Effects of Meal Consumption on Isotonic Lingual Endurance in Healthy Adults: A Multi-Institutional Study.

OBJECTIVE: This study investigated the impact of meal consumption on isotonic lingual endurance in a cohort of adults across the age span without dysphagia (swallowing impairment). METHODS: Participants at three academic centres completed pre- and post-meal lingual pressure tasks. Isotonic lingual endurance was assessed using the Iowa Oral Performance Instrument (IOPI) at 50% of maximum lingual pressure (Pmax) Participants consumed a standardised meal (bagel with peanut butter, carrots, and chocolate milk) between assessments. Mixed-model MANOVAs evaluated endurance changes across meal condit

Psychometric and structural properties of Traditional Chinese version of postoperative oral dysfunction-10 in patients with oral cancer undergoing facial free flap reconstruction surgery.

PURPOSE: Dysphagia is a frequent complication in oral cancer patients after free-flap reconstruction. Although the Postoperative Oral Dysfunction-10 (POD-10) is widely used to assess swallowing dysfunction, no validated Traditional Chinese version exists for Mandarin-speaking Taiwanese patients. This study aimed to translate the POD-10 into Traditional Chinese (TC-POD-10) and evaluate its reliability and validity in Mandarin-speaking Taiwanese patients with oral cancer undergoing free-flap reconstruction. METHODS: A cross-sectional study was conducted with 100 oral cancer patients post free-f

Chemoradiation (CCRT) Effects on Head and Neck Muscles: Insights Into CCRT-Induced Dysphagia.

OBJECTIVES/HYPOTHESIS: Concurrent chemoradiotherapy (CCRT) is an effective treatment for various head and neck cancers; however, CCRT can result in significant swallowing impairment, presumably due largely to dysfunction of the muscles of swallowing, which are within the field of radiation. This translational study examines the mechanisms underlying CCRT-induced muscle dysfunction using (1) a rat model of CCRT and (2) analysis of archival human specimens. METHODS: 131 Sprague Dawley rats were divided into experimental and control groups. Experimental rats received cisplatin and 5-fluorouracil

Oral Intake Recovery in Non-Intubated Patients With COVID-19 Pneumonia Compared With Non-COVID Pneumonia: A Retrospective Cohort Study Using the Functional Oral Intake Scale.

Dysphagia is a complication of coronavirus disease 2019 (COVID-19), often attributed to intubation and critical illness. However, the recovery trajectory in non-intubated patients with COVID-19 compared with that in patients with other pneumonia types remains unclear. This retrospective cohort study included non-intubated adults hospitalized with pneumonia between May 2022 and December 2023, comparing COVID-19 and non-COVID pneumonia patients regarding time to functional oral intake (Functional Oral Intake Scale [FOIS] ≥ 4) and longitudinal FOIS changes. Two Cox proportional hazards models wer

Observed changes in clinical proxy measures of airway protection after cervical epidural spinal cord stimulation in neurological disorders: a preliminary retrospective case series.

OBJECTIVES: Although spinal cord stimulation (SCS) has been investigated for cough and respiratory motor output after neurological injury, its relationship with clinical proxy measures of airway protection remains unclear. We explored observed changes in aspects of airway protection after cervical SCS in patients with neurological disorders. MATERIALS AND METHODS: We retrospectively analyzed six patients who underwent cervical SCS for spinal cord injury, amyotrophic lateral sclerosis, or disorders of consciousness. Clinical proxy measures included the Semi-quantitative Cough Strength Score (S

Stridor and Dysphagia Unmasking an Aberrant Right Subclavian Artery in a Toddler.

CASE PRESENTATION: A one-year-old girl presented with acute-onset dyspnea persistent for two days and recurrent choking episodes. Physical examination revealed stridor and sub-costal retractions. Despite initial treatment with inhaled nebulized epinephrine, stridor persisted. Lateral chest radiography finding indicated tracheal stenosis. Contrast-enhanced neck and chest computed tomography (CT) showed an aberrant right subclavian artery compressing the esophagus posteriorly, causing esophageal stenosis with associated food residue accumulation. The proximally dilated portion of the esophagus c

Aspiration-Related Alveolar Disease: From Acute Injury to Chronic Pulmonary Sequelae.

