Extensor mechanism disruption after total knee arthroplasty.

Extensor mechanism disruption is a rare and potentially devastating complication associated with total knee arthroplasty. Disruption can occur at the quadriceps or patellar tendons or, in the setting of a fracture, at the patella. Recognition of the risk factors for disruption and prevention via meticulous surgical technique are critical to avoid this complication. Various management techniques and the challenges associated with treatment have been described. Nonsurgical management consists of the use of walking aids and/or knee braces, which may not be acceptable for the active patient. Surgi

Fractures of the Talus: State of the Art.

UNLABELLED: Talus fractures occur rarely but are often associated with complications and functional limitations. Urgent reduction of associated dislocations is recommended with open reduction and internal fixation of displaced fractures when adjacent soft tissue injury permits. Delayed definitive fixation may reduce the risks of wound complications and infections. Restoration of articular and axial alignment is necessary to optimize ankle and hindfoot function. Despite this, posttraumatic arthrosis occurs frequently after talar neck and body fractures, especially with comminution of the talar

Chronic Scapholunate Ligament Injuries: Treatment with Supplemental Fixation.

Treatment of chronic scapholunate ligament injuries can be challenging. Traditional reconstructive techniques, including varied capsulodeses and tenodeses often yield inconsistent results with loss of reduction and radiographic deterioration. As a result, supplemental hardware fixation has become more popular and may allow more robust stabilization of the scapholunate reconstruction. However, these procedures have complications and few data regarding outcomes are currently available. This article evaluates the role of supplemental fixation in the management of chronic scapholunate instability.

Tragal reconstruction after tumor excision.

The tragus forms a distinct and important landmark in the overall artistic and anatomical landscape of the ear. Despite its small size, it functions to cover and prevent direct access to the ear canal. Aesthetically, it has an important role in defining the morphology of the auricle in 3 dimensions, not only covering the meatus but also casting a shadow into the conchal bowl that gives the impression of depth. Most of the techniques for tragal reconstruction are associated with creation of a tragus in patients with microtia. We present here a new technique for tragal reconstruction in the sett

Autologous reconstruction of a complex form of Poland syndrome using 2 abdominal perforator free flaps.

Poland syndrome is the most frequent cause of congenital breast aplasia and hypoplasia. Breast and possible chest wall deformities can be treated with several surgical techniques, including implants, and pedicled or free flaps. We describe the case of a young patient with severe Poland syndrome with amastia, athelia, and deformity of the chest wall, and aplasia of 2 ribs. Marked hypoplasia of the ipsilateral latissimus dorsi muscle ruled out a reliable reconstructive option. Two perforator flaps were performed in a single-stage operation. A hemi-deep inferior epigastric perforator flap was harve

[Prosthetic reconstruction in high amputations of the upper extremity].

BACKGROUND: Conventional upper arm prostheses are controlled via two surface electrodes that measure motor activity of two separately innervated muscle groups. The various prosthetic joints are chosen by co-contractions and controlled linearly by these two muscles. A harmonious and natural course of movements is not possible in this way. OBJECTIVES: Overview regarding surgical, therapeutical and prosthetic options in high amputations of the upper extremity. METHODS: Selective literature research including the authors' own experience in everyday clinical practice as well as a review of medica

Chronic Scapholunate Ligament Injury: Techniques in Repair and Reconstruction.

Although the true incidence of scapholunate interosseous ligament (SLIL) injury is unknown, a study found that 35% of cadaveric wrists had some degree of scapholunate tear. Of those wrists with SLIL injury, 29% had evidence of arthrosis. Early recognition and treatment of these injuries can delay or prevent the onset of arthritis. This article details treatment options for SLIL injury across the spectrum of pathology with a particular emphasis on chronic scapholunate repair and reconstruction. New techniques and outcomes data also are presented.

The philtrum in cleft lip: review of anatomy and techniques for construction.

The philtrum is imperfectly formed in both unilateral and bilateral cleft lip. Surgical construction of this exquisite feature in the upper lip is challenging and often takes a back seat to construction of Cupid's bow and the nose. However, the philtrum is one of the defining features of a normal lip. The purpose of this review is to compile information on philtral anatomy and techniques for construction to guide the reader on his or her own quest to optimize cleft lip repair. In this article, we review the normal anatomy of the philtral dimple and ridges, with particular attention to pars per

[Reconstructive method after resection of chest wall, diaphragm and pericardium, mainly using Composix-mesh].

This study examined utility and question about the reconstructive prostheses in 11 cases which were resected chest wall and/or diaphragm and/or pericardium. One of 3 patients, who were used metal plates after chest wall resection, was prevented from post-operative irradiation therapy by limitation and diffuse reflection of beam by the metal plates. Gore-Tex( ePTFE) sheets, which were used for coverage of diaphragm defect and pericardium defect, occurred no problems. Composix-meshes (Composix-Kugel, Hernia-patch) were used for 7 cases which had large defects due to resection of chest wall and d

[Congenital pseudarthrosis of lower leg treated by almost outdated method--case report].

INTRODUCTION: Congenital pseudarthrosis of tibia is a rare congenital deformity with progressive evolution. Treatment is vague and difficult, and many methods have been used--from once mandatory early amputation to contemporary operative (Ilizarov method, free microvascular fibular graft) and adjuvant methods (electrostimulation, biphosphonates, bone morphogenetic protein). We present the usage of once popular method of homologous graft insertion and intramedullary fixation. CASE OUTLINE: This is a case report of male patient with pseudarthrosis involving both crural bones (Boyd type 5), diag

Unexpected angular or rotational deformity after corrective osteotomy.

BACKGROUND: Codman's paradox reveals a misunderstanding of geometry in orthopedic practice. Physicians often encounter situations that cannot be understood intuitively during orthopedic interventions such as corrective osteotomy. Occasionally, unexpected angular or rotational deformity occurs during surgery. This study aimed to draw the attention of orthopedic surgeons toward the concepts of orientation and rotation and demonstrate the potential for unexpected deformity after orthopedic interventions. This study focused on three situations: shoulder arthrodesis, femoral varization derotational
