Contributions of genetic risk and fetal hypoxia to hippocampal volume in patients with schizophrenia or schizoaffective disorder, their unaffected siblings, and healthy unrelated volunteers.

OBJECTIVE: The authors examined in an epidemiologic sample the contributions of genetic predisposition and history of fetal hypoxia to hippocampal volume in patients with psychosis. METHOD: High-resolution magnetic resonance imaging was used to measure hippocampal volumes in 72 psychotic probands (60 with schizophrenia and 12 with schizoaffective disorder, ascertained so as to be representative of all such probands in a Helsinki birth cohort), 58 nonpsychotic full siblings of the probands, and 53 demographically similar healthy comparison subjects with no family history of psychosis. The longitudinal association between problematic Internet use and psychotic-like experiences in adolescents: Mediated by sleep disturbance and moderated by psychological resilience.

Problematic Internet use (PIU) has been empirically identified as a risk factor for future psychotic-like experiences (PLEs) among adolescents. This study aimed to examine the short-term predictive role of PIU on PLEs in adolescents and investigate their underlying mechanisms, focusing on the mediating role of sleep disturbance and the moderating role of psychological resilience. A total of 27,260 Chinese adolescents (52.2 % males, Meanage = 14.33 ± 1.49) were included for analyses in this two-wave longitudinal study conducted six months apart. The baseline survey (time 1, T1) was between Dece

Ketamine versus ECT for Nonpsychotic Treatment-Resistant Major Depression.

BACKGROUND: Electroconvulsive therapy (ECT) and subanesthetic intravenous ketamine are both currently used for treatment-resistant major depression, but the comparative effectiveness of the two treatments remains uncertain. METHODS: We conducted an open-label, randomized, noninferiority trial involving patients referred to ECT clinics for treatment-resistant major depression. Patients with treatment-resistant major depression without psychosis were recruited and assigned in a 1:1 ratio to receive ketamine or ECT. During an initial 3-week treatment phase, patients received either ECT three tim

The Emergence of Psychiatry: 1650-1850.

Western psychiatry emerged as a medical specialty caring for the mentally ill over the course of the late 18th and early 19th centuries. This emergence was a contingent process, dependent on the co-occurrence of three historical developments that together shaped the young discipline. The first was the rise of the mind as an entity with numerous active faculties in the conceptual space between the body and the Christian soul. Only by the latter half of the 18th century was it common to conceptualize conditions like mania or melancholy as mental illnesses. The second advance critical to psychiat

Persistence of antidepressant use among refugee youth with common mental disorder.

BACKGROUND: The objective of this study was to investigate whether persistence of antidepressant use differs between refugee youth and Swedish-born youth after a diagnosis of a common mental disorder (CMD), and if clinical and sociodemographic factors are associated with antidepressant discontinuation. METHODS: Youth aged 16-25 years, with an incident diagnosis of CMD (depression, post-traumatic stress disorder (PTSD), anxiety disorders) accessing specialized healthcare in Sweden 2006-2016 were included. New users were identified with a one-year washout period. Refugees (N=1575) were compared

A prospective study of the impact of subthreshold mixed states on the 24-month clinical outcomes of bipolar I disorder or schizoaffective disorder.

OBJECTIVES: The clinical significance of subthreshold mixed states is unclear. This study investigated the clinical outcomes in participants with bipolar I disorder or schizoaffective disorder, using the Cassidy and Benazzi criteria for manic and depressive mixed states, respectively. METHODS: Participants (N=239) in a prospective observational study of treatment and outcomes in bipolar I or schizoaffective disorder, bipolar type, were grouped based on study entry clinical presentation as having pure depression (n=63) if they satisfied DSM-IV-TR criteria for a Major Depressive Episode (MDE),

Are schizophreniform symptoms present in attenuated form in children with Tourette disorder and other developmental disorders.

Childhood schizophrenia is an increasingly difficult disorder to identify. We review the DSM-III criteria for schizophrenia and raise the possibility that schizophreniform symptoms may be present in children with Tourette disorder to identify. We review the DSM-III criteria for seen in Tourette disorder that resemble the symptoms of schizophrenia are discussed and data from selected patients are presented. This data is presented to assist clinicians in making more appropriate diagnoses (that is, avoiding the incorrect diagnosis of schizophrenia) and in refining distinctions between the symptom

[Epidemiology of depression].

