When balloon angioplasty of the pulmonary valve was attempted in 42 children, aged 1 day to 19 years (mean, 3.1 years), immediate relief of pulmonary stenosis was achieved in 37 cases. In two groups, angioplasty was not uniformly successful: these included (1) two neonates and (2) three out of seven children with dysplastic pulmonary valves, who had an unsuccessful early result. An operation utilizing the ductus arteriosus to construct an outflow tract from the right ventricle is described. This operation uses the right ventricle for pulsatile flow to both systemic and pulmonary arteries, provides complete reconstruction of the ascending aorta and arch, and delivers predictable pulmonary blood flow. Although unsuccessful in this case report, which involves a hypoplastic left heart syndrome patient, the technique may provide successful palliation with the use of ductal tissue. This method has not been previously described.

Lack of donor heart availability complicates the management of terminally ill patients who are candidates for cardiac replacement. The total artificial heart has been used as a bridge to transplantation in three patients with terminal cardiomyopathy. Acute allograft rejection and the lack of another donor heart prompted us to use the mechanical heart as a bridge to re-transplantation in a 33-year-old man. We report a case of sinus node reentry, an uncommon form of supraventricular tachycardia in a patient with no organic heart disease. This tachycardia appeared at times with wide QRS complexes, which made the differentiation between supraventricular and ventricular tachycardia somewhat difficult. The purpose of this report is to discuss features of sinoatrial node reentry tachycardia, both on surface electrocardiography and intracardiac electrography. Electrophysiologic studies are usually required to confirm the diagnosis.

Severe calcification of the mitral annulus presents a technical challenge in valve replacement, and in the case reported here, we replaced the valve with a new technique. Employing a rim of tissue created from the supra-annular left atrium imbricated to the edge of the posterior mitral leaflet, we created a substitute "annulus" for prosthetic valve fixation.

Patients with chronic atrial fibrillation, giant left atrium, left atrial thrombi, and previous embolic accidents are usually treated with life-long anticoagulation after bioprosthetic mitral valve replacement for fear of increased thromboembolic accidents. Clinically significant methemoglobinemia is rare and difficult to diagnose when other causes of cyanosis are likely. We report a patient in whom unstable angina pectoris and chronic obstructive pulmonary disease were assumed to be responsible for preoperative cyanosis. The use of cardiopulmonary bypass for aortocoronary grafting enabled the clinical diagnosis of methemoglobinemia to be made.

The presence of cyanosis following repair of congenital cardiac defects may result from several different mechanisms. We report two patients in whom early postoperative arterial hypoxemia manifested by cyanosis was caused by right-to-left interatrial shunting. Two-dimensional contrast echocardiography correctly identified the site and direction of shunting, leading to surgical correction of the complication.

To study the long-term results of tricuspid valvuloplasty, pre- and postoperatively (39 +/- 11 months), echocardiographic and catheterization data were collected in 37 patients (mean age, 58 +/- 8 years). A case is reported of a right aortic arch vascular ring composed of a right aortic arch with mirror image branches and a retroesophageal left aortic diverticulum with a left ductus arteriosus that appeared as a possible foreign body aspiration in an infant. This vascular ring caused partial occlusion of the left main bronchus with air trapping. A discussion of right aortic arch vascular rings and vascular anomalies that have caused unilateral lung hyperinflation is included in this report.

During a period of 6 years and 5 months, a group of 26 men and 16 women between 53 and 80 years of age underwent combined coronary artery bypass and mitral valve replacement. UNLABELLED: Idiopathic mitral valve prolapse (IMVP) is a very common cardiac abnormality that may be linked to carnitine deficit (inadequate nutritional intake or absorption). Giant lymph node hyperplasia is a rare, benign disease involving lymph nodes in various locations, predominantly in the mediastinum. There are two variants: plasma cell (earlier and/or acute) and hyaline-vascular, more chronic with an intermediate transitional type. We recently encountered a premature infant with a very unusual form of vascular ring consisting of a double aortic arch with atretic left segment and patent ductus arteriosus.
