Foot deformities associated with congenital myopathy are rarely treated surgically; therefore, few reports are available. We present cases of three patients who underwent surgery for foot deformities caused by congenital myopathy. Posteromedial release without talocalcaneal joint release is our surgical procedure. Surgical procedures in each case were determined on the basis of radiographic findings. Surgery resulted in plantigrade feet in the three patients, all of whom were able to walk without pain more than 3 years postoperatively.

In the past two decades our knowledge of the pathophysiology and surgical treatment options in chronic pancreatitis have improved substantially. Surgical treatment in chronic pancreatitis has evolved from radical to organ-preserving procedures. Scaphoid nonunion with avascular necrosis of the proximal pole remains a difficult problem. We have endeavoured to heal the fracture, restore scaphoid height and revascularize the proximal pole of the scaphoid by means of a vascularized dorsal interposition graft from the distal radius. The procedure has resulted in union of six of ten fractures. Fractures that healed had not been treated by a previous bone grafting procedure. Dissatisfaction was due to loss of motion in patients who had healed fractures, and pain in those patients with persistent non-unions.

The case of a 93-year-old man who received a Heimlich maneuver while choking is reported. After the procedure, the patient presented with abdominal pain and ultimately was found to have developed a gastric rupture. He was hospitalized for 66 days. Review of the literature showed that only four gastric perforations related to the Heimlich maneuver have been documented. Other complications have occurred. It is reasonable to perform the procedure as an alternative to asphyxiation, but emergency physicians must be aware of the fact that life-threatening complications may ensue.

Between 1972 and 1986, 246 patients, mainly alcoholic, with a mean age of 44 years, underwent a total of 270 laparotomies. Parenchymal resections were carried out in 99 patients and drainage and diversion procedures were carried out in 159. Pain was the principal symptom and led to surgery in 60% of cases; biliary or duodenal obstruction was present in 40 and 23.5% of cases respectively. Operative mortality was 9% for resections and 5.8% for other forms of surgery. In the long term, the best results were obtained in patients in whom a Whipple procedure was performed.

Thirteen out of 17 patients with a hip prosthesis according to the Girdlestone procedure were studied at a follow-up time of 26 months to 10 years. Ultrasonography was performed as the first imaging procedure in 100 patients who presented with acute right upper quadrant pain suggestive of cholecystitis or cholelithiasis. IMPORTANCE: Acute aortic syndrome (AAS), a potentially fatal pathologic process within the aortic wall, should be suspected in patients presenting with severe thoracic pain and hypertension.
