Copy number variation of LINGO1 in familial dystonic tremor.

OBJECTIVE: To elucidate the genetic cause of a large 5 generation South Indian family with multiple individuals with predominantly an upper limb postural tremor and posturing in keeping with another form of tremor, namely, dystonic tremor. METHODS: Whole-genome single nucleotide polymorphism (SNP) microarray analysis was undertaken to look for copy number variants in the affected individuals. RESULTS: Whole-genome SNP microarray studies identified a tandem duplicated genomic segment of chromosome 15q24 present in all affected family members. Whole-genome sequencing demonstrated that it compr

Slip and Trip Perturbations During an Object Transport Task Requiring a Lateral Change in Support.

The ability to counteract destabilizing external forces while simultaneously executing a complex task presents a novel way to ascertain one's ability to generate adaptive postural control responses to avoid a potential fall. In this study, participants performed an upper limb object transport task requiring a lateral change in support on a robotic platform that could remain fixed in space or translated (mimicking a slip or trip perturbation). No significant stability differences were observed at initial recovery step between slip and trip perturbations. Variability measures were greatest durin

Nintendo Wii Fit-Based Sleepiness Testing is Not Impaired by Contagious Sleepiness.

Sleep deprivation may cause accidents, and it has deteriorating effects on health. A measurement of postural steadiness by a portable and affordable Nintendo Wii Fit balance board can be used to quantify a person's alertness. At work, people are under the influence of their environment-often other people-that may affect their alertness. This work investigates whether sleep deprivation among people is "contagious," as quantified by sway measures. We measured 21 volunteers' postural steadiness while alert and sleep deprived. During the measurements, a screen placed in front of the participants s

Effects of two ergonomic improvements in brazing coils of air-handler units.

The research aims to address the physically loading task and quality and productivity problems in the brazing of coils of air-handler units. Eight operators participated in two intervention studies conducted in a factory in Malaysia to compare the status quo brazing with (1) the use of a new twin-brazing torch that replaced the single-brazing gun and (2) brazing in a sitting position. The outcome measures are related to quality, productivity, monetary costs, body postures and symptoms. After baseline, Interventions I and II were applied for 3 months respectively. The results show a 58.9% quali

Effect of seat and table top slope on the biomechanical stress sustained by the musculo-skeletal system.

The purpose of this study was to assess the effect of table and seat slope on the biomechanical stress sustained by the musculo-skeletal system. Angular position of the head and trunk, and surface electromyography of eleven postural muscles were recorded while seated under different conditions of seat slope (0°, 15° forward) and table slope (0°, 20° backward). The specific stress sustained by C7-T1 joint was estimated with isometric torque calculation. The results showed that the backward sloping table was associated with a reduction of neck flexion and neck extensors EMG, contrasting with a c

Musicians' medicine: musculoskeletal problems in string players.

There is increasing attention to medical problems of musicians. Many studies find a high prevalence of work-related musculoskeletal disorders in musicians, ranging from 73.4% to 87.7%, and string players have the highest prevalence of musculoskeletal problems. This paper examines the various positions and movements of the upper extremities in string players: 1) basic postures for holding instruments, 2) movements of left upper extremity: fingering, forearm posture, high position and vibrato, 3) movements of right upper extremity: bowing, bow angles, pizzicato and other bowing techniques. These

Four and a half LIM protein 1 gene mutations cause four distinct human myopathies: a comprehensive review of the clinical, histological and pathological features.

Mutations in the four and a half LIM protein 1 (FHL1) gene were recently identified as the cause of four distinct skeletal muscle diseases. Since the initial report outlining the first fhl1 mutation in 2008, over 25 different mutations have been identified in patients with reducing body myopathy, X-linked myopathy characterized by postural muscle atrophy, scapuloperoneal myopathy and Emery-Dreifuss muscular dystrophy. Reducing body myopathy was first described four decades ago, its underlying genetic cause was unknown until the discovery of fhl1 mutations. X-linked myopathy characterized by po

Passive and active mechanisms of correction of thoracic idiopathic scoliosis with a rigid brace.

