The meaning of quality of life and the effects of unrelated donor bone marrow transplants for chronic myeloid leukemia in adult long-term survivors.

Qualitative data, as part of a larger study, were collected to assess perceptions about quality of life (QOL) after bone marrow transplant (BMT) and its impact on long-term survivors who had undergone an unrelated donor bone marrow transplant. A sample of 28 survivors participated in the study. Quality of life was described in terms of "normality," enjoyment, and fulfillment with life: being psychologically and physically healthy, being independent, having family and relationships, having work, experiencing happiness, and having material support. There were many factors with which survivors fo

The clinical effectiveness and cost-effectiveness of low-intensity psychological interventions for the secondary prevention of relapse after depression: a systematic review.

BACKGROUND: Depression is the most common mental disorder in community settings and a major cause of disability across the world. The objective of treatment is to achieve remission or at least adequate control of depressive symptoms; however, even after successful treatment, the risk of relapse after remission is significant. Although the effectiveness of low-intensity interventions has been extensively evaluated to treat primary symptoms of psychological difficulties, there has been substantially less research examining the use of these interventions as a relapse prevention strategy. OBJECTI

Department of Defense Trauma Registry Infectious Disease Module Impact on Clinical Practice.

BACKGROUND: The Joint Trauma System (JTS) is a DoD Center of Excellence for Military Health System trauma care delivery and the DoD's reference body for trauma care in accordance with National Defense Authorization Act for Fiscal Year 2017. Through the JTS, evidence-based clinical practice guidelines (CPGs) have been developed and subsequently refined to standardize and improve combat casualty care. Data are amassed through a single, centralized DoD Trauma Registry to support process improvement measures with specialty modules established as the registry evolved. Herein, we review the implemen

Diagnosis and Management of Infectious Encephalitis in Adults: Guidelines From the European Academy of Neurology and European Society of Clinical Microbiology and Infectious Diseases.

BACKGROUND: Encephalitis is difficult to diagnose and manage, posing a challenge to clinicians. Since the last European guidelines were published in 2010, novel causes have emerged, diagnostic processes have evolved and treatments assessed which need to be reflected in updated guidelines. OBJECTIVE: We aimed to produce recommendations for the diagnosis and management of infectious encephalitis in adults based on current evidence to equip clinicians to improve patient outcomes. METHODS: These guidelines were developed jointly with the support of two European specialist medical organisations.

Assessing the Value of Digital Health Interventions: A Systematic Review of Evaluation Frameworks and Approaches, Key Dimensions and Criteria.

OBJECTIVES: Digital Health Interventions (DHIs) are systematically developed digital solutions that address key healthcare challenges such as accessibility, efficiency, and quality of care. These interventions optimize healthcare processes and benefit multiple stakeholders. Despite their increasing significance, the evaluation of DHIs remains complex due to their multidimensional impacts and value contribution. This review aims to identify and synthesize value assessment frameworks (VAFs) and further evaluation approaches for DHIs, focusing on value dimensions, value criteria, and methodologic

Large language models for accurate disease detection in electronic health records: the examples of crystal arthropathies.

OBJECTIVES: We propose and test a framework to detect disease diagnosis using a recent large language model (LLM), Meta's Llama-3-8B, on French-language electronic health record (EHR) documents. Specifically, it focuses on detecting gout ('goutte' in French), a ubiquitous French term that has multiple meanings beyond the disease. The study compares the performance of the LLM-based framework with traditional natural language processing techniques and tests its dependence on the parameter used. METHODS: The framework was developed using a training and testing set of 700 paragraphs assessing 'go

PAX-D: study protocol for a randomised placebo-controlled trial evaluating the efficacy and mechanism of pramipexole as add-on treatment for people with treatment resistant depression.

INTRODUCTION: Clinical depression is usually treated in primary care with psychological therapies and antidepressant medication. However, when patients do not respond to at least two or more antidepressants within a depressive episode, they are considered to have treatment resistant depression (TRD). Previous small randomised controlled trials suggested that pramipexole, a dopamine D2/3 receptor agonist, may be effective for treating patients with unipolar and bipolar depression as it is known to influence motivational drive and reward processing. PAX-D will compare the effects of pramipexole

Genetically modified foods and food allergy.

Genetic modification of foods is one of the many ways of processing that can enhance foods to increase desirable qualities, such as herbicide tolerance, bacteria and insect resistance, improved nutritional value, and delayed ripening. However, a theoretical potential to increase the allergenicity of food proteins has been the subject of concern from critics. To prevent adverse effects from genetically modified (GM) crops, national and international organizations tightly regulate their production and recommend rigorous safety testing. Some safety tests were developed to assess potential allerge

Heterogeneity in the psychological antecedents of influenza vaccination among Chinese older adults: A 5C model approach.

Seasonal influenza remains a significant public health concern among older adults, who face an increased risk of hospitalization and mortality. Despite the availability of free vaccination programs in many regions of China, influenza vaccination coverage among older adults remains low. Psychological antecedents play a crucial role in shaping vaccination decisions, yet few studies have explored how these factors interact with sociodemographic characteristics. A cross-sectional survey was conducted among 13,363 community-dwelling adults aged ≥60 y in six major Chinese cities between December 202

Laboratory-Negative Results as a Marker of Diagnostic Gap: A Retrospective Study in the One Health Context.

