Bessel beam optical coherence microscopy enables multiscale assessment of cerebrovascular network morphology and function.

Understanding the morphology and function of large-scale cerebrovascular networks is crucial for studying brain health and disease. However, reconciling the demands for imaging on a broad scale with the precision of high-resolution volumetric microscopy has been a persistent challenge. In this study, we introduce Bessel beam optical coherence microscopy with an extended focus to capture the full cortical vascular hierarchy in mice over 1000 × 1000 × 360 μm3 field-of-view at capillary level resolution. The post-processing pipeline leverages a supervised deep learning approach for precise 3D seg

Optical coherence elastography for evaluating customized riboflavin/UV-A corneal collagen crosslinking.

UV-induced collagen cross-linking is a promising treatment for keratoconus that stiffens corneal tissue and prevents further degeneration. Since keratoconus is generally localized, the efficacy of collagen cross-linking (CXL) treatments could be improved by stiffening only the weakened parts of the cornea. Here, we demonstrate that optical coherence elastography (OCE) can spatially resolve transverse variations in corneal stiffness. A short duration ( ? 1 ?? ms ) focused air-pulse induced low amplitude ( ? 10 ?? ? m ) deformations in the samples that were detected using a phase-stabilized opti

Enhancing Interpretability in Retinal OCT Analysis Using Multi-Channel Feature Encoding.

This study examines the training of lightweight classifiers, specifically ResNet-18 and DenseNet-121, for retinal analysis in optical coherence tomography. Multi-channel inputs were compared against standard grayscale-to-RGB copies, increasing the reliability of the results. While RGB replication is standard for transfer learning, the multi-channel approach enhances feature extraction. Clinical reliability was further validated via Integrated Gradients to audit decision boundaries. This work confirms that specialized input channels help create more interpretable and reliable clinical decision-

Editor's Highlights-August 2026.

Noninvasive imaging techniques are playing an increasingly significant role in dermatologic diagnosis, risk evaluation, and the monitoring of therapeutic outcomes. In the August issue of the Journal, we highlight how dermoscopy, onychoscopy, trichoscopy, and line-field confocal optical coherence tomography (LC-OCT) can refine clinical decision-making across acral melanocytic lesions, onycholysis, primary cicatricial alopecia, and hyperkeratotic actinic keratoses. These papers show that imaging is most useful when tailored to anatomical and clinical context, advancing standardized and clinicall

Acute Posterior Multifocal Placoid Pigment Epitheliopathy: Clinical and Iconographical Aspects of Two Cases.

Acute posterior multifocal placoid pigment epitheliopathy (APMPPE) is a rare inflammatory disorder of the choriocapillaris and retinal pigment epithelium. We report two atypical cases: one with unilateral involvement complicated by serous retinal detachment requiring corticosteroid therapy; the second with bilateral involvement that resolved spontaneously. These observations underscore the clinical heterogeneity of APMPPE and the pivotal role of multimodal imaging, including optical coherence tomography (OCT) and OCT-angiography (OCT-A), in elucidating its pathophysiology, guiding diagnosis, m

[Intraoperative OCT during cataract surgery].

Intraoperative Optical Coherence Tomography (OCT) is a promising tool in cataract surgery. This article explores available devices for intraoperative OCT and their practical applications in this surgical procedure. It highlights the potential benefits of this technology, particularly in evaluating incisions, detecting residual fragments, and confirming Descemet's membrane detachments. Despite its advantages, intraoperative OCT faces practical challenges, such as cost and the need for a learning curve. However, its judicious use could contribute to improving the quality and safety of cataract s

Gliotic tissue simulating a macular neovascularization in full-thickness macular hole.

The authors report a case of a male patient affected by macular hole. In particular, a hyperreflective tissue was found on optical coherence tomography (OCT) examination in macular region, just above the retinal pigment epithelium (RPE). OCT angiography (OCTA) did not show the presence of vascular tissue, thus the hyperreflective material was ascribed to primary gliotic tissue. This case highlights the ability for Müller cells placed near macular holes to migrate up to the RPE and to produce gliotic tissue.

[Persistent foveal retinal detachment after successful rhegmatogenous retinal detachment surgery by scleral buckling].