Contemporary bracing developed numerous novel technical solutions to most of main aspects of the correction of structural progressive idiopathic scoliosis. This paper presents a short review on principal biomechanical rules for the three dimensional scoliosis correction. Apart from the tissue transfer, which is a known passive mechanism of rigid bracing, the other passive mechanisms are described, containing the "cherry stone" distraction effect, the thoracic derotation and the bending. A demanding technical construction of the orthosis enables active mechanisms to develop: the corrective fact

Hip spine relationships: application to total hip arthroplasty.

Lumbo-sacral orientation in the sagittal plane is of utmost importance, as it plays a critical role in the function of the spine and the hip joints. Equilibrium of the trunk influences the tridimensional orientation of the acetabulum and the functional range of motion of the hips. Each patient is characterized by a "morphological" parameter named incidence angle; its sagittal balance is the consequence of a postural adaptation for other functional parameters (pelvic tilt, sacral slope, lumbar lordosis, acetabular sagittal tilt, functional anteversion). Understanding variations of the sacral sl

[Benign paroxysmal torticollis in infancy].

BACKGROUND: Benign paroxysmal torticollis (BPT) is an episodic functional disorder of unknown etiology, characterized by the periods of torticollic posturing of the head, that occurs in the early months of life in healthy children. CASE REPORT: We reported two patients with BPT. In the first patient the symptoms were observed at the age of day 20, and disappeared at the age of 3 years. There were 10 episodes, of which 2 were followed by vomiting, pallor, irritability and the abnormal trunk posture. In the second patient, a 12-month-old girl, BPT started from day 15. She had 4 episodes followe

[Automobile driving fitness of patients with nystagmus].

BACKGROUND: By law, applicants for driver's licenses not only have to have sufficient vision, but also have to use this sight reliably. The nystagmus patient takes advantage of ocularly induced head turns and other forms of compensation. In the approved test procedures he is deprived of such compensations. MATERIALS AND METHODS: The eye movements of a nystagmus patient were recorded oculographically. He was allowed to use compensatory mechanisms of body posture and movement for stabilization of vision. RESULTS: The nystagmus patient can achieve better visual performance when he is allowed to

Octreotide reduces alcohol-induced hypotension and orthostatic symptoms in primary autonomic failure.

Symptomatic postural hypotension is a major problem in patients with primary idiopathic autonomic failure, and ingestion of small quantities of alcohol may worsen the degree of postural hypotension. The proposed mechanisms include mesenteric vasodilation and release of vasodilatory gut peptides. We measured systemic, mesenteric, other regional vascular and biochemical responses to alcohol ingestion before and after pre-treatment with the somatostatin analogue Octreotide (which inhibits the release of a wide range of gut peptides normally released in response to food ingestion) in six patients

Neurological and cytogenetic study in early-onset ataxia-telangiectasia patients.

The clinical diagnosis of ataxia-telangiectasia (AT) is difficult before the age of 4 years. We report clinical and cytogenetic data on three early-onset, early-diagnosed AT patients at the age of 12, 18 and 22 months, respectively. Postural instability of the trunk, characterized by motor impersistence, was the earliest neurological sign detected as early as 1 year of life. Dystonic movements and postures of arms and trunk and a subtle disorder of eye movement (blinking before gaze changing, increased latency and dysmetry of saccades) were observed during the 2nd year of life. All patients ex

Effects of tilting in the anaesthetized and mechanically ventilated rat: a model of orthostatic hypotension.

The main of the present study was to determine the experimental conditions for an animal model of orthostatic hypotension based on tilting in rats. Blood pressure, heart rate and plasma catecholamine levels were measured before and after tilting in conscious and anaesthetized rats either breathing spontaneously or mechanically ventilated. In conscious rats, tilting did not alter arterial blood pressure, but increased heart rate and plasma catecholamine levels. In anaesthetized rats, tilting induced a drop in arterial blood pressure without any modification in plasma catecholamine concentration

Cerebral autoregulation in patients with orthostatic hypotension.

The present study was conducted to determine the relationship between autonomic nervous function and cerebral autoregulation in 17 patients with vertigo or dizziness. Autonomic nervous function was examined by measuring systolic blood pressure response and dynamic electrocardiogram R-peak to R-peak response upon standing up. Regional cerebral autoregulation was examined by comparing cerebral blood flow in both supine and standing positions with single photon emission computed tomography. Moreover, the relationship of each of these three measures to postural provocation of symptoms was studied.