BACKGROUND: Antimicrobial resistance (AMR) is a major global health concern, driven in part by inappropriate antimicrobial use and diagnostic uncertainty. Laboratory-based diagnostics play a critical role in guiding rational therapy; however, a high proportion of laboratory-negative results may reflect potential gaps in diagnostic processes. OBJECTIVE: The objective of this study is to assess the proportion of laboratory-confirmed infections among patients undergoing microbiological investigations and to explore a potential diagnostic gap using laboratory-negative results as an indirect and e

Normalization of Gastrointestinal Symptoms in Adults With Constipation With Daily Green Kiwifruit Consumption: Protocol for an Open-Label Intervention Study.

BACKGROUND: Irritable bowel syndrome with constipation (IBS-C) and functional constipation (FC) have significant personal, health care, and social impacts, affecting patients' quality of life. Treatment of these conditions is challenging. While green kiwifruit is a promising natural alternative to laxatives, its effectiveness in managing abdominal pain and the underlying mechanism of action is yet to be substantiated. OBJECTIVE: This study investigates the effect of consuming 2 green kiwifruit daily for 4 weeks (the habitual serving) on abdominal pain and discomfort in individuals with IBS-C

Egg donation beyond the standard model: fertility practitioners' clinical reasoning in single, combined and shared treatment options in Sweden.

The use of donor eggs, sperm and embryos in medically assisted reproduction (MAR) provide new possibilities for reproductive assistance and family-making. In clinical practice, it also brings to light questions of responsibility and ethical conduct. Despite this, fertility practitioners' reasoning in clinical decision-making remains surprisingly understudied. Drawing on Annemarie Mol's work on ontologies in medical practice (2002), we examine how fertility practitioners' clinical reasoning varies across different types of reproductive assistance involving donor eggs. Following Sweden's legal c

Validation and comparison of EuroQol and short form 6D in chronic prostatitis patients.

OBJECTIVE: Generic, preference-based health-related quality of life (HRQoL) instruments is increasingly used in health-care decision-making process. However, to our knowledge, no such HRQoL instrument has been validated or used in chronic prostatitis. We therefore aimed to assess and compare the psychometric properties of EuroQol (EQ-5D) and Short Form 6D (SF-6D) among chronic prostatitis patients in China. METHODS: Consenting patients were interviewed using EQ-5D and SF-6D. Convergent and discriminative construct validities were examined with five and two a priori hypotheses, respectively. S

Ethics of Telerehabilitation in Supportive Cancer Care Through a Responsible Innovation Lens.

In recent years, remote exercise services ("telerehabilitation") have been increasingly explored and implemented, including in supportive cancer care, on the promise of greater convenience, access independent of geographic location, reduced infection risk, and cost savings. Ethical commentary on telehealth in general is substantial, but sustained discussion of questions specific to telerehabilitation remains comparatively sparse. As contributors to PREFERBALE-II, a European Union-funded project evaluating live-remote exercise for people recovering from cancer treatment, we sought practical gui

A systematic multidisciplinary initiative for reducing the risk of complications in adult scoliosis surgery.

OBJECTIVE Systematic multidisciplinary approaches to improving quality and safety in complex surgical care have shown promise. Complication rates from complex spine surgery range from 10% to 90% for all surgeries, and the overall mortality rate is 1%-4%. These rates suggest the need for improved perioperative complex spine surgery processes designed to minimize risk and improve quality. METHODS The Group Health Research Institute and Virginia Mason Medical Center implemented a systematic multidisciplinary protocol, the Seattle Spine Team Protocol, in 2010. This protocol involves the following

A type II hybrid implementation-effectiveness study of the BECOME intervention: integrating Behavioral Community-Based Approaches for Mental Health and Non-Communicable Diseases delivered by community health workers-study protocol for a stepped wedge cluster randomized controlled trial.

BACKGROUND: Common mental health conditions (CMHCs) such as depression and anxiety often co-occur with noncommunicable diseases (NCDs) like hypertension and diabetes, compounding disability and mortality particularly in low- and middle-income countries (LMICs), with under-resourced health systems. This comorbidity is driven by shared behavioral risk factors including stress, isolation, tobacco use, inactivity, poor diet, and nonadherence to treatment. The World Health Organization recommends evidence-based stress reduction (EBSR), behavioral activation (BA), and motivational interviewing (MI)

Extended Sanitization Frequency for Bottles: Using Volume as an Alternative to Time-Scheduled Replacements for Mice in Disposable Individually Ventilated Caging Systems.

The Guide for the Care and Use of Laboratory Animals specifies that sipper tubes require weekly sanitization, although reduced frequency for sterile, disposable caging components may be justified with performance-based assessments; therefore, extending bottle duration beyond 1 week, and replacing instead at a lower volume limit, may be acceptable if it does not compromise mouse health or water quality. Weekly bottle change for sterile, disposable bottles results in excessive waste of both water and plastic-bottles are typically more than half full at 7 days. A volume-based replacement schedule

Clinicians and dyslexia--a computer-based assessment of one of the key cognitive skills involved in drug administration.

AIMS: This research investigates the relationship between dyslexia traits and nurse performance on a laboratory task designed to assess one of the key cognitive skills involved in drug administration. The potential moderating role of perceived performance control was also assessed, based on previous work demonstrating the importance of self-belief as a facilitator of vocational success. BACKGROUND: Dyslexia within the health care professions has been the subject of wide and emotionally charged debate but has not yet been scientifically examined. Those who fear clinicians with dyslexia do so b