We report the case of a phakic patient with late visual recovery after successful surgery of macula-off retinal detachment, using the scleral buckling procedure. Visual acuity remained low because of the persistence of subfoveal fluid despite the reattachment of peripheral retina. The amount of subfoveal fluid decreased slowly and final visual acuity finally recovered. We used optical coherence tomography (OCT) to quantify the remaining subfoveal fluid. Persistence of submacular fluid can explain late visual recovery after surgical treatment of retinal detachment.

Impact of Changes in the Optical Density of Postlens Fluid on the Clinical Performance of Miniscleral Lenses.

PURPOSE: To determine the impact of optical density changes in postlens fluid on clinical performance and to quantify these changes over time during miniscleral lens wear. METHODS: Twenty-three eyes of 13 patients with keratoconus were fitted with a miniscleral lens (scleral Misa lenses; Microlens Contactlens Technology, Arnhem, Netherlands). The lens fit was evaluated using the fluorescein pattern and also through anterior segment optical coherence tomography imaging. The optical density changes were measured using Scheimpflug tomography at 30 min, 1, 2, 3, and 4 hr. High- and low-contrast v

Evaluation of Spatially Targeted Scleral Stiffening on Neuroprotection in a Rat Model of Glaucoma.

PURPOSE: Scleral stiffening may protect against glaucomatous retinal ganglion cell (RGC) loss or dysfunction associated with ocular hypertension. Here, we assess the potential neuroprotective effects of two treatments designed to stiffen either the entire posterior sclera or only the sclera adjacent to the peripapillary sclera in an experimental model of glaucoma. METHODS: Rat sclerae were stiffened in vivo using either genipin (crosslinking the entire posterior sclera) or a regionally selective photosensitizer, methylene blue (stiffening only the juxtaperipapillary region surrounding the opt

The New Era of Retinal Imaging in Hypertensive Patients.

Structural and functional alterations in the microcirculation by systemic hypertension can cause significant organ damage at the eye, heart, brain, and kidneys. As the retina is the only tissue in the body that allows direct imaging of small vessels, the relationship of hypertensive retinopathy signs with development of disease states in other organs have been extensively studied; large-scale epidemiological studies using fundus photography and advanced semi-automated analysis software have reported the association of retinopathy signs with hypertensive end-organ damage includes the following:

Polypoidal choroidal vasculopathy as a complication of choroidal osteoma: A case report.

INTRODUCTION: Choroidal osteoma (CO) is a rare benign tumor that particularly affects young, healthy women. Its prognosis is influenced by complications, such as choroidal neovascularization (CNV), subretinal hemorrhage, subretinal fluid (SF), decalcification status, and overlying retinal pigment epithelium (RPE) atrophy. In case of CNV as the complication of CO, it is typically present in the classic form; however, reports on polypoidal choroidal vasculopathy (PCV) have been rare. Here, we report a case of an older, male patient with PCV as a complication of CO. PATIENT CONCERNS: A 70-year-o

Retinal biomarkers for visual prognosis in pituitary surgery: a narrative review.

BACKGROUND: Pituitary macroadenomas frequently cause visual impairment through compression of the optic chiasm, and endoscopic transsphenoidal surgery remains the standard of care for achieving decompression. Despite favorable surgical outcomes in many patients, visual recovery following chiasmal decompression is heterogeneous and difficult to predict using conventional visual field testing alone. Over the past two decades, optical coherence tomography (OCT) has emerged as a noninvasive retinal imaging modality capable of quantifying structural changes in the retinal nerve fiber layer (RNFL) a

Trans-Synaptic Retrograde Retinal Degeneration After Post-Chiasmatic Stroke: A Systematic Review of Empirical Evidence.

BACKGROUND: Retrograde trans-synaptic degeneration (RTSD) refers to the progressive loss of retinal cells following damage to the post-geniculate visual pathway. The advent of optical coherence tomography (OCT) has allowed the measurement and quantification of retinal layers, in turn providing compelling evidence of RTSD in stroke patients and highlighting that assessing the temporal and functional evolution of RTSD has important clinical implications. METHODS: We conducted a systematic review up until 7th July 2025 to summarize the OCT evidence on post-stroke RTSD. Eligible articles on this
