We report a neonate resuscitated from sudden death with a Brugada ECG pattern several days after the initial event. The baby was subsequently diagnosed with a fatty acid oxidation disorder (medium chain Acyl-CoA dehydrogenase deficiency or MCAD). We speculate that free fatty acid accumulation can give rise to the Brugada ECG pattern.

Thoracic great vessel injury may be secondary to blunt, penetrating, blast, or iatrogenic trauma. A surgeon should be the initial evaluator of and decision maker for these patients, and the aortogram remains the gold standard for specific diagnosis of the arterial injuries except in those patients requiring emergency thoracotomy. Two general types of incisions are employed for these injuries: resuscitative and elective.

Difficulty in resuscitating a neonate using bag and mask ventilation was followed by later episodes of apnoea and cyanosis. Intubation relieved the respiratory failure and revealed a large fleshy polyp obstructing the posterior pharynx. Computerised tomography showed there was no superior extension of the lesion and the polyp was removed completely on operation. Histological examination showed this lesion to be a dermoid or "Hairy Polyp".

The assessment and treatment of patients requiring cardiopulmonary resuscitation (CPR) is discussed. Cardiovascular pharmacotherapy is discussed as suppressive therapy, stimulant therapy and treatment of hypotension. Special situations covered include hyperkalemia, anaphylaxis and drug overdosages. It is concluded that the likelihood of success of CPR depends on the rapid initiation of ventilation and restoration of circulation, and is inversely dependent on the severity of the underlying disease or injury.

OBJECTIVES: We report a case of a bronchovenous fistula in an adult who could not be resuscitated following a mitral valve replacement.
METHODS: A 39-year-old man underwent a mitral valve replacement for severe mitral stenosis and regurgitation. Following uneventful valve replacement surgery, and while attempting to wean the patient off bypass, we encounted a bronchovenous fistula following mechanical ventilation.

Two cases of traumatic rupture of the basilar artery are reported. In the first case, severe basal subarachnoid hemorrhage (SAH) due to a complete transverse tear of the basilar artery was observed in a 53-year-old restrained male driver who was involved in a head-on collision while intoxicated and drowsy.
