Insomnia and the effect of zolpidem-extended-release on the sleep items of the Hamilton Rating Scale for Depression in outpatients with depression, insomnia, and suicidal ideation: Relationship to patient age.

BACKGROUND: There are limited data regarding gamma-aminobutyric acid (GABA) allosteric modulator sleep-aid medications in persons with depression, insomnia, and suicidal ideation (SI). AIMS: This secondary analysis examined the relationship of age to insomnia and the impact of age on the treatment of insomnia with zolpidem extended-release (zolpidem-ER) in depressed suicidal patients. A prior report found that the addition of zolpidem-ER promoted significantly superior reductions in global severity of insomnia in depressed outpatients with insomnia and SI over 8 weeks, but here we report the …

Efficacy of a combined exercise and nutrition intervention study for outpatients with possible sarcopenia in community-based primary care clinics (ENdSarC): study protocol for a multicenter single-blinded randomized controlled trial.

BACKGROUND: Sarcopenia is a geriatric disease characterized by loss of muscle mass and strength. Although combined exercise and nutrition intervention are known to be effective for sarcopenia, clinical trials involving outpatients with sarcopenia in primary care are scarce. We describe a protocol for a trial to examine the effects of a 12-week combined exercise and nutrition intervention in Korean older adults with possible sarcopenia in community-based primary care. METHODS: This multicenter, randomized, controlled trial will include 94 community-dwelling older outpatients aged 65-85 years w…

Clinical utility of WHO-recommended screening tools and development and validation of novel clinical prediction models for pulmonary tuberculosis screening among outpatients living with HIV: an individual participant data meta-analysis.

BACKGROUND: The World Health Organization (WHO) recommends that outpatient people living with HIV (PLHIV) undergo tuberculosis screening with the WHO four-symptom screen (W4SS) or C-reactive protein (CRP) (5 mg·L-1 cut-off) followed by confirmatory testing if screen positive. We conducted an individual participant data meta-analysis to determine the performance of WHO-recommended screening tools and two newly developed clinical prediction models (CPMs). METHODS: Following a systematic review, we identified studies that recruited adult outpatient PLHIV irrespective of tuberculosis signs and sy

Markers of disease severity and positive family history are associated to significant risk perception in rheumatoid arthritis, while compliance with therapy is not: a cross-sectional study in 415 Mexican outpatients.

BACKGROUND: Assessing risk perception (RP) helps explain how rheumatoid arthritis (RA) patients integrate their ideas concerning the disease and how this understanding affects their self-care management. Compliance with treatment impacts disease-related outcomes and could be associated with RP to variable degrees and at different levels. The primary objective was to determine a potential association between RP and compliance with therapy in RA outpatients and to identify additional factors. The secondary objective was to identify factors associated with judgment bias such as unrealistic RP. P…

Sonolucent cranial windows for bedside ultrasound identification of MRI-concordant glioblastoma recurrence during outpatient surveillance: an imaging correlation pilot study.

BACKGROUND AND OBJECTIVE: Glioblastoma (GBM) is characterized by near-universal recurrence after surgical resection, necessitating close postoperative surveillance. Although MRI remains the gold standard for detecting tumor progression, fixed imaging intervals may delay recognition of recurrence. Sonolucent cranial implants enable transcranioplasty ultrasound (TCUS), offering a potential point-of-care adjunct for interval monitoring between scheduled MRI studies. We evaluated the feasibility of identifying sonographic findings concordant with MRI-confirmed GBM recurrence during routine outpati

Blood pressure changes after stair climbing in busy and quiet outpatient clinic environments among Tanzanian adults: a randomised crossover study.

INTRODUCTION: Blood pressure (BP) measured soon after activity may differ from later seated readings. We examined serial BP changes after stair climbing in busy and quiet outpatient settings, without assuming that a particular waiting period represents complete physiological recovery. METHODS: In this randomized crossover study at Mbeya Zonal Referral Hospital, Tanzania, adults with and without a previous hypertension diagnosis drew folded numbered slips to determine environmental order. Staff recording BP were not blinded. Participants ascended and descended 19 steps before measurements imme

Case Report: Outpatient management of rectal infiltration by iodinated polyethylene glycol hydrogel and stabilized hyaluronic acid spacers in patients with prostate cancer.

Prostate cancer is one of the most commonly diagnosed cancers in men worldwide. Radiation therapy is a widely used treatment for prostate cancer, but it carries the risk of adverse effects on adjacent organs such as the rectum. Therefore, to provide additional protection for the rectum during prostate radiation, perirectal spacers are inserted into Denonvilliers' space, the thin fascial layer located behind the prostate gland and in front of the anterior rectal wall. Polyethylene glycol (PEG)-based hydrogels and recently FDA-approved stabilized hyaluronic acid (sHA) spacers have been designed

Therapist perspectives on the clinical utility of hand performance information from at-home egocentric video in outpatient neurorehabilitation: a multi-methods evaluation study.

BACKGROUND: Restoring hand function is a primary focus of rehabilitation after neurological injuries, such as stroke and spinal cord injury. However, monitoring hand use outside the clinic remains challenging. This study aims to evaluate how therapists perceive and would utilise information from a clinical decision support system (CDSS) that uses egocentric video to monitor patients' hand use at home. METHODS: Five patient-therapist dyads were recruited. Patients recorded daily activities using head-mounted cameras. Therapists reviewed dashboards of processed video data from their patients an
