clinically relevant limitation of therapy. The most common manifestations include keratopathy, blurred vision, and dry eye symptoms. These effects are generally reversible and manageable upon appropriate ophthalmologic monitoring. This review summarizes the current knowledge regarding the mechanisms, clinical presentation, incidence, p

[Neurofibromatosis Type 1 in Pediatrics: Recommendations for Diagnosis and Management].

Neurofibromatosis type 1 (NF1) is the most prevalent neurocutaneous syndrome, affecting approximately 1 in 3000 individuals. This genetic disorder involves multiple organ systems and exhibits marked variability in clinical presentation, with characteristic cutaneous, ophthalmological, neurological, cardiovascular, skeletal, and neurodevelopmental features. It may also be associated with multiple neoplasms. Timely and accurate diagnosis is essential to ensure optimal patient care. A lifelong, multidisciplinary management approach is recommended. The objective of this consensus is to summarize t

A Case of Posterior Cortical Atrophy in Alzheimer's Disease.

This is a case of a 67-year-old male diagnosed with posterior cortical atrophy. Posterior cortical atrophy is an underdiagnosed phenomenon seen in neurodegenerative diseases, such as Alzheimer's disease. When this condition manifests in patients with Alzheimer's disease, it is known as a visual variant of Alzheimer's disease. This condition leads to visual disturbances that are due to processing errors in the brain, leaving cognitive functioning unchanged. Patients tend to undergo years of unnecessary ophthalmologic workup leading to unnecessary cost and frustration to the patient. The present

Integration of onchocerciasis morbidity management and disability prevention services in the healthcare system in Tanzania: a call for action and recommendations.

Onchocerciasis is among the Neglected Tropical Diseases (NTDs) responsible for dermatological, ophthalmological, and neurological manifestations. With the ongoing burden of onchocerciasis clinical manifestations, morbidity management, and disability prevention services are required to alleviate the suffering of the affected populations. Unfortunately, despite the ongoing transmission of onchocerciasis, morbidity management, and disability prevention services are limited in Tanzania. Therefore, this article highlights the concept of onchocerciasis morbidity management and disability prevention,

NOD2 Mutation-Associated Case with Blau Syndrome Triggered by BCG Vaccination.

We describe a patient who developed multiple granulomatous skin lesions after Bacille de Calmette et Guérin (BCG) vaccination without significant effect by topical corticosteroid, followed by painless cystic tumors on the bilateral knees and hands and inflammatory changes on ophthalmologic examination. A functional mutation in NOD2 was detected by a genetic analysis, and he was diagnosed as sporadic Blau syndrome. Since NOD2 acts as a sensor for the BCG component, it is possible that BCG vaccination may trigger granuloma formation in Blau syndrome patients with such genetic background.

Ocular Oncology-Primary and Metastatic Malignancies.

Several neoplastic processes can involve the eye, either primarily or secondary to a systemic malignancy. The most common primary tumors of the eye include conjunctival and uveal melanoma, retinoblastoma, conjunctival and intraocular lymphoma, and ocular surface squamous neoplasia. Metastatic spread from systemic malignancies, especially of the breast and lung, also can involve the eye. A combination of ophthalmologic examination, ancillary testing, and cytologic/histopathologic evaluation leads to accurate diagnosis. Management consists of surgery, radiotherapy, chemotherapy, and immunotherap

Recurrent and Reversible, Bitemporal Field Defect from Presumed Chiasmitis in a Patient with Systemic Lupus Erythematosus.

Systemic lupus erythematosus (SLE) is a chronic, recurrent and remitting autoimmune disease affecting mostly women and presenting between late adolescence and the fourth decade of life. Ocular manifestations may occur in one-third of patients with SLE, with keratoconjunctivitis sicca as the most common and optic nerve and retina abnormalities as the most severe. Neuro-ophthalmological manifestations occur in approximately 1% of patients with SLE, usually in the form of optic neuropathy. The purpose of this paper is to describe a patient who developed recurrent optic chiasmitis as an uncommon s

Simultaneous squamous cell carcinoma and malignant melanoma of the conjunctiva in a teenager with xeroderma pigmentosum: Case report.

Xeroderma pigmentosum (XP) is an autosomal recessive disease with ophthalmic, dermatologic, and neurologic manifestations. Ophthalmological changes are described in up to 100% of XP patients. We report a young XP patient that presented with bilateral conjunctival masses. She was treated by surgical excision with supplemental cryotherapy. The histopathological analysis revealed squamous cell carcinoma with melanosis on right eye and conjunctival melanoma on the left eye. These patients need to be followed by dermatologists and ophthalmologists to identify malignant lesions as soon as possible a

Isolated Fourth Nerve Palsy in Nontraumatic Tentorial Hemorrhage.

A 46-year-old woman being treated with warfarin for antiphospholipid antibody syndrome experienced a rise in international normalized ratio (INR) to 5.4 and developed sudden headache and diplopia. Neuro-ophthalmologic examination disclosed a right fourth nerve palsy. Noncontrast computed tomography revealed a subdural hematoma layered along the right tentorium cerebelli, directly in the path of the nerve. With normalization of the INR, the palsy markedly improved over subsequent weeks. This is a unique case of a nontraumatic subdural hematoma causing a fourth nerve palsy.

