Pharmacoepidemiology of testosterone prescribing in Australia, 2010-2023.

OBJECTIVE: After decades of progressive increase, subsidized testosterone prescribing under the Pharmaceutical Benefits Scheme (PBS) in Australia reduced following tightening of prescribing criteria in 2015. Conversely, testosterone use as gender-affirming hormone therapy (GAHT) for transgender and gender-diverse individuals is increasing. This study aimed to investigate the longer-term impact of the 2015 tighter PBS regulations on testosterone prescribing in Australia and to characterize and quantify the potential expansion of testosterone prescribing for GAHT. DESIGN: Observational analysis

Gamified Cardiopulmonary Rehabilitation in a Child Undergoing LVAD Bridging Therapy to Heart Transplantation: A Case Report.

This report details the comprehensive, gamified cardiopulmonary rehabilitation of a 6-year-old girl undergoing a complex clinical journey from left ventricular assist device (LVAD) bridge therapy to successful heart transplantation. Given the paucity of standardised protocols for this high-risk population, a structured, multidisciplinary team (MDT)-guided pathway was implemented, adapting interventions to the distinct phases of LVAD support and post-transplant recovery. Key nursing and rehabilitation strategies included initiating very early mobilisation in the cardiac intensive care unit (CIC

Association between body composition and disease progression in adults with amyotrophic lateral sclerosis: a cross-sectional study.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder characterized by motor neuron degeneration, muscle wasting, and respiratory failure, with a median survival of 30 months. Due to the strong link between dysphagia, weight loss, and disease progression, this study investigates the relationship between body composition and clinical outcomes in ALS adults. This cross-sectional study involved 93 ALS adults (29 females, 64 males) from Imam Khomeini Hospital in Tehran, selected based on EI Escorial criteria. Researchers assessed body composition, functional abilities, an

The disease burden and health-related quality of life in Chinese children with genetic cholestatic liver diseases: a cross-sectional study of progressive familial intrahepatic cholestasis and Alagille syndrome.

BACKGROUND: Genetic cholestatic liver diseases (GCLDs), including progressive familial intrahepatic cholestasis (PFIC) and Alagille syndrome (ALGS), are rare inherited liver disorders that manifest in early childhood and lead to substantial morbidity and healthcare utilization. However, comprehensive data on their economic and humanistic burden in China are scarce. METHODS: A cross-sectional study was conducted in 2024 among caregivers of GCLD children recruited from a major specialized referral center in China. Data were collected via an online questionnaire covering demographic and clinical

Medical Management of Polycystic Liver Disease: A Position Statement From the European Reference Network on Hepatological Diseases.

Polycystic liver disease (PLD) is a rare genetic disorder characterised by progressive liver enlargement due to multiple cysts. The main symptoms are liver volume-related. Although randomised controlled trials have shown that somatostatin analogues (SSAs) reduce liver volume as well as symptoms, specific guidance on when and how to use SSAs in clinical practice is still lacking. A panel of 15 hepatologists and nephrologists developed practical guidance on SSA use, based on a systematic literature search, expert surveys, and clinical experience. This consensus was reached during a two-day works

Fatal Infantile Cardiomyopathy Associated with a Homozygous MYL2 c.413T>A (p.Met138Lys) Variant: A Case Expanding the Recessive MYL2 Phenotypic Spectrum.

BACKGROUND/OBJECTIVES: Infantile cardiomyopathy is a rare but often life-threatening condition in which monogenic causes are particularly relevant, especially when cardiac disease is preceded by hypotonia or multisystem involvement. Among sarcomeric genes, MYL2, encoding the ventricular regulatory myosin light chain, plays a critical role in myocardial contractility. However, biallelic MYL2-associated disease remains exceptionally rare, and its clinical spectrum is not fully defined. This study aims to describe a novel case and further delineate the phenotype of recessive MYL2-related cardiomy

Case Report: Early-onset euglycemic diabetic ketoacidosis following henagliflozin initiation in a treatment-naive patient.