Aspiration-related lung diseases comprise a broad spectrum of disorders resulting from the entry of oropharyngeal or gastric contents into the lower respiratory tract. Clinical manifestations range from acute aspiration pneumonitis and aspiration pneumonia to chronic airway and parenchymal injury, including bronchiolitis, bronchiectasis, fibrosis, and lipoid pneumonia. Aspiration pneumonitis is driven primarily by chemical injury, whereas aspiration pneumonia results from infection caused by aspirated microorganisms in susceptible hosts. Contemporary microbiologic data indicate that aspiration

Concurrent Use of Videofluoroscopic and Nutritional Intake Measures in the Assessment of Pediatric Patients With Dysphagia: A Scoping Review.

INTRODUCTION: The videofluoroscopic swallowing study (VFSS) is a radiographic procedure that assesses swallowing physiology in real time by visualizing bolus flow in coordination with structural movement throughout the upper aerodigestive tract. Nutritional intake measures are intended to identify restrictions placed on a patient's method of nutritional intake. There is a lack of rigorous evidence to inform recommendations about appropriate and adequate means of nutritional intake. The purpose of this scoping review was to explore the type and validity of nutritional intake measures and how th

A meaningful prediction of functional decline in amyotrophic lateral sclerosis based on multi-event survival analysis.

Amyotrophic lateral sclerosis (ALS) is a degenerative disorder of the motor neurons that causes progressive paralysis in patients. Current treatment options aim to prolong survival and improve quality of life. However, due to the heterogeneity of the disease, it is often difficult to determine the optimal time for potential therapies or medical interventions. In this study, we propose a novel method to predict the time until a patient with ALS experiences significant functional impairment (ALSFRS-R ≤ 2) for each of five common functions: speaking, swallowing, handwriting, walking, and breathin

Primary Peristalsis Is the Dominant Mechanism of Refluxate Clearance Following Gastroesophageal Reflux.

BACKGROUND: Refluxate volume and pH drop following gastroesophageal reflux are mostly cleared by peristalsis. We evaluated the roles of primary volume clearing peristaltic wave (VCPW), secondary VCPW, post-reflux swallow-induced peristaltic wave (PSPW), and late primary peristaltic wave (LPPW) in refluxate clearance. METHODS: We retrospectively analyzed pH-impedance studies performed off therapy in 40 patients with typical esophageal symptoms. Mechanism of refluxate clearance was evaluated for each reflux episode (primary VCPW vs. secondary VCPW vs. none), as well as presence of PSPW, LPPW wh

Magnetic resonance imaging of masticatory muscles in patients with duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) is the most common muscular dystrophy in children. Impairment of masticatory function and swallowing disorders, potentially leading to aspiration and gastrostomy, are linked to fatty infiltration in the masticatory muscles, as previously observed in muscle ultrasound. This study aims to quantify muscle volume and fat fraction in muscle magnetic resonance imaging (MRI) in the masticatory muscles in non-ambulant DMD patients compared to healthy controls and evaluate their correlation with maximum bite force (MBF), which has not been previously described. Fifteen

Feasibility of Endoscopic Evaluations of Laryngeal Function After Extubation for Research in Critically Ill Adults.

PURPOSE: Flexible nasal laryngoscopy (FNL) is commonly used in intensive care units (ICUs) to assess laryngeal anatomy and function after endotracheal intubation. The use of FNL in research may differ from clinical practice due to requirements of research protocols and related data collection and documentation. This study evaluates the feasibility of performing FNL post-extubation in critically ill adults within the context of a research protocol. Understanding the feasibility of this reference standard assessment is important for supporting future studies and improving patient care. METHOD:

Association of the Oral Frailty Index-8 With the Diagnosis and Results of Oral Hypofunction Testing.

BACKGROUND: Oral frailty is associated with physical frailty and the risk of requiring nursing care and death in older people. The Oral Frailty Index-8 (OFI-8) was developed for self-assessment of oral frailty, assuming that high-risk patients will visit a dentist for oral hypofunction tests. However, the screening utility of the OFI-8 for oral hypofunction and the relationship between them remain unclear. OBJECTIVE: In this study, we aimed to elucidate the relationship between the OFI-8 and the diagnosis and results of oral hypofunction tests through a cross-sectional survey of older outpati
