Cerebral and ocular toxicity induced by desferrioxamine.

Seven patients with rheumatoid disease were given the iron-chelating drug desferrioxamine (DFX) to evaluate its possible anti-inflammatory effects. Two of these patients, who also received the anti-emetic prochlorperazine, lost consciousness for 48-72 h and then fully recovered. Electroencephalography showed abnormalities of the type associated with metabolic disturbance. One of these patients showed pyramidal features and subsequently developed an optic neuropathy and pigmentary retinopathy. Analysis of his cerebrospinal fluid showed a decrease in loosely-bound (catalytic) iron and increase i

[Right-left differences in nystagmus direction. Observations in congenital and acquired ophthalmoneurologic syndromes].

We report about 8 patients all showing a nystagmus rotatorius or giratoire and differences of the horizontal component. In syndromes of the nervous system monocular nystagmus was combined with a scew-deviation or tremor of the lids. Similar to the syndromes of the fasciculus longitudinalis posterior defects in smooth or quick movements on both sides were different. After operation of a congenital cataract a vertical nystagmus was transformed in a nystagmus latens. In congenital squint syndromes monocular vertical nystagmus may be seen if one eye or both are closed. Separate pathways from the v

Serological Evidence of Soil-Transmitted Helminth Infections as a Potential Risk for Severity in Leprosy Patients.

Leprosy presents a broad clinical spectrum influenced by the host's immune response, and co-infections may further modulate disease progression. This study evaluated clinically diagnosed leprosy patients (n = 251) from Sergipe and Minas Gerais, Brazil, along with healthy controls (n = 43), soil-transmitted helminths-positive controls (n = 15), and household contacts (n = 176). Enzyme-linked immunosorbent assays were performed using predicted B-cell epitopes from immunogenic proteins of Ascaris sp., Trichuris trichiura, Strongyloides stercoralis, Ancylostoma duodenale and Necator americanus. Oligodendrocyte death, neuroinflammation, and the effects of minocycline in a rodent model of nonarteritic anterior ischemic optic neuropathy (rNAION).

PURPOSE: Optic nerve (ON) damage following nonarteritic anterior ischemic optic neuropathy (NAION) and its models is associated with neurodegenerative inflammation. Minocycline is a tetracycline derivative antibiotic believed to exert a neuroprotective effect by selective alteration and activation of the neuroinflammatory response. We evaluated minocycline's post-induction ability to modify early and late post-ischemic inflammatory responses and its retinal ganglion cell (RGC)-neuroprotective ability. METHODS: We used the rodent NAION (rNAION) model in male Sprague-Dawley rats. Animals receiv

Loss of Ikbkap Causes Slow, Progressive Retinal Degeneration in a Mouse Model of Familial Dysautonomia.

Familial dysautonomia (FD) is an autosomal recessive congenital neuropathy that is caused by a mutation in the gene for inhibitor of kappa B kinase complex-associated protein (IKBKAP). Although FD patients suffer from multiple neuropathies, a major debilitation that affects their quality of life is progressive blindness. To determine the requirement for Ikbkap in the developing and adult retina, we generated Ikbkap conditional knockout (CKO) mice using a TUBA1a promoter-Cre (Tα1-Cre). In the retina, Tα1-Cre expression is detected predominantly in retinal ganglion cells (RGCs). At 6 months, sig

Acetazolamide therapy in a case of fingolimod-associated macular edema: early benefits and long-term limitations.

UNLABELLED: Fingolimod is a potent drug in relapsing forms of multiple sclerosis. Visual impairment due to fingolimod-associated macular edema (FAME) usually leads to discontinuation of fingolimod therapy. METHODS: We report on a 24-year old woman with bilateral FAME. RESULTS: We continued fingolimod and added oral acetazolamide, which led to recovery of visual acuity and regression of macular edema. However, fingolimod had to be discontinued when fluorescein angiography revealed an enlarged foveal avascular zone. DISCUSSION AND CONCLUSION: Oral acetazolamide might be a treatment option for

Retinal nerve fiber layer analysis using GDx in 49 patients with chronic phase DUSN.

PURPOSE: To describe retinal nerve fiber layer changes in late-stage diffuse unilateral subacute neuroretinitis eyes and compare these results with healthy eyes observed through nerve fiber analyzer (GDx). METHODS: This is a retrospective case-control study in which 49 eyes in late-stage diffuse unilateral subacute neuroretinitis were examined from May/97 to December/01. First, eyes with diffuse unilateral subacute neuroretinitis and healthy contralateral eyes (Control Group I) were statistically matched. Subsequently, eyes with diffuse unilateral subacute neuroretinitis were compared with ey

An ocular cat-scratch disease patient positive for cytoplasmic anti-neutrophil cytoplasmic antibody.

BACKGROUND: We report a case of ocular cat-scratch disease with permanent vision reduction in a patient who was cytoplasmic anti-neutrophil cytoplasmic antibody (C-ANCA) positive. METHODS: Case report and review of the literature. RESULTS: While taking steroids and antibiotics, a 52-year-old man with uveitis associated with cat-scratch disease developed retinal vein occlusion and a macular exudate. His final visual acuity was poor because of residual macular degeneration and optic atrophy. Serum C-ANCA increased and decreased in parallel with ocular inflammatory activity. CONCLUSION: C-ANCA

Optic-spinal form of multiple sclerosis and anti-thyroid autoantibodies.

The optic-spinal form of multiple sclerosis (OSMS), characterized by recurrent involvement of optic nerve and spinal cord with rare brain magnetic resonance imaging lesions, is relatively common among Asians. While individual cases of OSMS with anti-thyroid autoantibodies (ATABs) have been reported, the frequency of ATAbs in OSMS and classical multiple sclerosis has not been studied. We studied serum ATAbs and anti-nuclear antibodies (ANA) in 46 Japanese patients with multiple sclerosis: 14 with OSMS, and 32 with non-OSMS. Six patients were positive for ATAbs: five women with OSMS and one man

[The factors affecting the growth of the agent, producing a mild cytopathic effect, isolated in patients with epidemic neuropathy].

Certain factors influencing upon the growth of the agent producer of mild cytopathogenic effect and isolated from the cerebrospinal fluid of patients presenting with epidemic neuropathy are described in this paper. It was demonstrated that a concentration of NaHCO3 was essential for the occurrence of the cytopathogenic effect. Determined concentrations of MnCl2 allowed for the visualization of the cytopathogenic effect and increased viral yields.

[The immunological factors characterizing neurochorioretinitis].

Comprehensive clinical and immunologic studies carried out in 38 patients with neurochorioretinitis have demonstrated the contribution of infection to the etiology and pathogenesis of this condition and helped identify the nature, mostly infectious, of the condition in 55.3% of patients. Toxoplasma infection, revealed in 39.5% of cases, is one of the major etiologic factors of neurochorioretinitis. Tuberculous etiology of the condition is more rare (7.9% of cases). The autoimmune systemic component was detected in 7.9% of the examinees. Immunopathologic shifts, presenting as hyperimmunoglobuli

Evoked potentials and CSF-immunoglobulins in MS: relationship to disease duration, disability, and functional status.

In 100 MS patients, BAEP and tibial SEP abnormality rates increased significantly with disease duration and clinical disability. VEP correlated non-linearly with disease duration, and median nerve SEP correlated with disability. In multifactorial analysis, however, BAEP correlated significantly only with clinical brainstem and cerebellar signs. These results suggest that evoked potentials correlate more strongly with neurological status of the functional subsystems than either overall disability or disease duration. These findings indirectly suggest that evoked potentials may be useful monitor