BACKGROUND: Sodium-glucose cotransporter 2 (SGLT2) inhibitors are associated with euglycemic diabetic ketoacidosis (euDKA), typically occurring after ≥2 weeks of therapy in the setting of intercurrent illness or surgical stress. The risk of early-onset euDKA in treatment-naive patients-those without prior glucose-lowering therapy or established metabolic stability-remains poorly characterized. We describe a case of severe euDKA occurring within 4 days of SGLT2 inhibitor initiation, precipitated solely by metformin-induced gastrointestinal adverse effects. CASE PRESENTATION: A 40-year-old woma

Pediatric hemophagocytic lymphohistiocytosis with predominant CNS involvement: MRI features and differential diagnosis.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory disorder resulting from immune dysregulation that leads to uncontrolled macrophage activation. The diagnosis of HLH is often challenging, as its clinical manifestations can mimic various systemic conditions, including sepsis, autoimmune disorders, and malignancies. This overlap, combined with the rapid progression of the disease, may delay treatment and increase morbidity and mortality. Neurological involvement is particularly frequent in pediatric cases. Neuroimaging plays a critical role in rai

Co-infection of Hepatitis A and E With Atypical Manifestation: A Case Report.

Hepatitis A virus (HAV) and hepatitis E virus (HEV) are common causes of acute viral hepatitis in developing countries where inadequate sanitation and contaminated water supplies facilitate fecal-oral transmission. Although both infections are usually self-limiting, co-infection may result in atypical presentations and pose diagnostic challenges. We report the case of a previously healthy 14-year-old girl who presented with a 5-day history of nausea, vomiting, and anorexia, followed by sudden-onset jaundice and dark urine. She denied fever, abdominal pain, pruritus, bleeding manifestations, or

My Amyotrophic Lateral Sclerosis (ALS) Journey from Weakness to Diagnosis: A Journey of Hope.

Amyotrophic lateral sclerosis (ALS) or Lou Gehrig's disease is a progressive neurodegenerative disease that attacks and kills motor neurons in the brain and spinal cord, leading to muscle weakness and atrophy, eventually causing respiratory failure and death within 2-5 years after diagnosis. By 2040, the global population of individuals living with ALS is projected to approach 400,000. Since ALS was discovered by Charcot 150 years ago, only two drugs (Edaravone and Riluzole) have been available, offering modest clinical benefits in slowing disease progression. The increasing number of cases, a

Clinical profile and outcome of infants with biliary atresia in the era of liver transplantation.

BACKGROUND: Biliary atresia (BA) is a progressive, fibro-inflammatory cholangiopathy of infants with Kasai portoenterostomy (KPE) as the first-line surgical treatment. Most children eventually develop end-stage liver disease requiring liver transplantation (LT). Some infants are treated with primary LT (PLT), but the majority undergo KPE with a subsequent salvage LT (KPSLT). This aim was to study the clinical profile and outcome of infants with BA in the era of LT. METHODS: In this retrospective study from a tertiary care hospital of North India, all infants with BA who presented from January

Frontotemporal dementia: a systematic review of artificial intelligence approaches in differential diagnosis.

INTRODUCTION: Frontotemporal dementia (FTD) is a neurodegenerative disorder characterized by progressive degeneration of the frontal and temporal lobes, leading to significant changes in personality, behavior, and language abilities. Early and accurate differential diagnosis between FTD, its subtypes, and other dementias, such as Alzheimer's disease (AD), is crucial for appropriate treatment planning and patient care. Machine learning (ML) techniques have shown promise in enhancing diagnostic accuracy by identifying complex patterns in clinical and neuroimaging data that are not easily discern

Neurological Mystery Post Travel: An Atypical Presentation and Partial Recovery With Immunotherapy.

This case highlights the diagnostic challenges in atypical neurological presentations and the role of empirical immunotherapy despite the absence of a definitive diagnosis. We present a South Indian woman in her 40s who developed progressive lower limb weakness and respiratory compromise after recent international travel. Initially suspected to have Guillain-Barré syndrome (GBS), extensive investigations, including electromyography (EMG), nerve conduction studies (NCS), and cerebrospinal fluid (CSF) analysis, failed to confirm a clear diagnosis. Despite this uncertainty, empirical intravenous
