[The establishment of the hospital-system in the Byzantine Empire].

Byzantine hospitals developed out of Christian institutions for the poor and homeless. Philanthropy provided the initial impulse to create hospices (xenons) and to expand these institutions into specialized medical centers (iatreons or nosokomeions). However the Byzantine nosocomeions resemble more closely modern hospitals than they do any of the institutions of Greek-Roman antiquity or any of the houses of charity in the Latin West during the Middle Ages. Since the 4th century the Byzantine hospitals have stressed the central position of the nosocomeion in Byzantine society at the intersectio

Comparison of kyphoplasty with use of a calcium phosphate cement and non-operative therapy in patients with traumatic non-osteoporotic vertebral fractures.

One of the current standard treatment options for younger patients with stable traumatic vertebral fractures is conservative treatment using braces. Kyphoplasty as a minimally invasive procedure has been shown to be effective in stabilizing vertebral body fractures, resulting in immediate pain relief and improved physical function. The purpose of this prospective study was to clarify whether patients with acute traumatic vertebral fractures benefit more from kyphoplasty or from conservative treatment with a brace. A prospective study was undertaken in two centers. Forty patients with acute pai

Emergency department care for patients with hemophilia and von Willebrand disease.

Patients with bleeding disorders such as hemophilia A, hemophilia B, and von Willebrand disease (VWD) are routinely treated at home, with their care managed in specialized centers. In emergency situations, these patients often present to their local emergency department (ED), where their management can represent a challenge to the emergency physicians and staff who rarely encounter them. Delays in diagnosis and administration of replacement therapy are the factors most commonly identified as predictive of death. Patients and family members are often very well educated in the disease and its ma

Stress and incidence of bleeding in children and adolescents with hemophilia.

OBJECTIVE: To examine the relationship of stress and incidence of bleeding in boys with hemophilia. STUDY DESIGN: We conducted a 6-month longitudinal study of 97 subjects (ages 4 to 16 years) from six hemophilia centers. Diaries recorded bleeding episodes (including site and history of previous trauma) and both child and parent daily stress. Parent and child stressful life event measures were obtained monthly. Socioeconomic data and clotting factor level were determined at enrollment. Logistic regression models examined the influence of recent stress on likelihood of bleeding on each day, con

Advancing child health through applied qualitative research.

Despite a decrease in child mortality since the establishment of the Millennium Development Goals, children and young people (CYP) continue to face threats to their health and well-being, including acute and chronic diseases, mental illness, and trauma-related injury. Qualitative research is underutilized in certain contexts, yet it is an essential methodology to advance child health outcomes. This paper presents recommendations generated by multidisciplinary experts at the Symposium on Applied Qualitative Research, outlining a value proposition for qualitative inquiry to illuminate the comple

Beyond the Burn: The Long-Term Effects of Pain, Body Image, and Physical Function in Pediatric Rehabilitation: A Burn Model System Study.

Childhood burn injuries can lead to physical and psychosocial challenges that linger well beyond the initial phases of the trauma. This study explores the interrelated roles of pain, body image, and physical function in children living with burn injuries using data from the Burn Model System National Longitudinal Database. A cohort of 110 children aged 8-17 was assessed 6 months postinjury. Descriptive and regression analyses revealed that, on average, pediatric burn survivors reported significantly lower pain interference compared with the general pediatric population. However, higher pain in

Menopause and the Social Determinants of Health.

This clinical perspective examines how social determinants of health (SDoH) such as economic stability, education and health literacy, health care access and quality, and social and community contexts shape menopause experiences and outcomes through a health equity lens. Menopause is not experienced uniformly: Black, immigrant, and LGBTQ+ (lesbian, gay, bisexual, transgender, queer+) individuals and those with low income often face earlier symptom onset, greater symptom burden, and reduced access to evidence-based care due to structural inequities, economic barriers, and cultural stigma. Menop

Case Report: Palmaris Brevis Syndrome Resulting From Acute Wrist Hyperextension.

A 24-year-old, 19-week pregnant female presented to a community hospital emergency department following an acute hyperextension injury to the left wrist. The pertinent findings on the initial examination included pain, swelling of the ulnar aspect of her palm, and decreased, painful flexion of her small finger. The emergency room physician suspected a flexor tendon injury, and urgent referral to a tertiary level plastic surgery center was made. After plastic surgery evaluation, it was determined that her symptoms were related to the ulnar nerve opposed to an acute tendon injury. After ruling o

Management of an Adult Open Diaphyseal Both-Bone Forearm Fracture With Severe Bone Loss: A Case Report, Literature Review, and Surgical Techniques.

Open forearm fractures involving both the radius and ulna are challenging orthopedic injuries, typically resulting from high-energy impacts. These injuries require complex surgical interventions, such as open reduction and internal fixation (ORIF) or intramedullary nailing, and often necessitate multiple approaches due to the proximity of neurovascular structures. Severe cases can lead to complications such as infection, nonunion, and functional impairment of the limb. Here, we present the case of a 42-year-old male who sustained multiple injuries in a motor vehicle accident, including an open

[Extracorporeal membrane oxygenation (ECMO) in severe acute respiratory distress syndrome due to blunt chest trauma: A case report].

We report a case of a patient with chest trauma (TT) complicated with severe acute respiratory distress syndrome (ARDS) who required extracorporeal membrane oxygenation (ECMO) as ventilatory support. Clinical case: A 48-year-old man, with no relevant history, after a high-energy vehicle collision admitted with severe TT. He evolved with respiratory failure that required intubation and connection to mechanical ventilation (MV). He persisted with high ventilatory requirements, requiring rescue ECMO and transfer to our center. He evolved with gasometric and ventilatory improvement that allowed di

A FoxA2+ long-term stem cell population is necessary for growth plate cartilage regeneration after injury.

Longitudinal bone growth, achieved through endochondral ossification, is accomplished by a cartilaginous structure, the physis or growth plate, comprised of morphologically distinct zones related to chondrocyte function: resting, proliferating and hypertrophic zones. The resting zone is a stem cell-rich region that gives rise to the growth plate, and exhibits regenerative capabilities in response to injury. We discovered a FoxA2+group of long-term skeletal stem cells, situated at the top of resting zone, adjacent the secondary ossification center, distinct from the previously characterized PTH