The enormous variation in values quoted in the literature concerning the occurrence of depressive diseases is caused by the lack of standardized diagnostic criteria. The difficulties of epidemiological research arise from nosological differentiation within the range of depressive diseases, from case identification and case definition. Further problems result from the limiting definitions of depressive disturbances, in respect to variations within the healthy condition on the one hand, and to signs and symptoms of other diseases, on the other hand. Moreover, it is not clear from most of the rel

Hallucinations in children with conduct and emotional disorders: II. The follow-up study.

In a controlled study, 20 children with hallucinations and emotional or conduct disorders were followed up into adulthood. The mean follow-up time was 17 years and the mean age at follow-up was 30 years. Hallucinations in childhood did not carry an increased risk for psychoses, depressive illness, organic brain damage or other psychiatric disorders. The continuation of hallucinations and episodes of altered awareness in some of the subjects may indicate a special predisposition to hallucinate.

A clinical report of thymoleptic-responsive atypical paranoid psychoses.

The paranoid disorders of five patients remitted after treatment with thymoleptic agents despite the absence of a full affective syndrome. Their clinical symptoms at admission included suspiciousness, hypervigilance, ideas of reference, hostility, belligerence, and delusions of persecution or of infidelity. Some had phasic disturbances with somatic complaints or prominent phobic anxiety symptoms, a family history of affective disorder, or prior responses to thymoleptic drugs. These observations suggest that some patients with paranoid psychosis who do not have a full DSM-III affective syndrome

The major psychoses and neuroses as omega-3 essential fatty acid deficiency syndrome: substrate pellagra.

Pellagra was once a major cause of three behaviorally different mental disorders-schizophreniform, manic-depressive-like, and phobic neurotic - plus drying dermatoses, autonomic neuropathies, tinnitus, and fatigue. In this preliminary study all three of the corresponding present-day mental diseases are found to exhibit, statistically, the same pellagraform physical disorders but to ameliorate not so much with vitamins as with supplements of a newly discovered trace omega-3 essential fatty acid (w3-EFA), which provides the substrate upon which niacin and other B vitamin holoenzymes act uniquely

A comparison of DSM-II and DSM-III in the diagnosis of childhood psychiatric disorders. II. Interrater agreement.

A case-history format was utilized to compare interrater agreement on childhood and adolescent psychiatric disorders, using DSM-II and DSM-III. The average interrater agreement was 57% for DSM-II and 54% for axis I (clinical psychiatric syndrome) of DSM-III. There was high agreement in both systems on cases of psychosis, conduct disorder, hyperactivity, and mental retardation, with DSM-III appearing slightly better. There was noteworthy interrater disagreement in both systems for "anxiety" disorders, complex cases, and in the subtyping of depression. Overall, the reliability of DSM-III appears

Psychopharmacological perspectives in childhood psychoses.

1. Neuroleptic drugs are still widely employed in childhood psychoses, but new experiences on a large number of cases in this field, have shown the usefullness of antidepressive drugs, either alone or associated with neuroleptics, also when depressive symptoms are not clearly evident. 2. This peculiar aspect, still noticed in pathological situations of less gravity, calls again attention to childhood depression and to the difficulty of identifying it. 3. The problem is developed and discussed as follows: a. The psychopathological features regarded from the psychodynamic point of view; b. The n

Psychiatric disturbances in drunk driving offenders referred for treatment of alcoholism.

The incidence of psychopathology in persons arrested for driving while intoxicated (DWI) has serious implications in both alcoholism treatment and in general highway safety. This study investigated the types of psychopathology, as measured by the Minnesota Multiphasic Personality Inventory (MMPI), in a sample of 116 males referred for treatment by a court diversion program.

Psychiatric hospital treament of suicidal children.

Three phases of psychiatric hospital treatment of children, ages 6 to 12 years, admitted to the hospital because of suicidal threats or suicidal attempts are described. Suicidal behavior in young children must always be taken seriously. Although suicidal behavior usually ceases upon hospitalization, suicidal ideas may continue to be expressed in play fantasy. In addition to individual and milieu therapy, treatment must also include concomitant assistance of the parents. The treatment procedures are illustrated by suicidal case examples of a depressed girl, an acting-out boy, and a psychotic bo

Evaluating the PANSS using item response theory in forensic psychiatric samples from five European nations.

Item Response Theory (IRT) describes a set of statistical models describing how individual items in a test or questionnaire relate to the underlying characteristic or trait that the test claims to measure. Until now IRT models have not been applied to the Positive and Negative Syndrome Scale (PANSS) in forensic and general psychiatric samples to establish its psychometric properties and explore the link between psychotic symptom severity and violent behavior in schizophrenia. This study investigated patients with schizophrenia spectrum disorders and a history of violence from forensic institut