The Repertoire of Adenovirus in Human Disease: The Innocuous to the Deadly.

Adenoviridae is a family of double-stranded DNA viruses that are a significant cause of upper respiratory tract infections in children and adults. Less commonly, the adenovirus family can cause a variety of gastrointestinal, ophthalmologic, genitourinary, and neurologic diseases. Most adenovirus infections are self-limited in the immunocompetent host and are treated with supportive measures. Fatal infections can occur in immunocompromised patients and less frequently in the healthy. Adenoviral vectors are being studied for novel biomedical applications including gene therapy and immunization.

Radiation retinopathy secondary to treatment of maxillary sinus carcinoma: a dramatic case.

CLINICAL CASE: A 53-year old male presented with visual impairment in right eye after irradiation of right maxillary sinus carcinoma. Funduscopy shows radiation retinopathy: haemorrhages, exudates, macular oedema, and peripheral retinal ischaemia. A poor outcome was achieved despite laser treatment and intravitreal injections of bevacizumab, resulting in evisceration of the affected eye. DISCUSSION: Radiation retinopathy must be considered in any loss of vision after head and neck irradiation. Ophthalmological long-term follow-up of these patients is essential for an early diagnosis.

Subacute Acrylamide Intoxication with Severe Visual Disturbance: A Case Report.

A 35-year-old man was admitted due to somnolence and progressive sensory-motor polyneuropathy, followed by severe visual impairment in both eyes after direct skin exposure to an acrylamide monomer solution. The results of an ophthalmological examination including central critical flicker fusion frequency and the decreased amplitude of action potentials observed in the visual evoked potential studies suggested that acrylamide intoxication resulted in neuronal degeneration in the optic pathways. Additional attention should be directed to the potential effect of acrylamide on the human visual sys

Late-onset Cogan's syndrome associated with large-vessel vasculitis.

Cogan's syndrome is a rare autoimmune disease that usually affects young Caucasian adults and is classically defined as the combination of nonsyphilitic interstitial keratitis and audiovestibular symptoms resembling Meniere's disease, both of them developed in an interval of less than two years. Nevertheless, cases with atypical ophthalmologic and audiovestibular features, with systemic manifestations or affecting children and older patients have also been reported, expanding the clinical spectrum of Cogan's syndrome. Herein, we present the case of a late-onset Cogan's syndrome associated with

Congenital dacryocystoceles controlled by nCPAP via nasal mask in a neonate.

Congenital dacryocystocele is a relatively rare type of nasolacrimal duct obstruction that may induce respiratory distress during the early neonatal period. We encountered a case of bilateral congenital dacryocystoceles with intranasal cysts in a premature infant delivered at 34 weeks of gestation. The patient developed symptoms of respiratory failure immediately after birth, but no ophthalmologic symptoms. Treatment with nasal continuous positive airway pressure via a nasal mask, instead of a nasal prong, effectively relieved the symptoms. Early diagnosis and appropriate treatment are critica

Ocular Manifestations of Ebola Virus Disease: An Ophthalmologist's Guide to Prevent Infection and Panic.

Ebola virus disease (EVD--formerly known as Ebola hemorrhagic fever) is a severe hemorrhagic fever caused by lipid-enveloped, nonsegmented, negative-stranded RNA viruses belonging to the genus Ebolavirus. Case fatality rates may reach up to 76% of infected individuals, making this infection a deadly health problem in the sub-Saharan population. At the moment, there are still no indications on ophthalmological clinical signs and security suggestions for healthcare professionals (doctors and nurses or cooperative persons). This paper provides a short but complete guide to reduce infection risks.

Dexmedetomidine and ketamine combination for a patient with xeroderma pigmentosa.

Patients of xeroderma pigmentosa (XP) have increased sensitivity to ultraviolet light and a defective nucleotide excision repair (NER) mechanism in their DNA. Several types of neurological, dermatological, and ophthalmological complications are common in these patients. There is increasing evidence of delayed recovery and worsening of neurological status following general anesthesia in such patients. Some reports have shown uneventful conduct of total intravenous anesthesia in patients of XP. The authors report a case of XP in a young girl for surgery, previously anesthetized with delayed reco

[Clinical associations between retinal vascular diseases and cardiovascular diseases in patients with systemic atheromatosis].

A clinical study was conducted on a sample of 48 patients examined within 3 months. Of these, 27 patients were recruited by ophtalmologic criteria and 21 recruited by cardiologic criteria, 25% of these patients coming for routine check. They were investigated by ophthalmic examination, cardiological examination, imaging and laboratory examination. Testing has shown a strong link between cardiovascular disease and the eye of the patients investigated. The study demonstrated the need for interdisciplinary consultation for patients with vascular complaints in the carotid territory and a close cor

Multiple osteochondritis dissecans of knee joint in a patient with Wilson disease, focusing on magnetic resonance findings.

A 17-year-old man was admitted with a complaint of knee pain. He was diagnosed with Wilson disease by ophthalmologic and laboratory studies during hospitalization. Initial plain radiography of both knees showed multiple osteochondritis dissecans (OCD) on the medial and lateral femoral condyles of both knees. Subsequent magnetic resonance imaging showed multiple OCDs, which were symmetric on both knees. Subchondral cysts on the medial condyle and trochlear dysplasia were additionally evident on both femurs. We report this case with a focus on the imaging findings.
